BACKGROUND: Cardiac dysfunction is common in septic shock and may be influenced by intravenous (IV) fluid therapy. We aimed to investigate the effects of restrictive versus standard IV fluid therapy on plasma N-terminal pro B-type natriuretic peptide (NTproBNP), a marker of cardiac dysfunction, in adult intensive care unit (ICU) patients with septic shock. METHODS: This prospective exploratory sub-study of the randomised CLASSIC trial enrolled adult ICU patients with septic shock at one Danish and one Swedish ICU between February 2020 and October 2021. Patients were randomised to restrictive v

BACKGROUND: Impairment of microcirculatory tissue perfusion is a central element of sepsis. Unfortunately, monitoring microcirculatory tissue perfusion to detect microcirculatory dysfunction at the bedside remains challenging. Therefore, the effects of sepsis and systemic infection on the intestinal microcirculatory tissue perfusion remain poorly understood.
METHODS: In this experimental study, lipopolysaccharide was infused in six healthy female adult Yorkshire cross-breed pigs to induce endotoxic shock. Subsequently, animals were primarily resuscitated using fluids and subsequently with add

The last specific international European recommendations regarding the management of cardiogenic shock (CS) regardless of the etiology were issued over 10 years ago. We present herein recommendations for the management of CS in adults, developed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system by an expert group of from the French Intensive Care Society [Société de Réanimation de Langue Française (SRLF)] and the French Society of Cardiology [Société Française de Cardiologie (SFC)], with the participation of the French Society of Anesthesia and Intensi

Severe infection and sepsis can significantly alter airway anatomy through inflammation, edema, and tissue friability, complicating definitive airway management in critically ill patients. These challenges are further magnified in individuals with prior failed intubation or airway trauma, where the margin for error is narrow, and loss of a functioning airway can be catastrophic. We report the case of a critically ill patient with influenza A-associated acute hypoxic respiratory failure and septic shock. Our patient required definitive airway management in the setting of severe airway edema and

Immunonutrition, once thought to reduce infections in critically ill patients, faced scrutiny in the early 2000s following large trials that showed limited benefits and potential harm, particularly from formulas supplemented with arginine and glutamine. Newer evidence on immune-enhancing diets (IEDs) has prompted a shift toward individualized, precision-based nutrition strategies. Omega-3 fatty acids remain promising, especially in patients with traumatic brain injury or critical illness-induced muscle-wasting. Current guidelines recommend selective use of IEDs, such as in patients with cancer

BACKGROUND: Intraoperative haemorrhage during equine anaesthesia is a rare but potentially fatal complication requiring prompt haemodynamic support. Postural manoeuvres such as Trendelenburg (TL) positioning and vasopressor support augment venous return and improve cardiac output (CO), yet their efficacy in anaesthetised hypovolemic horses remains unstudied.
OBJECTIVES: To evaluate haemodynamic effects of TL positioning during normovolemia and acute haemorrhagic hypovolemia, and to determine whether concurrent norepinephrine (NE) infusion enhances cardiovascular stabilisation.
STUDY DESIGN:

BACKGROUND: Blood lactate is a key biomarker of tissue hypoperfusion and metabolic distress, reflecting the balance between oxygen delivery and cellular metabolism. Early identification of rising lactate levels is critical for timely intervention in critically ill patients, yet existing predictive tools are static and limited to specific ICU subgroups.
METHODS: We developed and externally validated two machine-learning models that provide hourly, real-time predictions of hyperlactatemia (lactate >2 mmol/L) within 6- and 12-hour horizons in general ICU populations. This retrospective, multi-co

BACKGROUND: Propofol and midazolam are widely used in sedation and general anesthesia, but there has been a lack of systematic research on safety signals caused by the combination medication. METHODS: We collected data on drug safety signals related to propofol and midazolam from the FAERS database (2004Q1-2025Q1). Disproportionality analysis, Time-to-event, and Weibull distribution are performed for signal monitoring and risk assessment. We further analyze the potential mechanism of allergic reactions caused by the combination medication. RESULTS: A strong signal for Propofol Infusion Syndrom

BackgroundAcute kidney injury (AKI) is frequent in patients supported with venoarterial extracorporeal membrane oxygenation (VA-ECMO), largely due to the severity of cardiogenic shock and multiorgan failure. Renal replacement therapy (RRT) can be delivered either via an indwelling catheter (parallel system, PS) or through direct connection to the ECMO circuit (integrated system, IS). Their respective safety profiles, particularly regarding infectious, hemorrhagic, and circuit-related complications, remain insufficiently characterized.MethodsWe conducted a single-center retrospective analysis o

BACKGROUND: Severe odontogenic infections requiring surgical drainage pose significant airway management challenges. This study aimed to compare perioperative characteristics and clinical outcomes between patients managed with conventional laryngoscopy versus awake fiberoptic intubation, to identify factors associated with the selection of airway management technique, and to determine predictors of hospital length of stay.
METHODS: This single-center retrospective study included 85 adult patients who underwent surgical drainage of severe odontogenic infections under general anesthesia between

Background: Pediatric out-of-hospital cardiac arrest (OHCA) is rare and associated with poor outcomes. Evidence from physician-staffed EMS systems remains limited. This study aimed to describe the incidence, presenting rhythms, EMS response intervals, and outcomes of pediatric OHCA, and to describe incidence, presenting rhythms, EMS response intervals, and prehospital outcomes in a local physician-staffed EMS system. Methods: We conducted a retrospective study of all pediatric (0-17 years) OHCA cases managed by the ASL Lecce physician-staffed EMS (southern Italy) between 2013 and 2025. Data we

In order to reduce sepsis mortality, early treatment implementation and referral to the most appropriate ward (emergency department (ED) or intensive care unit (ICU) are recommended. This multicentre retrospective study aims to study the relationship between 30-day mortality and the admission mode to ICU or ED among patients with septic shock cared for by a prehospital mobile Intensive Care Unit (mobile-ICU). From May 2016 to December 2022, septic shock patients cared for by a prehospital mobile-ICU were retrospectively analysed. To assess the relationship between 30-day mortality rate and the

PURPOSE OF REVIEW: Climate change is already disrupting healthcare delivery with perioperative medicine, particularly pediatric anesthesia, being both highly exposed to climate-related shocks and a major contributor to healthcare-related greenhouse gas emissions (GHG). This review examines how mitigation and resilience strategies can be integrated into pediatric anesthetic practice.
RECENT FINDINGS: Sustainability measures in pediatric anesthesia are currently actionable and clinically beneficial. Reducing the use of volatile anesthetics through low-flow techniques, avoiding N2O or desflurane

BACKGROUND: Postoperative organ dysfunction is a leading cause of death and disability following hip fracture surgery in older patients. Intraoperative hypotension is a major modifiable risk factor for this complication, yet the optimal management strategy to prevent it remains controversial. We hypothesize that an individualized blood pressure management strategy is superior to standard management in reducing postoperative organ dysfunction.
METHODS: This single-center, randomized, controlled trial will enroll 180 patients aged 65-85 years with hip fractures under general anesthesia. Eligibl
