Repair of midline vertical lower lip deficiency after mandible reconstruction.

BACKGROUND: When lower lip incisions are combined with anterior oromandibular resection and composite flap reconstruction, increased potential exists for abnormal healing of the lower lip. The aim of this study is to describe these deformities and operative techniques for their correction. METHODS: A retrospective review of all patients undergoing oromandibular reconstruction with osteocutaneous free tissue transfer at Indiana University Medical Center and Affiliated Hospitals during the 5-year period between January 1991 and 1996 was performed. Patients requiring secondary lower lip revision

[Reconstruction of speech and chewing function after extensive tumor resection in the area of the jaw and face].

BACKGROUND: Following extensive resections of head and neck tumors, re-establishing speech and masticatory function are of crucial importance for the patient. METHODS: In 23 patients with vascularised jejunal grafts for reconstruction of the intraoral mucosa, tongue and floor of mouth, a speech intelligibility test was performed, tongue and floor of mouth mobility was investigated using a 3.5 MHz ultrasound scanner. In another 18 patients with vascularised bone grafts for reconstruction of the mandible, masticatory function was analysed using a T-scan system and a miniature pressure transduce

Common clonal origin of synchronous primary head and neck squamous cell carcinomas: analysis by tumor karyotypes and fluorescence in situ hybridization.

Two synchronously arising primary squamous cell carcinomas (SCC) originating from separate sites in the anterior floor of mouth (FOM) and the pyriform sinus (PS) were evaluated by karyotype and fluorescence in situ hybridization (FISH) to determine whether they were of common or independent ancestry. The primary tumors were designated Henry Ford Hospital (HFH)-SCC-8a (FOM) and HFH-SCC-9a (PS), and the respective recurrent tumors after chemotherapy and radiation were designated -8b and -9b. HFH-SCC-8a and -8b were cultured and had closely related hypotetraploid karyotypes of monoclonal origin.

[The myofascial pectoralis major flap for tissue replacement in the oral cavity].

Since the original description by Ariyan, the myocutaneous pectoralis-major-(p.m.-)flap has been widely used for soft tissue replacement in the head and neck. In our department this flap has been established as a reliable and versatile method for soft tissue replacement in a variety of tumour defects. In the oral cavity a great number of different surgical procedures have been described for reconstruction of soft tissue defects. However, most surgeons still seem to be using either myocutaneous p.m.-flaps or the so-called "free flaps" (radial forearm flap, free jejunal flap). Authors advocating

Advancement cheiloplasty for reconstruction of floor of mouth defects.

In many cases of carcinoma of the floor of mouth, oncologic resection includes marginal mandibulectomy. Reconstruction poses a significant challenge. Requirements include coverage with thin but supple tissue to allow for dental implant or denture, and recreation of a mobile tongue and sensate floor of mouth gutter. Reconstructive efforts have ranged from skin grafts to free flaps, with variable success in fulfilling the above-mentioned requirements. This paper describes the preferred technique of the author, in which external mandibular periosteum is saved and elevated with a submucosal flap o

Occult node metastases in head and neck squamous carcinoma.

The present study examined the Liverpool database in an attempt to determine what proportion of N0 necks for various head and neck primary sites harbored subclinical squamous cell carcinoma and whether empiric treatment of occult disease improved survival over and above a "wait-and-watch policy" (treatment when metastasis becomes manifest). One hundred seventeen neck dissections were carried out for N0 necks, with 32% of specimens found to contain squamous cell carcinoma. The risk of carcinoma was highest in the hypopharynx, with 50% of specimens associated with a pyriform fossa primary cancer

A method of closure after resection of anterior floor of mouth cancers.

A variety of methods may be used to reconstruct head and neck defects after ablative surgery for cancer. Increasingly, choices of reconstructive techniques must be made on the basis of functional results attained. Described is a method of reconstruction for anterior floor of mouth defects with marginal mandibulectomy, which has yielded good functional results. All patients undergoing this form of reconstruction are on a soft oral diet, without drooling and with good speech intelligibility.

Compression plates in the treatment of advanced anterior floor of mouth carcinoma.

Advanced anterior floor of mouth squamous cell carcinoma has been traditionally treated with wide excision in conjunction with mandibulectomy and radical neck dissection. This has resulted in significant mandibulofacial defects with functional and cosmetic significance. Efforts at primary reconstruction in the past using bone grafts, osteomyocutaneous flaps, and other methods have yielded unsatisfactory results. A history of prior irradiation has made this problem even more refractory. We present a series of 11 patients who underwent composite resection for advanced anterior floor of mouth car

Central island myomucosal tongue flap.

Resection of intermediate-sized (T2 and T3) lesions of the anterior floor of the mouth is often associated with defects in speech, glutition, and cosmesis despite reconstructive efforts. This paper describes the use of the central island myomucosal tongue flap to reconstruct defects of this size. Since this flap possesses the same vascular characteristics as the myocutaneous flap it offers similar advantages. Experience with seven patients in whom this flap was used suggested that this is a dependable method of anterior floor of mouth reconstruction since there was no evidence of nonviability,

Asymptomatic red lesion in anterior floor of mouth: diagnosis and management.

We have presented a case illustrating that an asymptomatic, red lesion in the oral mucosa should be considered a malignant neoplasm, until proved otherwise. A biopsy and microscopic examination of such lesions are necessary to establish a definitive diagnosis. An appropriate method of obtaining a tissue specimen from the floor of the mouth has been described, and the importance of taking a comprehensive clinical history has been reviewed. The value of transmitting this information to the pathologist, who examines the removed tissue, has been stressed. Small, asymptomatic erythroplastic lesions

Surgical rehabilitation following anterior resection for oral cavity carcinoma.

Resection of large tumors located in the anterior aspect of the oral cavity is commonly complicated by problems of functional disturbance and esthetic aberration. Oral incompetence with defects in swallowing and inadvertent drooling are often major hurdles to overcome. The cosmetic deformity produced by the loss of support of the anterior floor of mouth and tongue due to the resected mandibular arch provides one of the most challenging reconstructive exercises facing the head and neck surgeon. Surgical rehabilitation is most adequately achieved by a consideration of each physiological defect p

Swallowing disorders in three types of head and neck surgical patients.

This study examined swallowing transit times and motility problems in three groups of patients following ablative surgery for oropharyngeal carcinoma and in a control group of 10 normal subjects. A total of 30 patients was studied: 10 after anterior floor of mouth resection, 12 after tonsil/base of tongue resection, and 8 after supraglottic laryngectomy. Videofluoroscopic studies of liquid, thin paste, thick paste, and thick paste plus liquid swallows were completed 1 week post-initiation of oral feeding following surgery. From the videotapes, oral and pharyngeal transit times were measured, a

Repair of anterior floor of mouth defects: the island pedicle tongue flap.

The island pedicle tongue flap has been utilized for situations where split-thickness skin or dermis had been used previously. It has been found to provide rapid, reliable healing, without excess scarring or problems of desquamation. No functional deficit of the tongue is produced, so swallowing and speech remain unaffected. This method is recommended for those anterior floor of mouth lesions amenable to wide local excision, but too large for primary closure.
