INTRODUCTION: Mixed connective tissue disease (MCTD) is a rare systemic autoimmune disorder in which interstitial lung disease (ILD) and pulmonary hypertension (PH) constitute the leading causes of death. No MCTD-specific recommendations currently exist regarding the timing, frequency, or indications for key paraclinical investigations. This study aimed to assess current European clinical practices regarding the use of high-resolution computed tomography (HRCT), pulmonary function tests (PFTs), transthoracic echocardiography (TTE), and nailfold capillaroscopy (NFC) in MCTD patients.

Neuroendocrine tumors (NETs) are extremely rare in the middle ear. Histopathological findings are essential to establish the definitive diagnosis of these tumors. We present the case of a 43-year-old man with a clinical diagnosis of left otitis media with effusion for five years without improvement, with a history of myringotomy with a ventilation tube. He later underwent surgery for a simple mastoidectomy with biopsy. The final diagnosis, based on histopathology and immunohistochemical studies, was a NET of the middle ear. BACKGROUND: Refractory hepatic hydrothorax (HH) is the persistent accumulation of pleural effusion secondary to portal hypertension, in the absence of cardiopulmonary or pleural etiology. It is diagnosed in patients with cirrhosis and it is a diagnosis of exclusion. Initial management consists of diuretics and sodium restriction, with a success rate of 75-80%; the remainder progress to refractory HH. Diaphragmatic plication via thoracoscopy combined with chemical pleurodesis has emerged as a therapeutic alternative to improve quality of life and reduce hospitalizations.
