Stump appendicitis after laparoscopic appendectomy: case report.

Stump appendicitis is a rare delayed complication of appendectomy. The delay in diagnosis is usually because of a prior history of appendectomy. We report a case of stump appendicitis diagnosed pre-operatively with a computerized tomography (CT) scan after laparoscopic appendectomy. An 18-year-old male presented with a one-week history of lower abdominal pain, nausea and vomiting. He had a history of laparoscopic appendectomy for acute appendicitis. Physical examination revealed tenderness and guarding in the lower abdomen. CT scan showed free pelvic fluid with a tubular structure of about 2.5

Distally based superficial sural artery flap for foot and ankle reconstruction in children.

BACKGROUND: Wheel spoke injury of the ankle and foot is very common in children and its reconstruction is challenging. Reverse sural artery flap is very useful to cover these defects. Free tissue transfer is an option in children which needs a micro-vascular expertise, expensive equipment and long operating time. METHODS: Sixteen cases with ankle and foot defects were operated during a period of 18 months at the Children Hospital and Institute of Child Health Lahore. The efficacy of sural artery flap for the coverage of these defects was evaluated. RESULTS: Sixteen children presented with de

Two for the price of one: recycling a free flap.

Free flaps are now an established, successful, method of reconstruction for complex soft tissue defects. Improvements in microsurgical techniques, anatomical understanding and instruments have resulted in the evolution of free flap surgery to now encompass synchronous double free flap surgery and "free style" type flaps based on perforators. Although there have been reports of a single free flap split in two to reconstruct a defect at a single site, we report what we believe to be the first case of a single free groin flap used to reconstruct two distant and separate defects sequentially.

Early postoperative detection of tissue necrosis in amputation stumps with indocyanine green fluorescence angiography.

OBJECTIVE: Amputations of the lower extremity due to irreversible ischemic tissue loss are performed as distally as possible. Therefore, oftentimes wound-healing disorders develop, requiring additional surgical treatment. METHODS: The amputations stumps of 10 patients with irreversible ischemic tissue loss due to arteriosclerosis were investigated within 72 hours postoperatively with indocyanine green (ICG) fluorescence. RESULTS: For 6 of the investigated stumps, no perfusion deficit could be seen through fluorescence angiography. All stumps displayed primary healing. In the fluorescence ang

A novel technique for transtibial amputation in chronic occlusive arterial disease: Modified Burgess procedure.

The aim of this study was to compare results of transtibial amputations performed using the Burgess or Modified Burgess techniques with those performed using the Bruckner technique. Transtibial amputation (TTA) was performed in 117 patients with Fontaine phase IV chronic arterial occlusion. Fifty-six patients with a mean age of 51.4 years were amputated by the Burgess technique between March 2000 and January 2003; and 61 patients with a mean age of 47.8 years were amputated by Modified Burgess technique between February 2003 and March 2006. In the Burgess technique, all muscles in the amputati

Enhancing limb salvage by non-mobilized peripheral blood angiogenic cell precursors therapy in patients with critical limb ischemia.

BACKGROUND: Stem cell therapy has been proposed to enhance the salvage of critically ischemic limbs. OBJECTIVE: Assess the efficacy and safety of the implantation of non-mobilized peripheral blood angiogenic cell precursors (NMPB-ACPs) in patients with critical limb ischemia (CLI) who were poor candidates for standard revascularization treatment options. MATERIAL AND METHOD: Six patients with CLI due to the infrapopliteal artery occlusive disease were included in the present study. Intramuscular injections of NMPB-ACPs were administered in the ischemic limbs. The efficacy was evaluated by cl

Vacuum-assisted closure for bilateral severe ischemic foot after revascularization: a case report.

Vacuum-assisted closure (VAC) therapy is a unique system that helps promote wound healing. We report a case of severe ischemic foot in which VAC therapy markedly improved wound healing. A 73-year-old man underwent left axillopopliteal bypass and left 3rd, 4th , and 5th digital amputations for gangrene. Although his amputation stumps were complicated with methicillin-resistant Staphylococcus aureus (MRSA) infection, the stumps were successfully healed by VAC. He also had gangrene in his right 1st toe, which could not healed by VAC alone, and we performed right femoropopliteal bypass and right 1

[Polyspecialistic treatment of a polytrauma. Case report].

The Authors report the clinical course of M.L., a 22 years old man. He came to our observation after a road accident, in third care. In the former hospital admissions hemodynamic stabilization, surgical haemostasis of multiple hepatic lacerations with hemoperitoneum and right leg amputation were performed. He arrived to our department in a critical condition owing to mild respiratory insufficiency due to many rib fractures with sero-haematic spillage into the pleural cavity, in dialytic treatment for acute renal insufficiency consequent to right renal artery damage, soft tissues necrosis of th

Digital artery perforator (DAP) flaps: modifications for fingertip and finger stump reconstruction.

UNLABELLED: Various fingertip reconstructions have been reported for situations where microsurgical finger replantation is impossible. One method is the digital artery perforator (DAP) flap. Herein we report 13 DAP flaps for fingertip and finger stump reconstruction following traumatic finger amputations, highlighting modifications to the originally described DAP flap. METHODS: From October 1998 to December 2007, a total of 13 fingers (11 patients) underwent fingertip and finger stump reconstruction with modified DAP flaps following traumatic finger amputations. We performed six adipocutaneou

A reliable surgical approach for the two-staged amputation in unsalvageable limb and life threatening acute progressive diabetic foot infections: tibiotalar disarticulation with vertical crural incisions and secondary transtibial amputation.

BACKGROUND: If all efforts to treat acute progressive diabetic foot infection remain insufficient, the decision of major amputation should be undertaken. For this purpose, guillotine amputation is usually performed first. However, guillotine amputation below the knee level may cause the corresponding infection to spread to preserved anatomical spaces. METHOD: First stage of our procedure consists of tibiotalar disarticulation and vertical incisions performed throughout the lower leg to remove the septic foot and drain the compartments. During the interval period, appropriate antibiotherapy an

Two for one: salvage of bilateral lower extremities with a single free flap.

It is thought that free muscle flaps may remain dependent on their original pedicle for more than 1-year postoperation, particularly in the lower extremity of patients with peripheral vascular disease. We present the first case report of bilateral lower extremity salvage with a single rectus abdominis myocutaneous free flap by transecting the main pedicle. The pedicle of the free flap, going to the contralateral extremity, was safely divided at 6-weeks postoperation. A tourniquet was used to compress the pedicle at the division point to assess viability of the flap distal to the point of occlu

Salvage of finger length in septicemic necrosis using 3 free flaps from a single anterolateral thigh donor site.

Severe septicemic necrosis can result in extremely debilitating morbidity for patients, often resulting in 4-limb amputation. Further operative procedures to improve both function and cosmesis can be fraught with complexity and complications. The ideal aim in such patients would be to maintain sufficient length and provide soft tissue cover in a single 1-step procedure. We present a novel case in which twin free flaps were used acutely to provide 3 separate skin paddles to cover 2 hand stumps from a single anterolateral thigh donor site, thus maintaining the optimum digit length for subsequent

Temporary ectopic digital implantation: a clinical series of eight digits.

This paper reports our experience with temporary ectopic digital implantation. Four patients suffered 12 digital amputations with large defect over the proximal stumps. Only 8 digits were suitable for microsurgical salvage but the local conditions made direct replantation impossible. In our first patient, the two digits were ectopically implanted onto the foot, while in the second patient the four amputated digits were implanted onto the opposite forearm. After stump reconstruction, the digits were microsurgically transferred to the hand, restoring a functional pinch. One digit suffered a veno

Epidemiology and risk factors for pathologic scarring after burn wounds.

OBJECTIVE: To describe the clinical characteristics of postburn scars and determine the independent risk factors specific to these patients. While burns may generate widespread and disfiguring scars and have a dramatic influence on patient quality of life, the prevalence of postburn pathologic scarring is not well documented, and the impact of certain risk factors is poorly understood. METHODS: A retrospective analysis was conducted of the clinical records of 703 patients (2440 anatomic burn sites) treated at the Turin Burn Outpatient Clinic between January 1994 and May 15, 2006. Prevalence a

Lumbar disc herniation as a rare cause of stump pain. Case report.

The authors report the first description since 1957 of a lumbar disc herniation as a cause of stump pain. Most amputees frequently experience stump pain or phantom pain. The pathophysiology of phantom pain is not clearly defined; however, there are well-defined reasons for stump pain such as infection, tissue necrosis, hematoma, wound breakdown, bone spur, neuroma, and so on. During the differential diagnosis, radiculopathy due to lumbar disc herniation must also be evaluated.

Modified skew-flap below-knee amputation.

Between 1999 and 2001, 35 consecutive patients with diabetes (mean age, 59.4 years) were treated prospectively with a modified skew-flap below-knee amputation. The technique, results, and follow-up are described. By a mean follow-up of 3.5 years, 3 patients required below-knee amputation of the opposite extremity, 4 expired, and 28 were ambulating with a below-knee prosthesis. The modification has several advantages: A tibialis anterior muscle cushion on the distal end of the tibia prevents bone protrusion; anterior skin flaps made by the initial linear anterior incision prevent tension at the

Laparoscopic appendectomy without clip or ligature. An experimental study.

BACKGROUND: We aimed to test the efficacy and safety of closure of the appendeceal stump with only laparoscopic bipolar electrocautery in rats. METHODS: In this study, 40 female Wistar-Albino rats were used. In group I (n = 10), appendix vermiformis, approximately 1 cm in width, was completely ligated with 3/0 silk suture close to cecum, and removed. In group II (n = 20) and group III (n = 10), the appendeceal stump was coagulated by bipolar cautery. The coagulation of 70 mA took 10 s, and was repeated one more time. The stump was divided, and checked to ensure complete occlusion. Groups I an

Purpura fulminans skin loss: surgical management protocols at a regional burns centre.

People with significant skin loss after sepsis frequently require skin grafting; as such, they are often referred to regional burns units for definitive soft-tissue cover. Such individuals have complex requirements after severe sepsis, and usually have complicated wounds. We have developed a protocol for managing these patients and their wounds within a high dependency, intensive-care unit, using allograft as an adjunct in preparing the tissue bed before autografting. This protocol includes optimisation of nutritional needs, microbiological surveillance and definitive soft tissue cover. We ret
