Probable Malignant Hyperthermia Presenting With Refractory Hypotension in a Young Trauma Patient: A Case Report.

Malignant hyperthermia (MH) is a rare, life-threatening anesthetic complication caused by dysregulated skeletal muscle calcium homeostasis. We report a 25-year-old trauma patient who developed refractory hypotension with rising Etco2 despite progressive increases in minute ventilation, followed by hyperthermia, 2.5 hours after exposure to succinylcholine and sevoflurane. Hemodynamic instability was unresponsive to fluids and vasopressors but reversed rapidly after treatment for probable MH with dantrolene (Ryanodex). This case highlights early hemodynamic instability as a prominent manifestati

Acute akinetic collapse in Parkinson's disease: A retrospective study of sustained immobilization and clinical outcomes.

BACKGROUND: Acute deterioration in Parkinson's disease (PD) often results in emergency hospitalization. While parkinsonism-hyperpyrexia syndrome (PHS) represents the most severe malignant form, acute motor worsening without hyperthermia is frequently encountered. OBJECTIVE: To characterize the clinical features and outcomes of acute sustained immobilization in PD requiring emergency hospitalization. METHODS: We conducted a retrospective observational study of PD patients who developed acute immobilization lasting ≥12 h and required emergency transportation and hospitalization. Clinical chara

Monogenic Syndromes as a Cause of Adverse Drug Reactions in the Russian Population.

Adverse drug reactions (ADRs) remain a major public health issue, and genetic factors contribute importantly to interindividual variability in drug response. Pharmacogenetic testing helps reduce ADR risk by optimizing drug selection and dosage, particularly in monogenic disorders. Whole-exome sequencing of 6739 samples from the Russian population was performed on the DNBSEQ-G400 platform (MGI). Variants in 48 genes were examined, focusing on inherited arrhythmias, enzyme deficiencies (Glucose-6-Phosphate Dehydrogenase Deficiency [G6PDD], Porphyrias), Dravet Syndrome (DS) and Malignant Hyperthe

Integrated theranostic nanoplatform empowers precision cancer care via radionuclide-labeled NIR-II aggregation-induced emission luminogens.

Malignant tumours frequently evade conventional therapies due to their complexity and high recurrence rates. To overcome these limitations, an integrated theranostic nanoplatform was developed through modification of a NIR-II emitting aggregation-induced emission photosensitizer with folic acid and the polydentate ligand (DOTA), followed by radiolabeling with 68Ga. This strategy not only enhances cancer targeting precision but also confers superior positron emission tomography/computed tomography (PET/CT) imaging capabilities to the nanoparticles ([68Ga]DOTA-FA-BBTD-4C NPs). This nanoparticle

Malignant hyperthermia in France: characteristics of 93 diagnosed patients in one French reference centre: A retrospective data analysis.

BACKGROUND: Malignant hyperthermia (MH) is a rare but potentially fatal complication of general anaesthesia, triggered by volatile anaesthetics and depolarising muscle relaxants in genetically predisposed individuals. Although outcomes have improved with better monitoring and dantrolene administration, diagnosis remains challenging. This study aimed to describe patients referred after suspected MH crises in France and to compare clinical features and Larach grading between MH-susceptible and non-susceptible patients. MATERIAL: We conducted a retrospective, monocentric study of patients referr

Desflurane-Induced Delayed Onset of Malignant Hyperthermia: A Case Report.

Malignant hyperthermia (MH) is a rare but life-threatening pharmacogenetic disorder triggered by volatile anesthetics and/or depolarizing muscle relaxants, resulting in a hypermetabolic crisis. We report a case of postoperative fulminant MH in a 26-year-old man who underwent general anesthesia with desflurane and rocuronium for mandibular fracture repair. Despite a history of multiple uneventful anesthetics, the patient developed acute hypercapnia, tachycardia, muscle rigidity, and rapid hyperthermia during emergence, following sugammadex administration. Initial treatment for presumed bronchos

A tracheal stent integrating sustained release of paclitaxel/resiquimod and hyperthermia potentiates systemic antitumor immunity for enhanced malignant tracheal stenosis treatment.

Respiratory distress and tumor progression in malignant tracheal stenosis are both life-threatening. Anticancer drug-releasing tracheal stents achieve long-term localized chemotherapy to the tracheal tumor while dilating the airway; however, they cannot directly kill metastases. Furthermore, the anti-inflammatory stage during the chronic inflammatory response caused by stent placement reduces the effectiveness of stent-mediated chemotherapy. In this study, we propose a paclitaxel (PTX), resiquimod (R848) and Fe3O4 nanoparticles co-loaded tracheal stent (PTX/R848 stent), which exhibited self-ex

Multifactorial anticholinergic toxicity-like presentation and malignant parkinsonism in Perry syndrome.

Perry syndrome is a rare autosomal dominant neurodegenerative disorder characterized by parkinsonism, depression, weight loss, and central hypoventilation. Early diagnosis is challenging because presentations are heterogeneous. A 51-year-old man presented with hyperthermia, impaired consciousness, tachycardia, and gastrointestinal hypomotility suggestive of anticholinergic toxicity. Despite therapeutic fesoterodine levels, he developed hypercapnia requiring mechanical ventilation and later exhibited rigidity with marked creatine kinase elevation, consistent with malignant parkinsonism. Dopamin

The role of capacitive hyperthermia as an adjunct treatment in oncology: a systematic review of randomized phase III trials.

INTRODUCTION: Capacitive hyperthermia (cHT) has been established for superficial tumors but the efficacy of cHT for intermediate or deep-seated tumors remains a subject of debate. This review is an evaluation of the current clinical evidence on the effectiveness of cHT in the treatment of tumors that are located deeper within the body. METHODS: A systematic review was conducted in accordance with the PRISMA guidelines of Phase III randomized controlled trials that included cancer patients of all ages and sexes with confirmed malignant tumors undergoing radiotherapy (RT) and/or chemotherapy (C

Plasmonically Reinforced Self-Sufficient Nanozymes Dysregulating Redox Homeostasis for Augmented Cascade Catalytic Oncotherapy.

Tumor nanocatalytic therapy represents a promising transformative technology for treating malignant tumors, yet is often limited by insufficient catalytic activity and adaptive tumor microenvironment (TME) resistance. Herein, we develop a plasmonic Pt-CuO2 nanozyme that integrates plasmonic enhancement with self-sufficient H2O2 supply for augmented cascade catalytic therapy. The nanozyme comprises Pt nanoparticles (NPs) and CuO2 nanodots co-deposited on Au nanostars and encapsulated within PEGylated ZIF-8 metal-organic frameworks (MOFs). It exhibits strong near-infrared localized surface plasm

Neuroleptic Malignant Syndrome in a Young Male on Clozapine: A Diagnostic and Therapeutic Challenge.

BACKGROUND: Neuroleptic malignant syndrome (NMS) is a rare but potentially life-threatening adverse reaction associated with antipsychotic medications. It is characterized by hyperthermia, severe muscle rigidity, altered mental status, autonomic instability, and elevated creatine kinase (CK) levels. Although commonly linked with first-generation antipsychotics, it may also occur with atypical agents such as clozapine. CASE PRESENTATION: We report the case of a 34-year-old male with a history of psychiatric illness receiving long-term clozapine therapy who presented with altered sensorium and

Primary myotubes as an alternative to the calcium induced calcium release test in diagnosing malignant hyperthermia susceptibility.

BACKGROUND: Malignant hyperthermia (MH) is caused by the dysregulation of calcium (Ca2+) in skeletal muscle cells. In this study, we evaluated the accuracy of a method for diagnosing susceptibility to MH using myotubes as an alternative to Ca2+-induced Ca2+ release rate (CICR) test. METHODS: In this retrospective study, we retrospectively analyzed Ca2⁺ kinetics data obtained using fura-2/AM from myotubes that had been cultured from remnant muscle fibers of 98 patients who had previously undergone CICR testing for MH susceptibility in Japan. The 50% effective concentrations (EC50) of caffeine

Emerging phototherapeutic roles of covalent organic frameworks in malignant cell ablation, wound healing, and pathogen control.

Covalent organic frameworks (COFs) represent highly promising scaffolds for biomedical applications, notably in malignant cell ablation, infected wound healing, and antimicrobial phototherapies. Reticular design precisely controls the pore size, linkages, and dimensionality of COFs, enabling tailored functions such as pH/redox-responsive bonds. This review focuses on three application areas in which COFs have been most thoroughly investigated: phototherapeutic tumor ablation, antibacterial wound disinfection and biofilm disruption, and broader pathogen control. A key challenge in biomedical CO
