Pre-hospital time and risk of perforation in patients with acute appendicitis in 112 countries (AlliGatOr): an international, prospective, observational cohort study.

BACKGROUND: Acute appendicitis is the most common surgical emergency worldwide. Concern that delayed treatment might increase perforation can lead to unnecessary surgery and high rates of negative appendicectomy, where a normal, non-inflamed appendix is removed. However, whether perforation is related to pre-hospital time (symptom onset to first surgical assessment) has not been explored in contemporary global data. METHODS: We conducted an international, multicentre, prospective, observational cohort study (AlliGatOr) of patients undergoing emergency appendicectomy for suspected acute append

Mechanisms underlying rheumatic heart disease immune-mediated pathogenesis: T-cell responses, cytokine networks, and immune dysregulation as drivers of valvular injury.

Acute rheumatic fever is an immune-mediated sequela of Streptococcus pyogenes (group A Streptococcus) infection, presenting with fever, arthritis, chorea, rash, and, crucially, carditis. Without effective secondary prophylaxis and care, acute rheumatic fever can progress to chronic rheumatic heart disease, a leading yet under-recognised cause of cardiovascular morbidity and death that disproportionately affects children and young adults aged 5-24 years in low-resource settings. Robust evidence implicates the role of antigenic mimicry-particularly between streptococcal M protein and cardiac or

Outcomes of simultaneous microsurgical replantation of complete amputations at the distal third of the right forearm and left wrist: a case report and 22-month follow-up.

Microsurgical replantation of simultaneous bilateral upper extremity amputations is a complex procedure requiring a highly coordinated multidisciplinary team. Functional outcomes depend on anatomical level of injury, duration of ischemia, extent of crushing, degree of contamination, and quality of postoperative rehabilitation. We report a 45-year-old male industrial worker who sustained complete simultaneous amputations through the distal third of the right forearm and at the left wrist following an industrial paper-cutting machine accident. Total ischemia time was 8 h. Two microsurgical teams

Intermittent Hypoxemia and Brain Injury Biomarker S100B in Preterm Infants.

UNLABELLED: <p>Introduction: Intermittent hypoxemia (IH) is common in preterm infants and linked to brain injury. S100B is a glial-derived protein that rises early after neural injury and can be measured noninvasively in urine. We evaluated the relationship between IH burden and urinary S100B in preterm infants of ≤32 weeks' gestation. METHODS: Preterm infants of ≤32 weeks' gestation were prospectively enrolled. Oxygen saturation was continuously monitored, and IH profiles were quantified using validated algorithms. Urine S100B was measured by ultrasensitive immunoassay and normalized for uri

Injury Severity and Free-Water Volume Fraction Predict Regional White Matter Atrophy During the First Year Following Moderate-Severe Traumatic Brain Injury.

Traumatic axonal injury is a common endophenotype of traumatic brain injury (TBI) in which injury to axons is a dominant component. In previous investigations, we identified a significant reduction of white matter (WM) volume from 3 to 12 months post-injury, an increased rate of atrophy during this period associated with a longer duration of post-traumatic amnesia (PTA), widespread reductions in white-matter fractional anisotropy (FA), and elevated free-water (FW) content in a cohort of moderate-to-severe TBI patients. To provide a more comprehensive characterization of the spatiotemporal traj

Clinical Significance of Leakage Sign Patterns in Acute Subdural Hematoma.

The leakage sign (LS), defined as contrast medium extravasation on the 5-minute delayed phase after computed tomography (CT) angiography, has been reported as a predictive imaging marker for hematoma expansion in acute subdural hematoma (ASDH). We recently identified two different patterns of LS: local and diffuse. This study aimed to evaluate the clinical importance of these LS types in predicting brain swelling and determining the appropriate surgical intervention. We retrospectively reviewed 98 patients with LS-positive ASDH on contrast-enhanced CT between January 2013 and September 2024. L

Baseline CT radiomics for early prediction of post-traumatic hydrocephalus in traumatic brain injury: development and temporal validation.

BACKGROUND: Post-traumatic hydrocephalus (PTH) impairs recovery after traumatic brain injury (TBI), yet reliable early risk stratification based solely on baseline assessment remains limited. We aimed to develop and temporally validate a preoperative CT-based radiomics model for early prediction of PTH and to assess the incremental value of clinical predictors. METHODS: In this retrospective single-center cohort, 341 consecutive adults undergoing neurosurgical treatment for TBI were included. Patients treated in 2021-2023 (n = 289) formed the training cohort, and those treated in 2024 (n = 52

Association of D-dimer, fibrin degradation products, and coagulation parameters with progressive hemorrhagic injury following traumatic brain injury: A retrospective cohort study.

This study aimed to evaluate the predictive value of coagulation biomarkers, particularly D-dimer and fibrin degradation products (FDP), in the development of secondary progressive hemorrhagic injury (PHI) following traumatic brain injury (TBI). We conducted a retrospective cohort study based on hospital medical record data. All enrolled TBI patients were divided into a PHI group (n = 51) and a non-PHI group (n = 151) for comparison based on whether PHI occurred during hospitalization. Comprehensive coagulation profiles, including D-dimer, FDP, Prothrombin time, thrombin time, fibrinogen (FIB)

Uncorrected Tetralogy of Fallot Complicated by Brain Abscess and Postoperative Intracranial Haemorrhage: A Case Report.

Cerebral abscess is a rare but potentially fatal complication of cyanotic congenital heart disease, in which chronic hypoxia, polycythaemia and right-to-left shunting allow bacteraemia to bypass pulmonary filtration. A woman in her 20s with uncorrected tetralogy of Fallot presented with severe headache, right-sided weakness and seizures. Examination showed hemiparesis, hypoaesthesia and hyperreflexia; blood tests revealed secondary polycythaemia and leucocytosis. No dental, sinus, otological or respiratory source was identified. Unenhanced computed tomography demonstrated a left parietal space

Mandibular fracture with subsequent sequestrum formation and tooth-root abscess in a 3-year-old mare.

A 3-year-old quarter horse cross mare was presented to the Atlantic Veterinary College (Charlottetown, Prince Edward Island) for evaluation of a persistent draining tract on the ventral aspect of the left mandible, associated with a previous fracture. Radiography and ultrasonography confirmed the presence of a chronic sequestrum separated from the parent bone. Sequestrectomy and surgical debridement of the mandible were carried out, and the horse was discharged. Three months later, the mare was presented again to the Atlantic Veterinary College, as the draining tract had not resolved. A CT sca

Nerve Injury During Thyroid Radiofrequency Ablation: Mechanisms, Prevention, and Practical Management.

Voice change is an uncommon but clinically important complication of thyroid radiofrequency ablation (RFA), because it may indicate injury to the recurrent laryngeal nerve or, less commonly, the vagus nerve. Early recognition and appropriate intervention are important, because neural dysfunction may remain reversible before the occurrence of irreversible structural nerve injury. Thermal nerve injury during thyroid RFA is not always a single instantaneous event. Rather, it may involve direct thermal exposure of the nerves, leading to acute endoneurial or perineural edema, microvascular compromi

The effect of routine intraoperative neuromonitoring on thyroidectomy complications: A propensity score-matched retrospective cohort study.

Intraoperative neuromonitoring (IONM) is commonly used during thyroid surgery to facilitate recurrent laryngeal nerve (RLN) identification and potentially reduce related postoperative complications. However, the efficacy of routine IONM in decreasing complication rates remains debated. In this study, we investigated the impact of IONM on postoperative complications after thyroidectomy. This retrospective single-center study included 214 thyroidectomy operations performed between 2021 and 2024. Patients were divided into 2 groups: Group 1 (n = 107) underwent surgery with routine IONM, and Group

Beyond Counting Words: Verbal Fluency Components and Executive Functions in Traumatic Brain Injury.

BACKGROUND: Verbal fluency (VF) tasks are consistently included in neuropsychological assessment protocols because they provide a rapid and sensitive measure of lexical retrieval and executive functioning across a wide range of neurological conditions. In addition to considering the number of words produced by the individual, examining other variables can provide highly valuable information about their cognitive performance. AIMS: This study examines the clinical utility of looking into specific components of VF tasks by comparing the performance of patients with traumatic brain injury (TBI)

Brain photobiomodulation therapy for acute sport-related concussion: a randomized, single-blinded, pilot clinical trial (RECOVERY study).

BACKGROUND: The current standard of care for concussion relies on symptom management; there are almost no disease-modifying treatments. The RECOVERY study investigated the safety of RGn550 (REGEnLIFE), a novel neurostimulation device combining brain photobiomodulation with a static magnetic field, in sportspeople with acute concussion and described concussion-related parameters in patients receiving this therapy. METHODS: This prospective, randomized (central randomization, 1:1 ratio), single-blind, monocentric, pilot study compared two pulse frequencies (5 Hz and 10 Hz). Adult sportspeople w

Gestational Ketosis Compromises Nephron Endowment and Long-Term Kidney Function in Offspring.

KEY POINTS: Maternal ketosis disrupted nephrogenesis, reduced proliferation, and suppressed Myc and cell cycle signaling. Offspring exhibited persistent kidney deficits, including reduced nephron endowment and compromised long-term kidney function. Gestational ketosis developmental risks highlighted the need for metabolic monitoring and dietary guidance during pregnancy. BACKGROUND: The increasing popularity of ketogenic diets raises concerns regarding their safety during pregnancy. While mild ketosis is a common metabolic adaptation in gestation, the effect of diet-induced, supraphysiologica

Cross-cultural adaptation and validation in French of the cold intolerance symptom severity (CISS) questionnaire and potential work exposure scale (PWES) in patients with hand pathologies.

BACKGROUND: Cold intolerance is frequent after hand injuries and is the worst long-lasting problem that negatively affects other impairments in some patients. Two questionnaires are available to assess cold intolerance (CISS, PWES), but not in French. This study aimed to translate, cross-culturally adapt, and validate these questionnaires in French. METHODS: Population: outpatient and hospitalized patients between 2019 and 2024. Translation, back translation, cross-cultural adaptation according to standard guidelines. Ten experts evaluated the preliminary French versions for content validity.

Urinary symmetric dimethylarginine: A promising biomarker for acute kidney injury in dogs with pyometra.

BACKGROUND: Symmetric dimethylarginine (SDMA) is a biomarker of renal function derived from arginine methylation, unaffected by muscle mass or diet, and increases after only a 25% reduction in renal function, allowing earlier detection of renal dysfunction compared with serum creatinine. Although serum SDMA is well established in veterinary nephrology, urinary SDMA (uSDMA) remains comparatively underexplored, particularly in dogs with acute kidney injury (AKI). This study evaluated uSDMA concentrations and the uSDMA-to-creatinine (uSDMA/Cr) ratio as biomarkers for AKI, using renal histopatholo

RAGE in neutrophils triggers cerebral vasospasm in the hyperacute phase after subarachnoid hemorrhage.

Subarachnoid hemorrhage (SAH) due to the rupture of an intracranial aneurysm is a highly fatal type of stroke. Cerebral vasospasm (CVS) is a major post-SAH complication leading to delayed ischemic neurological deficits, thereby worsening patient outcomes. Previously, we found that lower plasma levels of the soluble receptor for advanced glycation end products (RAGE) predict symptomatic CVS in patients with SAH. However, the molecular mechanisms underlying CVS remain unclear. Here, using an SAH mouse model with endovascular perforation, we found that neurological deficits, CVS in the circle of

Association between Kaplan fiber injury and anterolateral tibial subluxation in patients with anterior cruciate ligament injury.

BACKGROUND: Kaplan fibers (KFs) are critical stabilizers of the knee, yet their role in anterior tibial subluxation of the lateral compartment (ATSLC) in anterior cruciate ligament (ACL) injury remains unclear. This study is to determine whether KF injury is independently associated with increased ATSLC on magnetic resonance imaging (MRI) in patients with an injured ACL, and to identify risk factors for high-grade ATSLC (≥ 4.5 mm). PATIENTS AND METHODS: This retrospective study enrolled 132 patients with ACL injury who underwent MRI within 12 weeks of trauma. MRI was used to evaluate the inte
