Can Pillow Use Prevent Postdural Puncture Headache? A Critical Review.

Postdural puncture headache (PDPH) is a recognized complication of neuraxial anesthesia that follows unintentional dural puncture during epidural techniques or intentional dural puncture during spinal anesthesia and other neuraxial procedures. It has traditionally been managed with a period of mandatory supine bed rest and pillow restriction, a practice grounded in the historical belief that recumbency limits ongoing cerebrospinal fluid (CSF) leakage, thereby preventing headaches. This "no-pillow" instruction remains widely taught and practiced, including among clinicians, despite an absence o

Operating Room Air Filtration-Sterilization and Surgical Infections: The ORACLE Randomized Trial in 110,538 Surgeries.

BACKGROUND: Surgical infections are the most common nosocomial infections and cause substantial morbidity and mortality. Operating rooms are relatively clean, but far from sterile. An unknown fraction of infections result from wound contamination by air-borne bacteria. We therefore tested the hypothesis that supplemental air purification reduces wound infections and related complications. METHODS: Operating rooms, our clusters, were each randomized in each 4-week blocks to either active or identical-looking sham supplemental air sterilization. All investigators, clinicians, and patients were

Organ damage after hemorrhagic shock: roles of anesthesia, shock duration, and assessment time.

Hemorrhagic shock (HS) is a condition associated with organ failure. Animal models are widely used to study pathophysiology and potential therapies. Our aim was to clarify how anesthetic choice, shock duration, decompensation phase length, and time post-reperfusion influence plasma damage markers in HS. For this, male rats were anesthetized with either thiopental or ketamine/xylazine and subjected to HS. Blood pressure was reduced to 40 mmHg for 60 or 90 minutes, followed by reperfusion with shed blood. Blood samples were collected 4 or 24 hours after reperfusion. Liver and renal damage were o

Clavipectoral fascia block associated with intermediate cervical plexus block versus intermediate cervical plexus block alone in clavicle surgeries: a randomized clinical trial.

BACKGROUND: Midshaft clavicle fractures account for approximately 5% of all bone fractures and frequently require surgical fixation to restore shoulder function. Regional anesthesia techniques are commonly used to improve perioperative analgesia for these procedures; however, the optimal strategy for clavicle surgery remains uncertain. This randomized clinical trial aimed to evaluate whether adding a clavipectoral fascia plane block (CFPB) to an intermediate cervical plexus block (ICPB) improves immediate postoperative analgesia compared with ICPB alone. METHODS: In this prospective randomize

Intra-operative oxygen concentration could affect postoperative morbidity-results of a randomised controlled feasibility trial (PULSE Ox).

The World Health Organisation recommends that all anaesthetised patients receive FIO2 0.8 during surgery to reduce the risk of surgical site infections but did not consider the effect this might have on other clinical outcomes. A Cochrane systematic review concluded that insufficient evidence exists for routinely administering high oxygen concentrations intra-operatively to reduce surgical site infections, and that doing so might increase mortality. A feasibility trial was conducted looking at the feasibility of controlling intra-operative FIO2 and measuring systemic redox state. Here we explo

Limiting collateral damage: Nosocomial infections in patients with temporary mechanical circulatory support.

The use of temporary mechanical circulatory support (tMCS) and extracorporeal membrane oxygenation (ECMO) for patients with cardiopulmonary compromise is on the rise. While these tMCS systems provide life-saving interventions, they also pose a significant risk of nosocomial infections, which are associated with higher mortality rates. Patients receiving tMCS are critically ill and require intensive care, increasing their vulnerability to infections. Common nosocomial infections include bloodstream infections (BSI) and ventilator-associated pneumonia (VAP) in patients with ECMO and wound-relate

Retrospective Comparison of the Pericapsular Nerve Group Block Versus the Fascia Iliaca Block for Hip Fractures in the Emergency Department.

INTRODUCTION: Hip fractures are a common injury among the elderly and a substantial cause of morbidity and mortality worldwide. Pain management is an essential component of care for patients with hip fractures, particularly for improving patient outcomes. Ultrasound-guided nerve blocks such as the fascia iliaca block (FIB) are evidence-based means of providing perioperative analgesia and reducing opioid use. Newer nerve blocks, such as the pericapsular nerve group (PENG) block, may serve as alternatives to the FIB. The objective of this study was to retrospectively compare pain reduction follo

Comparison of the efficacy of pentazocine and tramadol for reducing emergence agitation in children after general anesthesia: a retrospective cohort study.

BACKGROUND: Emergence agitation (EA) is a common postoperative complication in pediatric anesthesia that can lead to serious safety events and increased healthcare burden. Both pentazocine and tramadol are frequently used for postoperative analgesia in children, but their comparative effectiveness in preventing EA remains unknown. This study aimed to compare the efficacy of a single intravenous dose of tramadol versus pentazocine for reducing EA in children undergoing tonsillectomy and adenoidectomy. METHODS: This single-center retrospective cohort study included 240 children aged 2-12 years

Modified Suzuki Frame Fixation for the Treatment of Dorsal Proximal Interphalangeal Joint Fracture-Dislocation.

BACKGROUND: The proximal interphalangeal (PIP) joint is critical for hand function, contributing substantially to grasping and overall dexterity1-3. Injuries to this joint, often caused by axial loading, can lead to various fracture patterns and dislocations, resulting in long-term complications such as pain, stiffness, reduced range of motion, arthritis, and instability1,2. Traditional surgical approaches for unstable dorsal PIP joint fracture-dislocation include dorsal block pinning, open reduction and internal fixation (ORIF), hemi-hamate or hemi-capitate arthroplasty, and external fixation

Heart Transplantation in Adult Fontan Patients: New Insights from Fontan-Associated Liver Disease Beyond Traditional Predictors.

BACKGROUND: Fontan patients are considered high risk for heart transplantation (HT) owing to their complex anatomy, multiple prior chest entries, and universal Fontan-associated liver disease (FALD). The impact of FALD and other preoperative factors remains, however, uncertain. OBJECTIVES: We sought to identify factors associated with 90-day mortality after isolated HT in adults with Fontan circulation, with particular focus on the relationship between liver-related parameters and post-transplant outcomes. METHODS: A retrospective cohort study of all adult Fontan patients undergoing isolated

Children's Surgery: The Missing Link in the Global Child Survival Agenda.

Global child health is at an inflection point. Under-five mortality has fallen substantially over recent decades, yet progress is slowing and preventable deaths and disability remain concentrated in low- and middle-income countries. As mortality from vaccine-preventable diseases and other infections declines, birth defects and other surgically treatable conditions account for an increasing share of the residual burden. For many children, access to timely, safe and affordable surgical, anesthesia and perioperative care is the difference between survival, disability and death. Yet these services

Comparison of Shoulder Field Anesthesia (SFA) with Interscalene Block (ISB) for Analgesia After Arthroscopic Shoulder Surgery: A Randomized Noninferiority Trial.

BACKGROUND: An interscalene brachial plexus block (ISB) is commonly used for analgesia after shoulder surgery but is associated with neurologic and respiratory complications, including hemidiaphragmatic paralysis. Surgeon-performed shoulder field anesthesia (SFA) is a landmark-guided, phrenic nerve-sparing technique that may provide effective analgesia with fewer complications. The purpose of this study was to compare SFA with ISB for pain control after arthroscopic shoulder surgery. We investigated whether SFA would provide noninferior analgesia as measured by cumulative 48-hour postoperative

Clinical Application and Safety of Intrathecal Adjuvants in Single-Shot Thoracic Segmental Spinal Anesthesia: A Narrative Review.

Thoracic segmental spinal anesthesia (TSSA) is an emerging regional anesthesia (RA) technique. TSSA with local anesthetic (LA) alone is associated with a relatively short duration of surgical anesthesia and postoperative analgesia, which may limit its clinical utility. Intrathecal adjuvants have been widely used in lumbar spinal anesthesia (LSA) to prolong block duration and improve block quality. However, their role in TSSA remains poorly defined. This review summarizes commonly used intrathecal adjuvants during TSSA, their doses, adverse effects, and safety considerations. A comprehensive li

Feasibility and Safety of Intraoperative Abdominal Warming During Spinal Surgery in the Prone Position: A Pilot Study.

BACKGROUND: Inadvertent perioperative hypothermia caused by general anesthesia leads to several complications, including shivering, coagulopathy, surgical site infections, and delayed wound healing. Prone positioning for spine surgery severely restricts the available body surface area for warming, complicating intraoperative temperature management. This study investigated a novel approach utilizing the anatomical space beneath the chest and abdomen created by a standard four-point bolster support to maintain intraoperative core body temperature using standard pediatric underbody forced-air war

Comparison of Sugammadex and Neostigmine for Reversal of Vecuronium-Induced Neuromuscular Blockade in Pediatric Patients: A Prospective Randomized Study.

Background and objective Neuromuscular blocking agents (NMBAs) used during general anesthesia require effective reversal to prevent residual paralysis. Neostigmine, the conventional reversal agent, is associated with slower recovery and muscarinic side effects. Sugammadex selectively encapsulates amino-steroidal NMBAs, providing rapid and predictable reversal. This study compared the efficacy, safety, and recovery profile of sugammadex and neostigmine for reversal of vecuronium-induced neuromuscular blockade in children. Methods In this prospective, randomized, double-blind trial, 100 children

Syndrome of the trephined following burr hole surgery, decompressive craniectomy/cranioplasty, or craniotomy: contemporary vision from a military neurosurgery department.

The syndrome of the trephined (ST, 1939) described headaches, scar discomfort, and anxiety following cranial surgery. The sinking skin flap syndrome (1977) described neurological worsening after decompressive craniectomy (DC) and immediate improvement following cranioplasty (CP). They have been mistakenly confused with the other since 1984. The aim was to describe the ST. Symptoms were screened among 20 chronic subdural hematome (CSDH) patients operated on by neurosurgeon-1, and compared to (1) DC-CP patients operated on by neurosurgeon-1, (2) any craniotomy patients followed-up by neurosurgeo
