Modeling Framework for Economic Evaluation of Disease-Modifying Therapies in Parkinson's Disease: Evidence Review and Expert Opinion.

BACKGROUND: Parkinson's disease (PD) is associated with a substantial clinical, humanistic, and economic burden across its disease continuum. Economic models for PD have largely been developed to evaluate symptomatic therapies, and typically adopt narrow payer perspectives, focus on advanced disease stages, and rely on simplified state-transition structures that do not distinguish symptomatic relief from disease modification. As candidate disease-modifying therapies emerge, modeling approaches capable of evaluating long-term value under substantial uncertainty will become essential, particular

Brief report: Effectiveness and safety of recombinant zoster vaccine in inflammatory bowel disease patients aged 18 years and older: A matched cohort and self-controlled case series study.

Individuals with inflammatory bowel disease (IBD) are at increased risk of herpes zoster (HZ). Using a matched cohort design, we evaluated the vaccine effectiveness (VE) of recombinant zoster vaccine (RZV) against HZ among IBD patients aged ≥18 years. Within the Kaiser Permanente Southern California population, individuals who received 2 RZV doses were matched up to 1:3 to RZV-unvaccinated individuals. Individuals were followed from 31 days after the receipt of the 2nd RZV dose until the occurrence of a censoring event. VE was calculated using Cox proportional hazards regression. A self-contro

Leniolisib and rapamycin in Activated PI3-Kinase-δ-syndrome: a retrospective ESID registry-based analysis.

BACKGROUND: Activated PI3K-delta syndrome (APDS) is a rare inborn error of immunity characterized by variable immune dysregulation, recurrent infections, and increased risk of malignancy. Clinical management is challenging. Rapamycin has been widely used off-label to control immune dysregulation, while the selective PI3Kδ inhibitor leniolisib has recently become available as a disease-specific targeted therapy. Real-world data on these treatment approaches remain limited. OBJECTIVE: To assess real-world treatment indications, clinical course, immunological changes, and tolerability in patient

Diagnostic Performance of Donor-derived Cell-free DNA Combined With Immune Activation Markers for Detecting Acute Rejection After Liver Transplantation.

BACKGROUND: Accurate differentiation of acute rejection (AR) from other causes of graft dysfunction after liver transplantation remains challenging in real-world clinical practice. Although donor-derived cell-free DNA (dd-cfDNA) has emerged as a promising non-invasive biomarker of graft injury, its specificity may be limited in clinically suspected rejection due to concurrent inflammatory and non-rejection-related graft injuries. Immune activation markers may provide complementary information by reflecting ongoing alloimmune responses. METHODS: In this prospective diagnostic study, we enrolle

Autoantibody Profiles of Immune Checkpoint Inhibitor-Associated Myasthenia and Myositis.

IntroductionImmune checkpoint inhibitor (ICI)-associated myasthenia and myositis are rare, potentially life-threatening neuromuscular immune-related adverse events, but their serological spectrum remains incompletely characterized. This study aimed to elucidate their autoantibody profiles and explore associations with severe clinical features.MethodsThis retrospective, multicenter, serum-repository-based observational cohort study included 50 patients with ICI-associated myasthenia and myositis referred from institutions throughout Japan. Clinical data and serum samples were analyzed using con

A case report: an unusual case of ulcerative intestinal IgG4-related disease with clinical features overlapping with probable intestinal Behcet's disease.

BACKGROUND: IgG4-related disease (IgG4-RD) is a systemic fibroinflammatory condition characterized by IgG4-positive plasma cell infiltration, typically presenting with organ enlargement or tumor-like masses. Luminal gastrointestinal involvement is rare, and ulcerative lesions are even less common. Overlap syndromes of IgG4-RD with other immune disorders have been reported, but overlap of its clinical features with those of intestinal Behcet's disease has not been documented. CASE PRESENTATION: A 62-year-old man with a history of recurrent oral ulcers presented with sudden hematochezia and hyp

Measles Vaccine Boosters in Immunocompromised Adults to Reduce Morbidity and Mortality From Acute Measles Infection: A Scoping Review.

Measles is a highly contagious viral illness associated with significant global mortality, despite the availability of an effective vaccine. As vaccine hesitancy continues to increase following the COVID-19 pandemic, combined with a lack of measles-specific antiviral agents, the risk of outbreaks and epidemics remains elevated. Immunocompromised individuals have a higher risk of serious complications from measles, especially because the live-attenuated measles, mumps, and rubella vaccine may be contraindicated in severely immunocompromised patients. As such, these individuals depend on herd im

Treatment patterns and outcomes in advanced clear cell sarcoma and gastrointestinal neuroectodermal tumors.

BACKGROUND: Clear cell sarcoma (CCS) and gastrointestinal neuroectodermal tumor (GNET) are ultra-rare soft tissue sarcomas with no established standard of care for advanced disease. Data evaluating outcomes of systemic therapy remain limited. We aimed to evaluate systemic therapy in patients with metastatic CCS/GNET treated at a tertiary referral institution. METHODS: This is a retrospective, single-institution study of patients with metastatic CCS/GNET diagnosed between January 2000 and May 2025. The primary objective was to describe patterns of systemic therapy use and secondary endpoints i

Time to recovery from long COVID: a longitudinal analysis of symptom duration and risk factors using accelerated failure time models.

OBJECTIVES: Long COVID comprises heterogeneous symptoms that may persist for months to years after acute SARS-CoV-2 infection. While prevalence is well described, less is known about symptom duration and determinants of recovery. METHODS: We conducted a multicenter longitudinal cohort study across 12 institutions in Korea (December 2022-March 2025), enrolling adults with confirmed infection and uninfected controls. Participants were followed for up to 22 months with repeated assessments of symptoms, vaccination, and comorbidities. Eight common symptoms were analyzed. Time to resolution was es

Eczema in inborn errors of immunity with atopy: frequency, timing, phenotypic clustering.

BACKGROUND: Inborn errors of immunity with atopy (IEIs-A) are an IEI subgroup with eczema as a prominent clinical feature that can mimic atopic dermatitis (AD). However, IEIs-A-associated eczema can differ from that of classic AD, and its frequency, genotype-specific clinical patterns and temporal relationships with other IEIs-A manifestations (infections, allergic manifestations, autoimmunity) remain poorly defined. OBJECTIVES: To characterize eczema frequency, timing and phenotypes in IEIs-A patients, and explore eczema's relationship with other clinical IEIs-A manifestations. METHODS: Thi

HPV-related cancer prevention in Spain: progress and remaining challenges in vaccination, screening, and surveillance.

Human papillomavirus (HPV) is a necessary causal factor for most cervical cancers and contributes to several other HPV-related malignancies. The nonavalent HPV vaccine targets the most prevalent oncogenic genotypes and has the potential to prevent up to 90% of HPV-attributable cancers. However, cervical cancer screening remains essential, as vaccination does not cover all oncogenic HPV genotypes, does not treat established HPV infection, and cannot prevent disease in individuals after HPV exposure. Available Spanish data do not yet show a clear sustained decline in HPV-related cancers comparab

Additive effects of anxiety/depression and transportation noise exposure on cardiovascular risk and measures of neural-immune mechanisms.

BACKGROUND: Transportation noise exposure and stress-related psychiatric conditions (e.g., anxiety, depression) are well-known contributors to cardiovascular disease (CVD). Both exposures activate shared biological pathways, including heightened stress-associated neural activity (SNA), autonomic dysfunction, and systemic inflammation that culminate in CVD; however, it remains unknown whether their co-occurrence confers additive CVD risk. OBJECTIVE: We hypothesized that individuals with both anxiety and/or depression and high noise exposure (vs. those with one or neither exposure) have greater

Practice patterns and outcomes in cancer patients developing immune checkpoint inhibitors-related AKI.

BACKGROUND: Acute kidney injury (AKI) is a known immune-related adverse event of cancer immune checkpoint inhibitor (ICI) therapy. Further population-based data on AKI incidence, risk factors and practice patterns post-ICI therapy are needed. METHODS: We measured the cumulative incidence of AKI among advanced cancer patients while receiving ICI therapy and non-ICI systemic therapy in Ontario, Canada (2012-18). An increase in serum creatinine was used to define AKI and graded according to event severity. Time to event modeling was used to compare the risk of developing AKI, pre-disposing facto

Reversibility of Immune Dysfunction Following Pediatric Thermal Injury.

Pediatric thermal injury induces immune dysfunction, which is associated with adverse clinical outcomes (eg, nosocomial infections [NIs]). As such, it is crucial to identify those most at risk for developing NI and determine immunomodulating therapeutics to augment the immune response. Our hypothesis was that immune suppression after pediatric thermal injury is reversible ex-vivo using the immunomodulators recombinant human granulocyte macrophage-colony stimulating factor (GM-CSF) and varlilumab (CD27-agonist). We enrolled 141 pediatric patients with acute thermal injuries from a single burn c

Emerging livestock science and technologies can drastically alter the projections of global food security models.

The incidence and prevalence of all types of diabetes mellitus is increasing at an alarming rate. Modern therapy involves greater and earlier use of intensive insulin regimens in order to achieve better control of blood glucose levels and reduce the long-term risks associated with the condition. Insulin therapy is associated with important cutaneous adverse effects, which can affect insulin absorption kinetics causing glycemic excursions above and below target levels for blood glucose. Common complications of subcutaneous insulin injection include lipoatrophy and lipohypertrophy. The developme

Lost and Found: Is Olfactory Recovery More Promising After COVID-19 Than Other Causes, Even 2 Years Later?

BACKGROUND: Olfactory dysfunction is a hallmark feature of COVID-19, yet the potential for recovery with long-standing COVID-19-related smell loss (CRSL) remains uncertain, particularly when treatment is initiated years later. This study evaluated olfactory outcomes in patients with CRSL compared with non-COVID-19-related smell loss (non-CRSL), with emphasis on delayed treatment initiation. METHODS: This retrospective cohort study included 104 patients evaluated at a tertiary clinic between January 2023 and February 2025, comprising 47 patients with CRSL and 57 with non-CRSL. Nanoparticle retention enables non-invasive detection of metastases by magnetic particle imaging in murine breast cancer models.

Early detection of metastatic disease improves cancer survival, yet existing modalities are limited in their detection capabilities. We propose that magnetic particle imaging (MPI), an emerging technology, can be used for early detection of primary tumors and metastases. MPI detects minute quantities of magnetic particles that act as "cold tracers" which accumulate in areas of high immune activity. Methods: Pegylated Synomag® nanoparticles were intravenously injected into mouse models of breast cancer bearing primary tumors and spontaneously developed lung metastases. After 72 h, mice were sub
