Anti-angiogenic Therapy at the Vascular Crossroads: Tumor Vessel Normalization, Excessive Pruning, and Systemic Vascular Toxicity.

Tumor angiogenesis contributes to tumor growth, metastasis, and treatment resistance, but the newly formed vessels are often structurally abnormal and functionally inefficient. Antiangiogenic therapy, particularly inhibition of Vascular Endothelial Growth Factor (VEGF) signaling, is now used in the treatment of multiple cancers. Its vascular effects depend strongly on the extent of angiogenic inhibition. Moderate inhibition may temporarily improve the structure and function of abnormal tumor vessels. This vascular normalization can improve perfusion, oxygenation, immune cell infiltration, and

AI-Based Delirium Risk Prediction Compared With the Validated 4AT-Score Screening Tool in Patients With Trauma: Prospective Cohort Study.

BACKGROUND: Delirium is a complex neuropsychiatric syndrome whose severe and lasting consequences in hospitalized patients are largely considered preventable. Prevention primarily relies on the early identification of high-risk patients prior to syndrome onset. However, this remains challenging in clinical practice due to the absence of a diagnostic gold standard and the considerable administrative burden associated with established screening tools such as the 4AT score. OBJECTIVE: The aim of this study is to compare the 4AT score with an established AI-supported prognostic tool in applied cl

Cumulative Incidence and Surgical Risk Factors for Severe Postoperative Complications in the Treatment of Primary Uterovaginal Prolapse: A Systematic Review and Meta-Analysis.

INTRODUCTION AND HYPOTHESIS: Apical repairs for uterovaginal prolapse (UPOP) are generally elective procedures with significant quality-of-life benefit; however, uncommon and potentially severe complications may occur. Our goal was to perform a systematic review and meta-analysis to understand risks for and estimate the incidence of major complications of common surgical approaches to UPOP repair. METHODS: Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we searched MEDLINE and EMBASE through 11/11/25 for studies describing risk factors for, or

Neonatal Cardiac Tamponade Related to Peripherally Inserted Central Catheter: A Rare Case Report.

BACKGROUND: Peripherally inserted central catheters (PICCs) are commonly used in neonatal intensive care units (NICUs) to provide prolonged intravenous access for preterm and critically ill infants. They are generally preferred over surgically placed central venous catheters because of easier insertion and, in some settings, lower infection rates. However, rare but life-threatening complications may occur. Cardiac tamponade, resulting from catheter malposition or delayed migration with perforation of the superior vena cava (SVC) or cardiac wall, is one such complication. This case highlights t

Gastrointestinal and Urological Tract Injuries During Cesarean Delivery: A Surgical Review With National Epidemiologic Data.

BackgroundCesarean delivery is the most common major surgical procedure performed worldwide, accounting for approximately one-third of US deliveries. Gastrointestinal (GI) and urological (URO) tract injuries are uncommon but disproportionately morbid complications. Increasing complexity, particularly repeat cesarean and placenta accreta spectrum (PAS), and uneven on-site subspecialty coverage have expanded the role of general surgeons as intraoperative consultants for these injuries, especially outside large tertiary centers.MethodsWe analyzed the National Inpatient Sample (NIS, 2005-2019) to

The role of the epidermal growth factor receptor (EGFR/ErbB1) in diabetic retinopathy and its therapeutic implications.

BACKGROUND: Diabetic retinopathy (DR) remains a major cause of preventable vision loss worldwide and arises from the convergence of chronic inflammation, oxidative injury, and metabolic dysregulation that ultimately compromises vascular and neuronal integrity of the retina. Although intravitreal anti-VEGF agents have improved the management of DR, significant limitations such as incomplete therapeutic response, repeated administration and high cost underscore the need for alternative or adjunctive strategies. MAIN BODY: Emerging preclinical evidence suggests that dysregulated signaling via th

Delayed life-threatening aortic rupture after rib fractures: a case report.

BACKGROUND: Aortic injury secondary to rib fractures is an extremely rare but life-threatening complication of blunt thoracic trauma. Most of the documented cases result in prehospital mortality or death shortly after arrival because of massive hemothorax. Although the surgical stabilization of rib fractures (SSRF) has become increasingly integrated into thoracic trauma care, there remains a lack of awareness regarding the potential for posteriorly displaced rib fractures to cause delayed aortic injury. CASE PRESENTATION: A 71-year-old female patient sustained multiple left posterior rib frac

Robotic-assisted living-donor hysterectomy for uterus transplantation: a systematic review of surgical techniques, donor safety, and recipient outcomes.

Uterus transplantation (UTx) has emerged as a promising treatment for women with absolute uterine factor infertility (AUFI), enabling pregnancy and live birth with their own genetic offspring. Living-donor uterus procurement remains the most technically demanding component of the procedure. Robotic-assisted donor hysterectomy has been introduced to improve surgical precision, reduce donor morbidity, and preserve graft quality, while recipient implantation is performed via laparotomy. This systematic review evaluated current evidence on robotic-assisted living-donor hysterectomy for UTx, focusi

When Insulin Must Wait: The Role of Renal Replacement Therapy in Refractory Hypokalemia During Severe Diabetic Ketoacidosis.

BACKGROUND Diabetic ketoacidosis (DKA) is a common life-threatening metabolic emergency characterized by hyperglycemia, high-anion-gap metabolic acidosis, and ketosis. Severe hypokalemia before insulin therapy is uncommon but may complicate management and delay treatment. CASE REPORT A 37-year-old man presented to the emergency department with a 2-day history of generalized abdominal pain and progressive shortness of breath. He was drowsy and tachypneic, with Kussmaul respirations. Laboratory evaluation revealed severe DKA, acute kidney injury, and severe hypokalemia. Insulin therapy was initi

Retrospective Comparison of the Pericapsular Nerve Group Block Versus the Fascia Iliaca Block for Hip Fractures in the Emergency Department.

INTRODUCTION: Hip fractures are a common injury among the elderly and a substantial cause of morbidity and mortality worldwide. Pain management is an essential component of care for patients with hip fractures, particularly for improving patient outcomes. Ultrasound-guided nerve blocks such as the fascia iliaca block (FIB) are evidence-based means of providing perioperative analgesia and reducing opioid use. Newer nerve blocks, such as the pericapsular nerve group (PENG) block, may serve as alternatives to the FIB. The objective of this study was to retrospectively compare pain reduction follo

Clinical and Financial Value of Cardiac Point-of-Care Ultrasound During Traumatic Cardiac Arrest.

INTRODUCTION: Traumatic cardiac arrest is associated with poor survival and significant consumption of hospital resources. Determining which patients have survivable injuries allows for judicious use of resources. Our aim was to determine whether cardiac activity on point-of-care ultrasound (POCUS) during traumatic cardiac arrest was associated with increased patient survival and the charges of continued resuscitation in patients that do have cardiac activity on POCUS. METHODS: We conducted this single-center retrospective study at an urban, Level I trauma center of consecutive patients arriv

Negative pressure wound therapy for the prevention of surgical site complications in emergency laparotomies: an updated systematic review and meta-analysis.

INTRODUCTION: Surgical site infections (SSIs) are common after emergency laparotomy and are associated with substantial morbidity, prolonged recovery, and increased health care costs. Negative pressure wound therapy (NPWT) has been increasingly used prophylactically on closed incisions in high-risk surgical patients; however, evidence supporting its effectiveness after emergency laparotomy remains heterogeneous. This updated systematic review and meta-analysis aimed to reassess the effectiveness of NPWT compared with standard postoperative dressings for the prevention of SSI and other wound-re

Mixed Presentation of Hyperglycemic Hyperosmolar State and Diabetic Ketoacidosis in a Pediatric Patient: a Case Report and Literature Review.

The coexistence of diabetic ketoacidosis (DKA) and hyperglycemic hyperosmolar state (HHS), known as mixed hyperglycemic crisis, is a rare but potentially fatal metabolic emergency in pediatrics, and is frequently underrecognized because it is misclassified as isolated DKA. It carries a higher risk of complications than either entity alone, particularly in patients with complex comorbidities. We describe the case of a 16-year-old female with myelomeningocele, neurogenic bladder, obesity, and insulin resistance, who had self-discontinued treatment with metformin, atorvastatin, and allopurinol on

Capillary stalling after traumatic brain injury: an emerging microcirculatory contributor to secondary brain injury.

Secondary brain injury after traumatic brain injury (TBI) has traditionally been understood and managed through the lens of intracranial pressure, cerebral perfusion pressure, and global cerebral blood flow. Yet a substantial proportion of patients continue to exhibit persistent tissue hypoxia and metabolic crisis even when these macrocirculatory parameters appear adequately controlled. Capillary stalling-the transient or sustained interruption of red blood cell transit within individual or focal capillary networks-may represent an under-recognized microcirculatory disorder that conventional m

Successful balloon-assisted percutaneous thrombin injection for common femoral artery pseudoaneurysm after venoarterial extracorporeal membrane oxygenation decannulation.

Femoral artery pseudoaneurysm is a serious vascular complication after venoarterial extracorporeal membrane oxygenation (VA-ECMO) decannulation, and optimal management remains challenging. We report the case of a 74-year-old man with acute pulmonary embolism who developed a common femoral artery pseudoaneurysm after VA-ECMO removal. Ultrasound-guided compression was unsuccessful, and surgical repair was considered but deferred because of the patient's clinical condition and operating room unavailability. Balloon-assisted percutaneous thrombin injection achieved complete thrombosis without dist

Safety considerations and clinical monitoring during pulsed field ablation.

Pulsed field ablation (PFA) has emerged as a transformative technology in cardiac electrophysiology, offering a potentially superior safety profile compared with thermal ablation due to its non-thermal mechanism of irreversible electroporation. Clinical experience confirms a substantial reduction in traditional complications such as pulmonary vein stenosis, atrio-oesophageal fistula, and permanent phrenic nerve injury, though PFA introduces a distinct spectrum of physiologic effects requiring consideration. Coronary vasospasm is the most characteristic phenomenon, ranging from transient, subcl
