Hepatitis B reactivation in HBsAg-/anti-HB-negative patients diagnosed with hematological malignancy: A case series.

BACKGROUND: Hepatitis B virus (HBV) reactivation is a well-recognized complication of immunosuppressive therapy in patients with hematologic malignancies. It usually occurs in those with chronic or resolved HBV infection identified by positive hepatitis B surface antigen (HBsAg) or hepatitis B core antibody (anti-HBc). Reactivation in patients who test negative for both markers is exceedingly rare and poses a diagnostic challenge. METHODS: We report on three patients with hematologic malignancies who developed HBV reactivation despite negative baseline serology. Cases were identified and mana

Exploring the landscape of flagellate erythema as a rare cutaneous symptom of idiopathic inflammatory myopathy: a case series-based review.

Flagellate erythema is an exceptionally rare cutaneous manifestation of idiopathic inflammatory myopathies (IIM) with poorly understood clinical significance. We report two patients who developed zebra-like flagellate erythema: one with immune-mediated necrotising myopathy (IMNM) and one with anti-Mi-2-positive cancer-associated myositis (CAM). To place these observations in broader context, we performed a thorough literature search across PubMed, Scopus and Embase and identified 34 previously published cases of IIM-associated flagellate erythema. The most frequent underlying diagnosis was der

Association between xerostomia and peri-implant health: A retrospective cohort study.

BACKGROUND: Dental implants are a long-term and reliable solution to replace missing dentition, but subject to different patient-related and biological changes that could alter its success. Xerostomia, or the alteration of the normal salivary flow, can impair oral function and potentially affect the success of any treatment, including implant outcomes. The purpose of this study is to study the association between xerostomia and peri-implantitis. METHODS: A retrospective cohort study was conducted using records of patients who received dental implants between 2010 and 2023 at the University of

Outcomes in Patients with Tuberculous Meningitis: A Prospective Cohort Study in North India.

Tuberculous meningitis (TBM) is the most severe form of extrapulmonary tuberculosis, carrying the highest mortality among all forms. This multicentric prospective cohort study conducted in North India aimed to determine the incidence of complications, functional outcome (modified Rankin scale [mRS]), and mortality in TBM. Patients 12 years old and older with TBM were followed up until completion of 9 months of antitubercular treatment. Patients with concomitant HIV infection, other infections causing meningitis, or multidrug-resistant tuberculosis were excluded. The primary outcome was mRS at

Composite inflammatory, immune, and nutritional indices for predicting surgical site infection following spine surgery: a systematic review with narrative synthesis.

BACKGROUND: Surgical site infection (SSI) is an important complication after spine surgery, with a reported incidence of approximately 1%-4% and a substantial associated healthcare burden. Early identification of high-risk patients remains difficult. Composite indices derived from routine laboratory tests that integrate inflammatory, immune, and nutritional parameters may assist perioperative SSI risk stratification. METHODS: This systematic review with narrative synthesis was conducted in accordance with the PRISMA 2020 statement and was registered in PROSPERO (registration number blinded fo

Neurological Manifestations in Whipple's Disease: A Scoping Review of Clinical Features, Laboratory Findings, and Neuroimaging Characteristics of 111 Cases.

BACKGROUND: Whipple disease (WD) is a rare, potentially fatal infection caused by Tropheryma whipplei (TW). Central Nervous System involvement (CNS-WD) is a severe complication of WD with a challenging diagnosis, due to limited data integrating neurological features, imaging, cerebrospinal fluid analysis, PCR, and intestinal histopathology. Prompt recognition and treatment are essential to improve patient outcomes. METHODS: A scoping review of the literature from 2000 to 2025 was conducted, including all CNS-WD case reports and series, to outline its clinical spectrum and diagnostic workup.

Advances in Oncologic Therapies During Pregnancy: Balancing Maternal Cure With Fetal Safety in Reproductive Cancers.

Gynecologic cancer during pregnancy requires treatment decisions that preserve maternal oncologic efficacy while minimizing fetal harm. This focused narrative review critically evaluates systemic therapy for cervical, ovarian, and other selected gynecologic malignancies during pregnancy, with surgery, radiotherapy, obstetric outcomes, and emerging technologies discussed only when they directly inform systemic-treatment decisions. Evidence is interpreted using a transparent four-category framework based on study design, consistency, directness, and applicability to pregnancy. The most consisten

Association between the site of emergency care and patient-centered outcomes in patients with cancer: rationale and design of the French COVER cohort.

Patients living with cancer now account for a substantial and growing share of emergency department (ED) visits and carry a high burden of hospitalization and in-hospital death. Their acute presentations are frequently multifactorial - driven by disease progression, treatment toxicity, immunosuppression, or the decompensation of comorbid conditions. They may present with higher rates of complications because of novel therapies, such as immune checkpoint inhibitors and cellular therapies, whose emergency manifestations remain unfamiliar to many emergency physicians. Despite this growing importa

The Gut-Brain Axis in Parkinson's Disease: From Clinical Observation toward Mechanism-Based Precision Medicine.

Gastrointestinal dysfunction is increasingly recognized as a core feature of Parkinson's disease (PD), affecting quality of life, nutrition, medication response, and potentially the earliest stages of pathogenesis. This perspective traces the evolution of the gut from a neglected source of non-motor symptoms to a central component of biological models of PD. Gastrointestinal manifestations are physiologically heterogeneous and require objective phenotyping to distinguish dysphagia, gastroparesis, slow transit, defecatory dysfunction, and overlapping mechanisms. The body-first/brain-first frame

Early detection of chimeric antigen receptor-T-associated neurotoxicity via cytokine monitoring in serum.

Immune effector cell-associated neurotoxicity syndrome (ICANS) is a common and life-threatening complication of chimeric antigen receptor (CAR) T-cell therapy, with early detection being critical for timely intervention and improved outcomes. Cytokines such as interleukin-6 (IL-6) are key mediators of the inflammatory cascade underlying ICANS pathogenesis, but prospective clinical evidence establishing whether serial IL-6 elevations precede ICANS onset remains limited. Here we quantify IL-6 levels in a prospective cohort of 40 CAR-T patients (270 serum samples), using a simple in-house microfl

Lemierre Syndrome After Influenza A Virus Infection.

Lemierre syndrome is a rare condition characterized by septic thrombophlebitis of the internal jugular vein with metastatic septic emboli, most commonly affecting the lungs. We report a case of Lemierre syndrome temporally associated with influenza A infection in a previously healthy 21-year-old male. The patient initially presented to the emergency department with severe headache and tested positive for influenza A. Over the following seven days, his symptoms progressed to neck pain with decreased range of motion, hemoptysis, cough, and persistent myalgias. Despite four additional emergency d

Guillain-Barré Syndrome As the Initial Manifestation of Human Immunodeficiency Virus Infection: A Case Report.

Guillain-Barré syndrome (GBS) is an acute immune-mediated polyradiculoneuropathy typically triggered by an antecedent infection. Human immunodeficiency virus (HIV) is an uncommon but recognized association, and GBS may rarely constitute the first clinical manifestation of previously undiagnosed HIV infection. A 36-year-old man presented with subacute facial diplegia, predominantly distal tetraparesis, sensory ataxia, glove-and-stocking paresthesia, and hyporeflexia. Cerebrospinal fluid (CSF) analysis showed albuminocytologic dissociation; electrophysiological studies demonstrated a sensorimoto

Triple M Overlap Syndrome: A Rare Complication of Immune Checkpoint Inhibitors.

Immune checkpoint inhibitors are a major part of many cancer treatment protocols. Most side effects are generally minor, including fatigue, rashes, flu-like symptoms, headaches, or cough. However, some potential side effects may require prompt treatment to ensure optimal outcomes, so it is essential for clinicians to be knowledgeable about these rare but possible complications. We report a case of an 88-year-old woman who presented with the triple M overlap syndrome (TMOS) of myasthenia gravis, myositis, and myocarditis after receiving four rounds of atezolizumab treatment. This diagnosis was

Modern multidisciplinary management of brain metastasis.

Brain metastases (BrM) are no longer considered to be a uniform end-stage complication of cancer, but as a biologically and clinically heterogeneous disease requiring coordinated management. This article synthesises the fundamentals of BrM diagnosis and treatment, relevant to primary and urgent care settings. Treatment sequencing should account for acuity of neurologic symptoms, anticipated intracranial response, extracranial disease tempo, steroid requirements, and risks of local therapy. Focal radiotherapy is now the dominant local modality for limited BrM. Surgery remains central for large,

Association of RSV Vaccination With Clinical Outcomes in Older Adults With Heart Failure: A Propensity-Matched Analysis.

BACKGROUND: Respiratory syncytial virus (RSV) is increasingly recognized as a trigger for adverse outcomes in older adults with heart failure (HF). Following FDA approval of RSV vaccines for adults ≥ 60 years in 2023, real-world evidence in HF populations remains limited. METHODS: We conducted a multicenter retrospective cohort study using the TriNetX US Collaborative Network. Adults ≥ 60 years with HF were identified and stratified by RSV vaccination status from May 1, 2023, to January 01, 2026. Propensity score matching (1:1) was performed. The primary outcome was all-cause mortality. Secon

Incidence, Clinical Characteristics, Management, and Outcomes of Hematologic Immune-Related Adverse Events Associated With Immune Checkpoint Inhibitors: A Single-Center Japanese Retrospective Cohort Study.

INTRODUCTION: Immune-related adverse events (irAEs) are complications associated with immune checkpoint inhibitors (ICIs). Hematologic irAEs (heme-irAEs) are rare but potentially serious and may interfere with cancer treatment. However, their clinical characteristics remain insufficiently described. METHODS: We conducted a single-center retrospective study using the National Cancer Center Hospital East medical database from April 1, 2014, to March 31, 2024. Potential heme-irAE cases were identified using diagnostic name searches, bone marrow examination records, and positive DAT results and w
