Intelligent thermoregulation strategy versus routine thermal management on myocardial injury after noncardiac surgery during gastrointestinal tumor resection in elderly patients (SAGE): a multicenter, parallel-group, superiority trial.

BACKGROUND: Inadvertent perioperative hypothermia is associated with adverse cardiovascular outcomes, but adequately powered trials using high-sensitivity troponin endpoints is lacking. The SAGE trial evaluates whether an intelligent, closed-loop thermoregulation strategy reduces myocardial injury after noncardiac surgery (MINS) in high-risk elderly patients. METHODS: This prospective, multicenter, parallel-group, randomized controlled superiority trial conducted at seven tertiary hospitals in China. Participants aged ≥65 years scheduled for elective gastrointestinal tumor resection under gen

Emergent Cervical Laminoplasty in a Patient with Chronic Thromboembolic Pulmonary Hypertension.

Patients with chronic thromboembolic pulmonary hypertension (CTEPH) present with a variable degree of pulmonary hypertension (PH), right ventricular dysfunction, and hypoxia. It is challenging if the patient requires an emergent surgical procedure other than pulmonary endarterectomy. Herein, we present a unique case of cervical laminoplasty for symptomatic cervical spinal cord compression in a patient with CTEPH and severe PH.

Redefining mechanical ventilation in thoracic anesthesia: flow-controlled ventilation from bench to bedside-a narrative review.

One-lung ventilation (OLV) remains a cornerstone of thoracic anesthesia but continues to challenge perioperative respiratory management. Restriction of ventilation to a reduced functional lung volume promotes ventilation-perfusion mismatch, alveolar instability, and exposure of the dependent lung to increased mechanical stress. Although contemporary lung-protective ventilation strategies have evolved considerably, postoperative pulmonary complications remain common, highlighting the need for innovative approaches to respiratory support. Flow-controlled ventilation (FCV) introduces a distinct p

Management of Bilateral Subacute Subdural Hematomas with Concomitant Subarachnoid Hemorrhage in a Diabetic Patient with HIV in Cameroon: A Case Report.

Bilateral subdural hematomas occurring simultaneously with subarachnoid hemorrhage in a traumatic context are rare and pose considerable diagnostic challenges. Comorbidities such as diabetes and HIV may further worsen prognosis and complicate management. This case underscores the vital importance of early diagnosis and multidisciplinary care in patients with subacute subdural hematomas and subarachnoid hemorrhage, particularly when compounded by comorbidities in resource‑limited settings. We report the case of a 44-year-old housewife who was diabetic and living with HIV. She presented with sev

Pregnancy Complicated by Citrin Deficiency and Gestational Diabetes Mellitus: Perioperative Metabolic Management and Postoperative Analgesic Challenges Following Caesarean Delivery.

Citrin deficiency is a rare autosomal recessive metabolic disorder caused by pathogenic variants in the SLC25A13 gene. Pregnancy in affected women is uncommon and presents unique challenges because conventional obstetric management may conflict with metabolic requirements. The coexistence of gestational diabetes mellitus (GDM) introduces additional complexity. A 25-year-old, gravida 2 para 0+1 woman with genetically confirmed citrin deficiency (homozygous SLC25A13 c.1763G>A mutation) was managed jointly by obstetric and inherited metabolic disease (IMD) teams. Her pregnancy was complicated by

Iatrogenic Horner syndrome following internal jugular vein catheterization in an infant: a case report and literature review.

INTRODUCTION: Horner syndrome (HS) is a rare but recognized complication of internal jugular vein (IJV) catheterization in children, resulting from injury to the cervical sympathetic pathway. Neonates and infants are particularly vulnerable due to the compact anatomy of the neck. PRESENTATION OF THE CASE: We report a 38-day-old female infant who developed ipsilateral HS following open surgical placement of a Broviac catheter via the right IJV. The catheter was inserted under local anesthesia with sedation for prolonged intravenous therapy. Within 24 hours postoperatively, the patient presente

Navigating Electroconvulsive Therapy in Patients on GLP-1 Receptor Agonists: Balancing Safety, Compliance, and Treatment Consistency: A Case Report.

Electroconvulsive therapy (ECT) is typically performed under general anesthesia without a secured airway, placing patients with retained gastric contents at increased risk of aspiration. As more patients present for ECT while taking GLP-1 receptor agonists, standard fasting requirements pose unique safety and patient-experience challenges. A 24-hour clear-liquid diet every other day is burdensome, while frequent rapid sequence intubations may prolong treatment and recovery. To address this, ECT and anesthesia leadership developed a novel pre-ECT fasting protocol for patients on GLP-1 RAs. We r

Persistent Supraventricular Tachycardia Following Central Venous Catheter Insertion: When Withdrawal of the Mechanical Stimulus Is Not Enough.

Cardiac arrhythmias are a recognised complication of central venous catheterisation and are usually attributed to mechanical irritation of the endocardium by the guidewire or catheter tip. These arrhythmias typically resolve following withdrawal of the mechanical stimulus. We report a 42-year-old man with heart failure with reduced ejection fraction and diabetic ketoacidosis who developed an abrupt regular narrow-complex tachycardia at approximately 217 beats/min immediately following advancement of a right internal jugular central venous catheter. The arrhythmia was associated with transient

Pulmonary Herpes Simplex Virus Reactivation in Invasively Mechanically Ventilated Patients With COVID-19 in Intensive Care Unit.

Patients in intensive care units (ICU) under mechanical ventilation, particularly those with SARS-CoV-2 infection, are highly susceptible to Herpes simplex virus (HSV) reactivation. Given the lack of data on the SARS-CoV-2 Omicron and Delta variants, this study aimed to assess the prevalence of pulmonary HSV reactivation in mechanically ventilated patients with SARS-CoV-2 pneumonia. The impact of HSV replication on SARS-CoV-2 replication, and conversely, was investigated, along with the susceptibility of HSV strains to antiviral therapies. This retrospective study included 381 mechanically ven

Silent Intraoperative Right Coronary Artery Occlusion at a Prior Stent Site Progressing to Ventricular Fibrillation During Hip Hemiarthroplasty: A Case Report.

Perioperative myocardial infarction may occur without typical ischemic symptoms in elderly patients undergoing hip fracture repair. We describe the case of an 86-year-old woman with hypertension, coronary artery disease, and prior right coronary artery stenting who underwent right hip hemiarthroplasty under spinal anesthesia after clopidogrel had been withheld for one week. Continuous electrocardiographic monitoring showed progressive ST-segment elevation first recognized by the anesthesia nurse, who verified the finding and immediately informed the anesthesiology team. ST-segment elevation pr

Delayed Postoperative Malignant Hyperthermia After Total Intravenous Anesthesia: A Case Report.

Malignant hyperthermia (MH) is a rare pharmacogenetic disorder typically triggered by volatile anesthetics or succinylcholine, but its occurrence after total intravenous anesthesia (TIVA) remains exceptional and underreported. Propofol-based TIVA is recommended for MH-susceptible patients to avoid known triggers, yet absolute prevention cannot be guaranteed. A 24-year-old male with prior suspected MH underwent spine surgery under propofol-remifentanil TIVA after strict preventive preparation. Despite stable intraoperative parameters, he developed fever, tachycardia, rigidity, and myoglobinuria

Eleven-Year Undiagnosed Bronchial Foreign Body in an Adult: A Case Report.

Foreign body aspiration (FBA) in adults is rare, and tooth aspiration represents an exceptionally uncommon subset. Even rarer is the prolonged retention of a tooth in the left main bronchus, which is anatomically less favorable for aspiration than the right. However, individual airway variations may predispose to such occurrences. We report the case of a 56-year-old woman with an 11-year history of recurrent "pneumonia-like" episodes characterized by chronic productive cough, intermittent fever, hemoptysis, and pleuritic chest pain. She had been repeatedly treated with empirical antibiotic reg

To block or not to block: A propensity score matched analysis of TAP blocks in ventral hernia repairs.

BACKGROUND: Transversus abdominis plane (TAP) blocks reduce opioid use and improve pain control. We aimed to assess whether this improves pain control and early post-operative complications after ventral hernia repair (VHR). METHODS: We performed a retrospective study using the Abdominal Core Health Quality Collaborative (2014- 2023) comparing patients with TAP blocks. Primary outcomes included length of stay (LOS) and post-operative pain at 1-, 3-, 6-, and 12 months. Secondary outcomes included 30-day rates of ileus, deep venous thrombosis, pneumonia, and intubations. RESULTS: Among 21,608

Plasma proteomics defines two reproducible subphenotypes of sepsis-associated acute kidney injury with distinct outcomes.

Sepsis is the leading cause of acute kidney injury in critically ill patients, and this complication carries a high risk of death. The biology underlying it varies between patients, which may explain why treatments have not succeeded. Here we show, using an ensemble method that groups patients by patterns across hundreds of blood proteins in three independent groups of patients, that sepsis-associated acute kidney injury comprises two reproducible subphenotypes. One subphenotype shows widespread activation of inflammation, metabolism, and oxidative stress; the other shows a quieter profile. Pa

Unraveling the Enigma of Postablation Rhabdomyolysis: Diagnostic Dilemmas and Therapeutic Strategies.

Rhabdomyolysis is characterized by the breakdown of skeletal muscle tissue and the release of intracellular contents into the circulation, and it poses diagnostic challenges due to its heterogeneous presentation and potential for serious complications. We present a case of a 44-year-old male with a history of hypertension, anxiety, and persistent atrial fibrillation (AF) who developed rhabdomyolysis the morning after a scheduled catheter ablation. He presented with myalgias, muscle cramps, weakness, and brown-colored urine; laboratory studies revealed a creatine phosphokinase (CPK) of 23,757 U

Management of Neuromuscular Blocking Drug Extravasation With Sugammadex: A Case Report.

Extravascular infiltration of neuromuscular blocking drugs (NMBDs) is a perioperative complication that may result in delayed and recurrent paralysis due to gradual systemic absorption from subcutaneous tissue. We report the case of a 64-year-old female who experienced inadvertent peripheral intravenous infiltration (PIV) during intraoperative administration of vecuronium and rocuronium. Following routine reversal with sugammadex, she developed recurrent episodes of respiratory compromise and profound weakness in the post-anesthesia care unit, consistent with delayed absorption of the extravas

Digital Twin-Assisted Risk Disclosure in Adults Undergoing Elective Bronchoscopy: Multicenter Randomized Controlled Trial.

BACKGROUND: Risk disclosure before bronchoscopy should provide sufficient information for informed consent, but detailed text-based risk disclosure may increase procedural anxiety. Patient-specific visualization with a digital twin-based bronchoscopy simulator may help patients understand bronchoscopy and its risks in a more individualized manner. OBJECTIVE: This study evaluated whether digital twin-assisted risk disclosure reduces prebronchoscopy anxiety and improves postbronchoscopy satisfaction compared with conventional risk disclosure in adults scheduled for elective bronchoscopy. Graded Epidural Anaesthesia for Caesarean Section in the Presence of an Untreated Spetzler-Martin Grade V Cerebral Arteriovenous Malformation With Radiological Progression Over Six Years: A Case Report.

Cerebral arteriovenous malformations (AVMs) are rare during pregnancy, when the haemodynamic and hormonal changes of gestation may increase the risk of rupture. Lesions that are large, located in eloquent regions, and drained by deep venous channels represent a surgically high-risk subtype, carry substantial surgical morbidity, and are frequently managed conservatively when unruptured. We describe a 33-year-old G3P1L1A1 parturient at 36 weeks and six days of gestation with an untreated right parieto-temporo-occipital AVM, who declined intervention across three encounters and underwent an elect
