Free Thyroxine as a Predictor of Mortality in Critically Ill Septic Patients-A Retrospective Study.

Background/Objectives: Euthyroid sick syndrome (ESS), and particularly low T3, have been associated with increased mortality in septic patients, yet the prognostic value of free thyroxine (fT4) remains controversial. This study aims to evaluate the association between fT4 on ICU admission and mortality in septic patients. Methods: We conducted a single-center, retrospective observational study including 149 adult patients with sepsis or septic shock admitted to the Anesthesia and Intensive Care I Department of the Cluj County Emergency Hospital, Cluj-Napoca, Romania, between January 2019 and S

Sex-related mortality differences in septic shock: A propensity score-matched study.

PURPOSE: This study aimed to evaluate sex-specific differences in 28-day mortality among young adults (18-55 years) with septic shock using a large propensity score-matched cohort. MATERIALS AND METHODS: We performed a retrospective propensity score-matched analysis using the TriNetX network. Adults 18-55 years old developing septic shock between January 2011 and January 2020 were identified. Women were matched 1:1 to men. The primary outcome was 28-day all-cause mortality; 90- and 180-day mortality were prespecified secondary outcomes. RESULTS: A total of 100,894 young adults (≥18 and ≤ 55

Closed-loop and Artificial intelligence guided cardiopulmonary resuscitation using real-time physiological signals: a systematic review and meta-analysis.

BACKGROUND: Current cardiopulmonary resuscitation (CPR) includes breaks to check rhythm and pulse, while high-quality CPR requires continual chest compressions. Artificial intelligence (AI)-driven "closed-loop" or "near-closed-loop" systems may evaluate CPR-contaminated signals, guide shock and pulse decisions. METHODS: According to PRISMA statement 2020, We searched databases from 2010 to 2025 for studies on animal or human cardiac arrest where AI-based or algorithmic tools gave real-time intended guidance during CPR or CPR-dominant analysis windows. Evidence was divided into animal perfusio

Delayed superior vena cava perforation: a rare and life-threatening case report.

BACKGROUND: Central venous catheterization (CVC) is routinely performed in emergency and critical care settings and is generally considered safe, particularly when ultrasound guidance is used. However, rare and potentially fatal delayed complications may still occur. Delayed perforation of the superior vena cava (SVC) is exceedingly uncommon and may be under-recognized, especially following right-sided catheterization. CASE PRESENTATION: A 63-year-old woman underwent ultrasound-guided right subclavian central venous catheterization prior to emergency laparotomy for small bowel obstruction. The

From Ingestion to Septic Shock: A Case Report on Esophageal Perforation and Mediastinitis Following Foreign Body Ingestion.

Acute mediastinitis is a rare but life-threatening condition, most commonly secondary to esophageal perforation. Foreign body ingestion in adults is an unusual etiology, with limited data from Morocco, where only a few cases have been reported. We report a case of mediastinitis complicating esophageal perforation following accidental foreign body ingestion in a Moroccan patient, highlighting diagnostic challenges and management in a resource-limited setting. We report the case of a 38-year-old patient admitted with septic shock, complicated by dysphagia and epigastric pain, five days after acc

Physiologically Difficult Airways in Emergency Medicine: A Narrative Review of Recognition, Resuscitation, and Management Strategies.

The focus of managing critically ill patients' airways has traditionally centered around anatomically complicated issues, however an increasing amount of literature indicates that peri-intubation complications are likely to be related to the patient's physiologic state at the time of intubation. Physiologic instability may lead to severe hypoxemia, cardiovascular collapse, or cardiac arrest even when the anatomy of the airway is uncomplicated. Shock states, severe metabolic acidosis, right heart failure, or an insufficient oxygen reserve may contribute to these unstable conditions but they oft

Suicide left ventricle after aortic valve intervention: Mechanisms, risk phenotypes, and clinical considerations.

Suicide left ventricle (SLV) is a rare but potentially catastrophic complication of surgical or transcatheter aortic valve replacement, characterized by acute hemodynamic collapse after relief of aortic stenosis. Abrupt reduction in left ventricular afterload unmasks dynamic intracavitary obstruction driven by adverse ventricular geometry, hypercontractility, and impaired filling, resulting in a low-cardiac output state. Although increasingly recognized, SLV remains a rare yet potentially fatal condition that continues to pose diagnostic and therapeutic challenges. This review synthesizes avai

Dr. Harold Griffith and Sir Robert Macintosh: a British response to curare's introduction to anesthesia.

Curare's implementation into anesthesia care by Dr. Harold Griffith was facilitated by the availability of medical-grade curare, the demonstration that it could provide safe muscle relaxation in psychiatric patients undergoing "shock therapy," and the foresight of a Squibb scientist who realized its potential in the operating room. Griffith's demonstration that curare safely provided muscle relaxation in surgical patients eliminated the need for dangerously high doses of anesthetic gases. This helped pave the way to "balanced anesthesia," whereby small doses of selectively acting drugs are giv

Timing of Kidney Replacement Therapy Initiation in Patients With Septic Shock: A Descriptive Study Using a Japanese Inpatient Database.

BACKGROUND: Kidney Replacement Therapy (KRT) timing in septic shock remains debated. We assessed temporal trends in KRT initiation and outcomes in Japan. METHODS: Using a nationwide EMR database (2016-2023), we included adults (≥ 20 years) with septic shock admitted to the intensive care unit (ICU) or high-dependency care unit (HDU) on day 0 who received KRT. Outcomes were KRT timing by hospital day, in-hospital mortality, and KRT dependency at discharge; sensitivity analyses used consistently reporting hospitals. RESULTS: Among 688 hospitalizations (median age 74 years), KRT initiation shif

Clinical utility of proenkephalin A 119-159 for prediction of worsening renal function and prognosis in patients with sepsis -results of a patient-level meta-analysis.

BACKGROUND: Proenkephalin A 119–159 (penKid) is a functional kidney biomarker inversely correlating with the glomerular filtration rate. We conducted a patient-level meta-analysis to evaluate whether penKid provides additional value to serum creatinine (sCr) for assessing kidney function and outcome in patients with sepsis or septic shock. METHODS: To identify eligible studies, the database PubMed and internal resources have been searched in March 2025. Randomized controlled trials or observational trials enrolling patients with sepsis or septic shock admitted to ICU (> 100 patients per study)

Ventricular arrhythmia ablation timing and outcomes in electrical STORM requiring V-A ECMO support: the STORM-ECMO study.

BACKGROUND: Electrical storm (ES) with refractory cardiogenic shock carries high mortality. While veno-arterial extracorporeal membrane oxygenation (V-A ECMO) may stabilize circulation, the role and timing of ventricular arrhythmia (VA) ablation under ECMO remain unclear. OBJECTIVES: To assess the impact and optimal timing of VA ablation on ECMO weaning success in patients supported with V-A ECMO for ES-related refractory cardiogenic shock. METHODS: We conducted a French multicenter retrospective cohort study including 218 patients treated between 2015 and 2023 in 9 tertiary centers. Patients
