Humidity impacts volatile anesthetic and heat responses of malignant hyperthermia susceptible mice.

BACKGROUND: Malignant Hyperthermia (MH) is characterized by life-threatening whole-body hyperthermic reactions triggered by volatile anesthetics in susceptible individuals. MH susceptibility (MHS) and exertional heat stroke (EHS) are linked to mutations in the type I ryanodine receptor (RYR1). However, the discordance between the estimated prevalence of RYR1 MHS mutations in the general population (1:625-1,075) and the occurrence of MH episodes during anesthesia (1:100,000) suggests additional factors beyond genetics. METHODS: Using an established MHS mouse model (Y524S or YS mice), 10-12-wee

Isoform-specific structure and function of calsequestrin: Implications beyond calcium buffering in health and disease.

Calsequestrin (CASQ) plays an important role in muscle contraction by buffering Ca2+ inside the sarcoplasmic reticulum (SR). Intriguingly, mammals express two CASQ isoforms encoded by separate genes with highly conserved protein structure. CASQ1 is mainly expressed in fast-twitch skeletal muscles; whereas CASQ2 predominates in slow-twitch muscles and heart. CASQ2 function is poorly defined in rhythmically beating heart where SR Ca2+-release is graded through Ca2+-induced Ca2+-release (CICR), compared to CASQ1 in skeletal muscle where Ca2+-release is all or none. A unique property of CASQ is th

PSMA RNA aptamer A10-3.2 conjugated with indocyanine green for rapid intraoperative imaging and effective mild photothermal therapy of prostate cancer.

In prostate cancer (PCa) precision therapy, several challenges remain; among them, the delayed onset of intraoperative fluorescence imaging and the absence of targeted temperature control for local hyperthermia restrict the flexible perioperative use of tracers and increase the risk of thermal damage to adjacent sensitive tissues. To address these issues, we developed A10-3.2-ICG, a prostate-specific membrane antigen (PSMA)-targeting RNA aptamer conjugated with indocyanine green (ICG) via amine coupling. The probe was validated by mass spectrometry and spectroscopic analysis. For intraoperativ

Volatile Anesthetics: A Comprehensive Review of Pharmacology, Delivery Systems, and Safety Considerations for ICU Practitioners.

OBJECTIVE: Volatile anesthetics (VAs) are gaining renewed interest as a sedation strategy in the intensive care, offering an alternative to traditional IV agents. VAs provide several pharmacologic advantages, including rapid onset and offset, minimal systemic metabolism, and favorable recovery profiles. Advances in delivery systems enabled the safe and practical administration of volatile agents in the ICU. Thus, we aimed to describe the pharmacology and safety aspects of inhaled agents as well as the systems designed to deliver VAs in the ICU. DATA SOURCES: Relevant literature was identified

Continuous Epidural Anesthesia for Living-related Renal Transplant in a Patient With Malignant Hyperthermia and Reduced Cardiac Function: A Case Report and Narrative Review of the Literature.

Kidney transplantation is most often performed under general anesthesia, with or without regional anesthesia for adjunctive pain control. Select patients may benefit from neuraxial anesthesia as their primary anesthetic, especially when their comorbidities preclude the safe provision of general anesthesia. In this report, we describe the case of a 75-year-old man with malignant hyperthermia and ischemic cardiomyopathy who underwent a living-related kidney transplant under epidural anesthesia. Intraoperative hemodynamics were stable, and pain control was excellent; the surgeon reported no diffi

Targeting Triple-Negative Breast Cancer: A Special Focus on Phototherapy and Nanomaterials.

Triple-negative breast cancer (TNBC) is an aggressive subtype of breast cancer lacking estrogen, progesterone, and HER2 receptors. This characteristic limits the effectiveness of hormonal and targeted therapies, and despite advances in chemotherapy (ChT), radiotherapy (RT), surgery, targeted therapy (TT) and immunotherapy (IT), clinical outcomes remain poor, highlighting an urgent need for new therapeutic strategies. The development of advanced nanotechnology-based strategies has opened new avenues for the diagnosis and therapy of TNBC. This review focuses on photothermal therapy (PTT) combine

Magnetic fluid hyperthermia for different breast categories: Effect of nanoparticle distribution patterns and temperature-dependent perfusion models on thermal transport.

Magnetic fluid hyperthermia (MFH) offers a targeted means of thermally degrading malignant tissue by impregnating superparamagnetic nanoparticles (SPMNPs) within a tumor and exciting them with an alternating magnetic field. Although numerical modeling has been attempted before to guide MFH treatment optimization, systematic studies jointly integrating specific tissue categories, clinically realistic nanoparticle distributions, and dynamic perfusion evolution remain absent from the literature, leaving a critical gap in personalized MFH treatment planning. To address this, we develop a finite-el

Successful management of malignant hyperthermia with dantrolene complicated by upper limb DVT: A case report.

Malignant hyperthermia (MH) remains the monster every anesthesiologist can meet suddenly in the OR. It is a rare but life-threatening hypermetabolic reaction triggered by certain anesthetic agents. Here we present a case of a 40-year-old male, who developed MH 45 min after induction of anesthesia for functional endoscopic sinus surgery (FESS) in King Abdulaziz Medical City in KSA. It was not the first time the patient was exposed to general anesthesia, with no relevant family history. Early recognition, prompt administration of dantrolene, and aggressive supportive measures resulted in full re

Total intravenous anesthesia for cardiac transplantation in a teenager with Becker muscular dystrophy: A case report.

Becker muscular dystrophy (BMD) is a genetic disorder caused by partial deficiency of dystrophin, leading to progressive skeletal muscle weakness and cardiac failure. Up to one-third of affected individuals develop dilated cardiomyopathy, eventually requiring heart transplantation at a young age. Although BMD is not directly linked to malignant hyperthermia (MH), patients may experience severe MH-like reactions, particularly when exposed to succinylcholine or volatile anesthetics. Therefore, total intravenous anesthesia (TIVA) is often preferred in this population, despite the cardioprotective

Anaesthetic considerations for a patient with Emery-Dreifuss muscular dystrophy undergoing cardiac resynchronisation therapy with pacemaker implantation.

Emery-Dreifuss muscular dystrophy is a rare inherited neuromuscular disorder characterised by early joint contractures, slowly progressive humero-peroneal weakness and cardiac conduction defects or cardiomyopathy. Although contractures and weakness usually begin in childhood, cardiac complications, such as atrioventricular block, arrhythmias and dilated cardiomyopathy, typically emerge in early adulthood and may cause sudden cardiac death, if unrecognised. Anaesthetic management is challenging due to potential airway complications from cervical contractures, restrictive respiratory physiology

Nanomagnetic Hyperthermia Sensitizes Gemcitabine Chemosensitivity in Pancreatic Cancer by Inhibiting HSPB1 to Amplify ACSL4-Mediated Ferroptosis.

Pancreatic cancer, one of the most malignant solid tumors with the poorest prognosis, is commonly treated with gemcitabine (GEM)-based systemic chemotherapy. However, chemoresistance remains a significant therapeutic challenge. Ferroptosis, a novel form of programmed cell death, has demonstrated exceptional susceptibility in chemoresistant cancer cells. Due to their unique magnetic responsiveness, iron oxide nanoparticles can generate heat under alternating magnetic fields, which enables magnetic hyperthermia (MH) therapy. This approach not only directly induces tumor cell apoptosis but also e

Case Report: Atypical neuroleptic malignant syndrome induced by paliperidone palmitate-diagnostic challenges and clinical considerations.

Poor treatment adherence is common in schizophrenia and constitutes a major risk factor for relapse and clinical instability. Long-acting injectable (LAI) antipsychotics are frequently prescribed to mitigate this risk; however, their use can be associated with rare but serious adverse events, including neuroleptic malignant syndrome (NMS), with certain atypical presentations posing considerable diagnostic challenges. We report the case of a 51-year-old patient with schizophrenia who, during hospitalization for a psychotic relapse following prolonged refusal of all oral medications, received an

A Rare Case of Suspected Malignant Hyperthermia in a Three-Day-Old Neonate: A Case Report.

Malignant hyperthermia (MH) is a rare, life-threatening pharmacogenetic disorder of skeletal muscle calcium regulation, most commonly triggered by volatile anesthetics and depolarizing muscle relaxants. While pediatric patients are at increased risk compared to adults, MH is exceedingly rare in neonates, and its clinical presentation may differ by age group, with hyperthermia and generalized rigidity more prominent in the youngest patients. This report describes a three-day-old female who developed a rapid rise in end-tidal CO2, tachycardia, metabolic acidosis, and hyperthermia intraoperativel

A Proposed North American Approach for Genetic Testing of Individuals at Risk for Malignant Hyperthermia.

Malignant hyperthermia, a pharmacogenetic disorder of skeletal muscle, is a potentially fatal reaction triggered by exposure to volatile anesthetics or succinylcholine. Genetic testing of the three known involved genes (RYR1, CACNA1S, and STAC3) has become the first line of testing for malignant hyperthermia susceptibility. However, genetic testing has sensitivity of only up to 70%. Contracture testing may be used to rule out the disorder for some individuals. This study outlines a stepwise approach, using clinical history and assessment as well as interpretation of genetic variants to balance

A Biomimetic Nanoplatform for Near-Infrared-Assisted Heat-Mediated Synergistic Therapy for Glioblastoma.

Cell membrane (CM) camouflage has emerged as a powerful biomimetic strategy for enhancing the precision of targeted drug delivery, offering an effective solution to the major limitations of phototheranostic nanomaterials, which often suffer from reduced therapeutic efficacy and imaging performance due to tumor heterogeneity and microenvironmental delivery barriers. Here, we report a biomimetic phototheranostic nanoplatform engineered for the synergistic treatment of glioblastoma multiforme (GBM) via near-infrared (NIR)-assisted, heat-enhanced chemotherapy.
