Modified non-transarticular K-wire external fixation versus traditional transarticular pinning for bony mallet finger: a prospective comparative cohort study.

BACKGROUND: Traditional transarticular distal interphalangeal joint (DIPJ) pinning for bony mallet fingers risks stiffness and iatrogenic cartilage injury. This study compared a modified non-transarticular K-wire external fixator with traditional transarticular pinning for surgically treated bony mallet finger. METHODS: This single-center prospective nonrandomized comparative cohort study included 120 adults with acute bony mallet finger. Sixty patients underwent traditional transarticular DIPJ pinning, and 60 underwent modified non-transarticular K-wire external fixation. The primary outcome

The Dangers of Chest Compression: A Case Report.

Chest injuries after cardiopulmonary resuscitation (CPR) are common, but subsequent infected parasternal collections after CPR are uncommonly described. We present a case of a 49-year-old female with a history of intravenous drug use (IVDU) who developed multiple sternal and rib fractures and a large infected parasternal collection after community CPR following a heroin overdose. She was treated with antibiotics and required surgical evacuation of the collection. This case highlights the importance of recognizing and managing post-CPR chest wall complications early to prevent serious morbidity

The Effect of Psychiatric Disorders on Ankle Fracture Outcomes.

This study examines the association between psychiatric disorders and in-hospital outcomes following ankle fractures. The authors hypothesized that the presence of psychiatric disorders would be associated with inferior in-hospital outcomes compared to those found in patients without psychiatric disorders. Presence and type of psychiatric disorder, demographics, comorbidities, time to surgery, length of stay (LOS), and total charge were compared for patients hospitalized with ankle fractures. Multivariate regression analysis of National Inpatient Sample (NIS) database showed that the presence

Prophylactic Antibiotic Use in the Setting of Non-Operative, Low-Velocity Gunshot Wounds.

The management of low-velocity gunshot-induced fractures (LGF), particularly regarding prophylactic antibiotic use, is widely debated. Recent studies have suggested no significant difference in infection rates between patients receiving prophylactic antibiotics for LGF and those who do not. This retrospective study compared infection rates between patients receiving prophylactic antibiotics and those who did not for non-operative, LGF between 2015 and 2020. Data were analyzed for variables including demographics, injury characteristics, and infection within 30 days. Of the 74 patients included

Surgery under fire: war injuries and the consequences of delayed care in Ethiopia's Tigray region.

BACKGROUND: The armed conflict in Ethiopia, particularly in the Tigray region, has caused a substantial burden of war-related injuries in the context of a fragile and disrupted health system. Restricted access to timely surgical care has resulted in delayed presentation and may contribute to secondary complications, contractures, peripheral nerve dysfunction, and long-term disability. However, the consequences of delayed surgical treatment for conflict-related extremity trauma in active war settings remain insufficiently described. This study aimed to characterize conflict-related injuries in

Oral inulin prebiotic encapsulating Cortex Fraxini extracellular vesicles alleviates colitis and associated renal stress via reinforcing intestinal barriers and remodeling microbiota.

Background: Gut-kidney axis dysfunction contributes to extraintestinal complications in inflammatory bowel disease (IBD), yet effective therapeutic strategies targeting this axis remain limited. This study developed an oral prebiotic delivery system by encapsulating Cortex Fraxini-derived extracellular vesicles (CF-EVs) within an inulin-based hydrogel (CF-EVs@Inulin) and evaluated its therapeutic potential in ulcerative colitis and associated renal injury. Methods: CF-EVs were isolated and characterized for morphology and antioxidant activity. An inulin-based hydrogel was employed to encapsula

Neonatal mechanical ventilation.

Neonatal mechanical ventilation is a cornerstone in the management of respiratory failure in newborns (NBs), particularly in preterm infants, in whom pulmonary and neurologic increases their vulnerability to ventilation-induced lung injury. At present, the primary objective of respiratory support is to ensure adequate ventilation while minimizing lung and brain injury through lung-protective strategies and careful oxygen titration. Noninvasive ventilation (NIV) is currently the first-line approach for both initial respiratory support and post-extubation care, as it reduces the need for invasiv

Composite Cap Grafting for Fingertip Amputations in Pediatric Patients: Does Patient Age Matter?

BACKGROUND: Fingertip avulsions are one of the most common pediatric hand injuries with composite "cap" grafting being a typical intervention. However, composite grafting can be unpredictable, and little is published on its expected clinical course. Therefore, we sought to determine if there is an age cutoff for superior graft outcomes. We hypothesized that graft success would be highest in children under 8 years old. Secondarily, we aimed to characterize a postoperative timeline for graft failure versus success. METHODS: A retrospective chart review was performed with the following inclusion

Glass intraocular foreign body removal assisted by biological amniotic membrane: a case report.

BACKGROUND: The extraction of large, sharp-edged nonmagnetic intraocular foreign bodies carries risks of iatrogenic retinal injury with conventional instruments. Biological amniotic membrane is known to be safe for intraocular use, but its application as a protective wrap to remove a foreign body has not been reported to our knowledge. We describe this novel approach. CASE PRESENTATION: A man in his forties presented with blurred vision and a retained intraocular glass fragment (5 × 3 × 1 mm) in the left eye, with visual acuity of 20/32. During vitrectomy, attempts to mobilize the sharp fragm
