Cytoreductive Surgery With or Without HITHOC for Pleural Mesothelioma: A Meta-analysis of Survival and Perioperative Outcomes.

BACKGROUND: Pleural mesothelioma (PM) is a rare and aggressive malignancy with limited overall survival (OS) despite multimodal therapy. Cytoreductive surgery is the standard of care for selected patients. However, locoregional recurrence remains common. Hyperthermic intrathoracic chemotherapy (HITHOC) has been proposed as an adjunct to enhance local disease control. METHODS: A systematic review and meta-analysis were performed according to PRISMA guidelines and registered in PROSPERO (CRD420251132268). This study searched PubMed, Embase, and Cochrane Library up to 18 July 2025. Pooled hazard

First report of malignant hyperthermia syndrome in an American Guinea hog.

A 3-year-old intact female American Guinea hog that was presented for an elective ovariohysterectomy appeared systemically healthy, with no overt medical conditions. Premedication with clonazepam, tiletamine-zolazepam, and xylazine; anesthetic induction with propofol and maintenance with isoflurane with oxygen on spontaneous ventilation; a ketamine infusion; and morphine for analgesia were provided. Approximately 95 min after isoflurane was initiated, the pig acutely became hyperthermic and severely hypercapnic, with hyperkalemia, hyperlactatemia, and severe metabolic and respiratory acidosis.

Evaluation of clinically relevant sevoflurane concentrations in the development of malignant hyperthermia in an Ryr1 mutant mouse model.

BACKGROUND: Malignant hyperthermia (MH) is associated with mutations in the RYR1 gene; however, the effect of volatile anesthetic concentrations on the clinical symptoms of patients with MH is unknown. We investigated the effects of high concentrations of volatile anesthetics on MH development and progression. METHODS: MH mouse models with R2509C mutation in Ryr1, all male, approximately 15 weeks old were administered 5%, 2.5% or 0.6% sevoflurane. The time at maximum temperature, maximum temperature, time at maximum heart rate, maximum heart rate, and time to asystole in the first experiment w

Enhancing Malignant Hyperthermia Response via Rapid Cycle Deliberate Practice: An In Situ Interprofessional Simulation Quality Improvement Project.

Malignant hyperthermia (MH) is a life-threatening anesthetic emergency that requires rapid recognition and intervention to prevent severe complications. Delays in treatment can result in poor patient outcomes, making MH preparedness crucial in perioperative care. The aim of this quality improvement project was to enhance perioperative team preparedness for MH crises through in situ simulation using rapid cycle deliberate practice. The project was implemented over five days in a 14-room operating suite and a 31-bed postanesthesia care unit at an academic medical center in the United States. Int

Formulation and characterization of polyethylenimine-coated magnetopolymeric nanoparticles with potential application in thermal cancer therapies.

Use of superparamagnetic nanoparticles as heat generators contributed to the development of alternatives to conventional cancer therapies. These nanoparticle-based photothermal and/or magnetothermal therapies can kill malignant cells with negligible effects on surrounding tissues. Maghemite nuclei embedded into a poly(ε-caprolactone) nanomatrix were formulated by interfacial polymer disposition, and the resulting nanostructure was coated with polyethylenimine. DLS, TEM, EDX spectroscopy, electrophoretic and FTIR analysis were used to characterize the resulting ≈ 220 nm-sized (core/shell)/shell

Malignant Hyperthermia in Slavonic cohort - clinical and genetic findings beyond standard diagnostics.

BACKGROUND: Malignant Hyperthermia (MH) is a rare, hereditary, life-threatening pharmacogenetic disorder. The genetic background of MH was identified only in approximately half of MH-susceptible (MHS) patients, differing between geographical areas. Variants recognised as being sufficiently functionally characterised to be used in diagnostics for MH are on the “European Malignant Hyperthermia Group list of diagnostic variants”. However, the list may be a bit biased in the sense of rare variants in favour of better-described and published MHS cohorts. Also, the understanding of MH genetics is in

Structural and Dynamic Perturbations in the RyR1 Selectivity Filter Induced by Ala4893 Mutations: Implications for Malignant Hyperthermia and Myopathy.

Mutations at Ala4893 within the selectivity filter (SF) of ryanodine receptor 1 (RyR1) are associated with distinct clinical phenotypes, including malignant hyperthermia (MH) and congenital myopathy. Here, we employed atomistic molecular dynamics simulations to examine how four Ala4893 variants (A4893T, A4893S, A4893P, and A4893G) influence the structure, dynamics, and energetic features of the RyR1 pore domain. The A4893T mutation is associated with an expanded SF geometry, increased local flexibility, enhanced intra-subunit hydrogen bonding, and reduced inter-subunit coupling, features that

Visualisation of magnetic field-induced nanoparticle clusters and mechanical property changes in a breast phantom for inductive moderate hyperthermia.

BACKGROUND: Visualising magnetic nanoparticle (MNP) clusters is important for inductive moderate hyperthermia (IMH) as their formation influences mechanical force transduction and heat generation in malignant tumours. An applied inhomogeneous stationary magnetic field (ISMF) can direct the arrangement of MNP clusters and the force exerted on cancer cells. Herein, we analysed MNP cluster formation and changes in mechanical properties using digital breast tomosynthesis (DBT) and ultrasound shear wave elastography (SWE) for a breast phantom containing MCF-7 breast cancer cells under the influence

Hyperthermia combined with intraperitoneal perfusion of drugs improves malignant ascites caused by gastrointestinal tumors by reshaping the tumor immune microenvironment.

Malignant ascites treatment remains challenging. Hyperthermia is a promising adjunct to intraperitoneal therapies, but its mechanisms are unclear. We aimed to investigate whether combining hyperthermia with endostatin and cisplatin alleviates malignant ascites by reprogramming immune microenvironment. A total of 28 patients with malignant ascites were enrolled and divided into three groups: untreated, recombinant human endostatin plus cisplatin, and hyperthermia combined with recombinant human endostatin and cisplatin. Ascites volume was monitored by B-ultrasound. VEGF, IL-10, and IFN-γ levels

Cascade-driven β-Lapachone cycle in a photothermal/self-oxygenating MXene nanoplatform for synergistic tumor therapy.

The high heterogeneity of malignant tumors and inherent limitations of conventional monotherapies have propelled multimodal synergistic treatments to the forefront of current research. Photothermal therapy (PTT) has garnered significant attention due to its non-invasive nature and favorable safety profile; however, its therapeutic efficacy is often constrained by the limited tissue penetration depth of near-infrared (NIR) light and insufficient targeting capabilities. In recent years, enzyme-based catalytic therapies have emerged as a highly promising strategy for tumor intervention. Two-dimen

Genotype-phenotype relationships in butyrylcholinesterase deficiency: a systematic review.

BACKGROUND: Butyrylcholinesterase (BChE) deficiency is a recessive condition that can cause prolonged paralysis after suxamethonium or mivacurium. Conventional phenotyping using dibucaine and fluoride inhibition can overlook heterozygous carriers. The aim of this systematic review was to identify studies reporting paired BCHE genotype and biochemical phenotype data to propose a risk-assessment protocol. METHODS: We searched PubMed, EMBASE, and the Cochrane Library (1946-2024; PROSPERO CRD420250627891) for human studies reporting both BCHE genotype and biochemical phenotype. Extracted data inc

Malignant Hyperthermia in Sweden: Clinical Presentations and Genetic Findings.

INTRODUCTION: Malignant hyperthermia (MH) is a pharmacogenetic, hypermetabolic and potentially lethal reaction to potent volatile anaesthetics and the muscle relaxant succinylcholine. To improve the understanding of MH, the aim of this retrospective study was to describe the Swedish cohort with respect to clinical manifestations, demographics and genetic findings. METHOD: The Swedish MH Registry covers a total of 2852 individuals belonging to 544 different families investigated for MH since 1980. For this study, the index case in each family was included for further description. RESULTS: In

Hot and Cold: Malignant Hyperthermia Without Fever in a Burn Patient.

Malignant hyperthermia (MH) is a rare, lifethreatening condition typically characterized by a rapid increase in body temperature and muscle rigidity in response to anesthetics. However, patients with extensive burn injuries have impaired thermoregulation and intraoperative heat loss, which can mask this classic hyperthermic response. We present the case of a 38-year-old female with a 23% total body surface area flame burn who developed a severe MH crisis during tangential excision following exposure to sevoflurane. Despite profound hypercapnia, acidosis, hyperkalemia, and extremity rigidity, t

A proposed North American approach for genetic testing of individuals at risk for malignant hyperthermia.

Malignant hyperthermia, a pharmacogenetic disorder of skeletal muscle, is a potentially fatal reaction triggered by exposure to volatile anesthetics or succinylcholine. Genetic testing of the three known involved genes (RYR1, CACNA1S, and STAC3) has become the first line of testing for malignant hyperthermia susceptibility. However, genetic testing has sensitivity of only up to 70%. Contracture testing may be used to rule out the disorder for some individuals. This study outlines a stepwise approach, using clinical history and assessment as well as interpretation of genetic variants to balance

Pathogenic CaV1.1 Variants Cause Hyperpolarizing Shift of Depolarization-Induced Ca2+ Release in Malignant Hyperthermia Susceptibility.

Malignant hyperthermia (MH) is a potentially fatal genetic disorder triggered by certain anesthetics, leading to excessive calcium release in skeletal muscle. While most MH-associated mutations affect the type 1 ryanodine receptor, a Ca2+ release channel in the sarcoplasmic reticulum, variants in CaV1.1-the L-type calcium channel critical for excitation-contraction coupling-have also been implicated. Here, we investigate four novel CaV1.1 variants identified in MH-susceptible individuals (A560T, S879P, F1161L, and D1382V) using a reconstituted Ca2+ release platform in HEK293 cells. Two variant

Anesthesia Management in a Patient With a History of Fulminant Malignant Hyperthermia and a Homozygous RYR1 p.Arg530His Variant Undergoing Laparoscopic Surgery: A Case Report.

BACKGROUND Malignant hyperthermia (MH) is a rare but potentially fatal pharmacogenetic disorder triggered by volatile anesthetics or suxamethonium, most commonly associated with variants in the ryanodine receptor type 1 (RYR1) gene. Patients with confirmed or suspected MH susceptibility are advised to avoid triggering agents and undergo regional anesthesia or total intravenous anesthesia (TIVA). Remimazolam, an ultra-short-acting benzodiazepine approved in Japan for general anesthesia, has been shown in vitro not to increase intracellular calcium concentrations in cells expressing MH-associate
