INTRODUCTION: Infiltration between the popliteal artery and the capsule of the knee (IPACK) and the selective tibial nerve block both provide analgesia for the posterior aspect of the knee after anterior cruciate ligament reconstruction. This randomised controlled triple-blinded trial tested the hypothesis that IPACK provides superior analgesia to a selective tibial nerve block after anterior cruciate ligament reconstruction.
METHODS: Eighty patients having anterior cruciate ligament reconstruction under general anaesthesia with a femoral triangle block were allocated randomly to receive eith

Postoperative pulmonary complications (PPCs) remain common and are exacerbated by perioperative systemic inflammation. Dexamethasone, administered to millions of patients each year for prophylaxis against postoperative nausea and vomiting, effectively attenuates systemic inflammatory responses, rendering it a plausible adjunct for PPC prevention. In a recent issue of the British Journal of Anaesthesia, Zorrilla-Vaca and colleagues present a well-designed secondary analysis of the individualised, perioperative open-lung ventilation strategy trials showing that antiemetic doses of dexamethasone

Penetrating tracheal injuries present unique challenges for airway management, ventilation, and surgical repair due to the risk of rapid airway decline. We present a case of a 34-year-old male who sustained an arrow injury to the neck. Given the risk of airway compromise with induction or movement, venovenous extracorporeal membrane oxygenation (VV ECMO) was initiated prior to anesthesia. Awake bronchoscopy confirmed tracheal injury with an embedded arrow. The patient underwent successful tracheal repair and robotic arrow extraction under ECMO support. He was weaned off ECMO intraoperatively a

Background Trigger finger (stenosing flexor tenosynovitis) is a common cause of hand pain and disability. Percutaneous release is a minimally invasive alternative to open surgery, but few guide-assisted, low-cost techniques have been described for resource-limited settings. This study evaluated the long-term outcomes, recurrence rate, safety, and patient satisfaction of a modified percutaneous trigger finger release using a custom-made Kirschner wire (K-wire) guide. Methods In this retrospective observational case series, 13 patients (26 trigger digits) treated between May 2016 and May 2026 at

Introduction Rib fractures cause significant morbidity due to pain-induced respiratory issues. While regional anesthesia (RA) provides excellent pain relief, evidence for its administration outside of theatres remains limited. This study aimed to evaluate the feasibility and clinical impact of out-of-theatre RA for traumatic rib fractures. Methods A retrospective cohort study was conducted at a district general hospital and regional trauma unit between January 2021 and December 2023. The primary outcomes were the feasibility and complications of RA. Secondary outcomes were mortality and length

BACKGROUND: Dog bite injuries (DBIs) are a common cause of facial trauma injuries requiring oral and maxillofacial surgery (OMS) input. They can present a dual burden of physical trauma and long-term psychological morbidity, yet these effects remain underrecognized and under-supported.
PURPOSE: This study aims to quantify the OMS workload associated with DBI's and identify the physical and psychological outcomes experienced by victims of these injuries.
STUDY DESIGN, SETTING, SAMPLE: Retrospective case series of dog-related injuries presenting to the OMS department from January 2020 to Febru

Intracranial hypertension (IH) is a frequent and serious complication of acute brain injury, strongly influencing morbidity and mortality. While invasive intracranial pressure (ICP) monitoring is standard, it has limitations. Intracranial compliance (ICC) offers additional insight into the brain's ability to handle changes in volume. Noninvasive ICP waveform analysis, such as the peak 2 (P2)/peak 1 (P1) ratio and time to peak (TTP), shows promise in evaluating ICC and detecting IH. The brain4care (B4C) system provides continuous, noninvasive monitoring for neurocritical care. We systematically

BACKGROUND: Status epilepticus (SE) is any type of seizure that lasts more than five minutes or two or more seizures which occur within a five-minute period without complete inter-ictal recovery of consciousness. Non-convulsive status epilepticus (NCSE), on the other hand, is any SE that occurs without any prominent motor activities. Due to its subtle manifestation, it's often underrecognized and underdiagnosed in various populations. This systematic review with meta-analysis aimed to assess its prevalence in patients with altered mental status (AMS).
METHODS: Three databases - PubMed, Cochra

BACKGROUND: Testosterone may assist in traumatic hemostasis given its well-established role in erythropoiesis, platelet activation, and vasoconstriction; however, whether exogenous testosterone improves acute outcomes in traumatic hemorrhage is unknown.
METHODS: Following PRISMA 2020, we registered the protocol in PROSPERO (CRD42024617294; 12 October 2024). MEDLINE, EMBASE (both via Ovid), and PubMed were searched from inception to November 2024 for studies that investigated the acute effects of testosterone administration in adults with hemorrhage. Two reviewers independently screened citati

Right-to-left intracardiac shunting may complicate patent foramen ovale, with percutaneous closure being the standard treatment. We present the case of a 63 year old man admitted for percutaneous patent foramen ovale (PFO) closure due to severe right-to-left shunting. Despite treatment with diuretics and noradrenaline, he developed persistent hypoxemia with pulmonary edema. Intubation was initially avoided because of the risk of worsening shunt physiology and anticipated airway management difficulties. To optimize oxygenation while maintaining femoral access for intervention, awake veno-venous

Artificial intelligence (AI) may improve perioperative risk prediction by integrating complex clinical and physiological data, but its performance and readiness for clinical use remain uncertain. We evaluated AI models predicting postoperative nausea and vomiting (PONV), perioperative hypotension, prolonged intensive care unit (ICU) stay, in-hospital mortality, and neurological complications after general anesthesia. We searched bibliographic databases from inception to June 2026 for studies developing or validating AI-based prediction models in surgical patients undergoing general anesthesia.

Perioperative neurocognitive disorders (PND) are a prevalent and disabling complication in elderly patients after anesthesia and surgery, imposing a growing clinical burden in aging societies. Despite its high incidence, effective therapies for PND remain unavailable. Surgery-induced neuroinflammation, characterized by excessive oxidative stress, microglial activation, and blood-brain barrier (BBB) disruption, plays a central role in the development of PND. Here, we develop a PCN biomimetic nanozyme platform based on homologous, highly adhesive neutrophil membrane-encapsulated Pd/hCeO2 nanopar

BACKGROUND: Sugammadex minimizes the residual neuromuscular block risk. By enhancing the function of expiratory muscles, it improves the clearance of secretions and helps prevent lung collapse, leading to a reduction in early postoperative respiratory issues. The study aims to compare the efficacy of sugammadex in promoting the recovery of expiratory muscle power to that of neostigmine following extubation.
STUDY OBJECTIVES: The main objective of this study was to evaluate the effectiveness of sugammadex and neostigmine as a reversal of neuromuscular block, assessed through ultrasonographic e

BACKGROUND: Regional anesthesia (RA) is increasingly used due to its safety and effectiveness in reducing postoperative pain and opioid use. As in other areas of clinical research, transparent and consistent outcome reporting is important for interpreting trial evidence. This study assessed the extent and nature of temporal changes to prespecified outcomes in interventional RA trials registered on ClinicalTrials.gov after the implementation of the Final Rule and the Food and Drug Administration Amendments Act 801.
METHODS: We conducted a registry-based cross-sectional analysis of RA trials in
