[Evaluation and risk assessment of geriatric ankle fractures? : What must be considered?].

The frequency of fractures in old age is steadily increasing including geriatric ankle fractures. These injuries are associated with increased morbidity and prolonged hospitalization. The consequences of courses prone to complications represent a relevant socioeconomic factor and the goal of treatment is to preserve the patient's mobility and quality of life. To ensure optimal perioperative care, the identification of geriatric patients, evaluation of the functional status and existing risk factors with subsequent interdisciplinary treatment are required. Various surgical procedures are availa

Increased fracture risk associated with iron-deficiency anemia: a population-based propensity score matched cohort study.

UNLABELLED: Anemia seems to be associated with an increased risk of fracture. This cohort study of the UK primary care data shows in 84,932 anemic patients and 158,731 controls an increased fracture risk in anemic patients, also including patients < 50 years of age. Severe anemia was associated with 2 × vertebral fracture risk. Awareness of this clinically relevant association should increase. PURPOSE: In this study, we evaluated the risk of fracture among adult patients with anemia, compared to propensity score matched controls, stratifying by fracture site, age, and severity of anemia. Predicting survival of patients with spinal involvement in multiple myeloma using PATHFx 3.0 - a validation study of 100 patients in Germany.

BACKGROUND: Spinal malignant lesions are a common feature in multiple myeloma (MM) and often require surgical intervention. Accurate survival prediction is critical for guiding treatment decisions in these patients. While PATHFx is a widely used, machine-learning-based prognostic tool for skeletal metastases, it has not been validated for malignant bone lesions in MM so far. OBJECTIVE: To evaluate the predictive performance and clinical utility of PATHFx 3.0 in a well-characterized cohort of MM patients with spinal malignant lesions. METHODS: A retrospective cohort of 100 MM patients with radi

Predictors of Vertebral Compression Fracture Following Stereotactic Body Radiation Therapy in SINS Potentially Unstable Spinal Metastases.

PURPOSE: The Spinal Instability in Neoplasia Score (SINS) is the gold standard to determine if the metastatically involved spine is stable, potentially unstable, or frankly unstable. In potentially unstable spines, clarity is needed about the risk of post-stereotactic body radiation therapy (SBRT) vertebral compression fracture (VCF) and which patients may benefit from early stabilization. We aimed to identify predictors of VCF following spine SBRT in patients with potentially unstable SINS spinal metastases.. METHODS AND MATERIALS: A retrospective review of a prospectively maintained databas

Effect of Doctor Car dispatch on neurological outcomes in children with status epilepticus: A single-Center retrospective cohort study.

BACKGROUND: Status epilepticus (SE) requires prompt treatment because prolonged seizures increase the risk of neurological injury. We investigated whether a physician-staffed ambulance (commonly referred to as "Doctor Car" in Japan) improves neurological outcomes in pediatric SE. METHODS: We conducted a medical record-based retrospective study of children aged ≤15 years with SE transported to a tertiary emergency center between December 2014 and November 2024. The primary endpoint was neurological sequelae at hospital discharge. Secondary endpoints included time to antiseizure medications (AS

Vertebral fractures identified on lateral DXA images by deep learning predict incident fractures in older women.

UNLABELLED: XVFA is an AI-based method for identifying vertebral fractures on DXA images. In 423 women followed for 8 years, vertebral fractures identified by XVFA or manual assessment were associated with a twofold increased risk of incident fractures. XVFA predicted fracture risk comparably to manual assessment, supporting automated vertebral fracture detection. PURPOSE: Vertebral fractures (VFs), identified by vertebral fracture assessment (VFA) using dual-energy X-ray absorptiometry (DXA), predict incident fractures independently of clinical risk factors (CRFs) and bone mineral density (B

When Stroke Mimics Peripheral Nerve Injury: A Case Report of Isolated Wrist Drop.

Isolated wrist drop as a presentation of stroke is uncommon and may pose a diagnostic dilemma. We present a case of a 65-year-old man with hypertension and diabetes who presented with sudden-onset weakness of the left hand and inability to extend fingers, without any other focal neurological deficits. Noncontrast computed tomography of the head was normal. However, diffusion-weighted magnetic resonance imaging of the brain revealed an acute ischemic infarct involving the right frontoparietal and occipital lobes. A diagnosis of isolated left-sided wrist drop secondary to acute ischemic stroke w

Stable valley-like concavity at the midbody of the medial meniscus mimicking meniscocapsular separation: a case report.

A 39-year-old woman presented with catching, pain during deep knee flexion, and intermittent locking. Magnetic resonance imaging showed no definite meniscal tear, and diagnostic arthroscopy was performed. Arthroscopy revealed hypermobility of the posterior horn of the lateral meniscus with no tear identified. This hypermobility was considered the source of the patient's mechanical symptoms and was stabilized with three all-inside sutures. In addition, systematic inspection of the medial compartment incidentally identified a smooth, well-demarcated valley-like concavity at the meniscocapsular j

Postoperative Epidural Fibrosis after Lumbar Disc Herniation Surgery: Our Experience.

PURPOSE: Postoperative epidural fibrosis (PEF) is a significant complication following lumbar disc herniation surgery. It is characterised by the formation of non-physiological scar tissue that may lead to recurrent radicular pain and contribute to failed back surgery syndrome (FBSS). Despite its clinical relevance, the exact incidence and effective management strategies for LPEF have not been adequately quantified in the existing literature. METHODS: This study analyses a cohort of 2673 patients who underwent surgical intervention for herniated lumbar discs between September 2010 and August

Should we remove flexible intramedullary nails after diaphyseal forearm pediatric fracture? A cohort study of 288 patients.

BACKGROUND AND PURPOSE: Routine removal of elastic stable intramedullary nails (ESIN) after pediatric diaphyseal forearm fractures remains common practice despite limited supporting evidence. We aimed to evaluate refracture risk following ESIN retention compared with implant removal and assessed complications and healthcare resource use. METHODS: In this retrospective cohort study, patients treated with ESIN for diaphyseal forearm fractures at a single center were included. Following a policy change, routine implant removal was discontinued in favor of retaining implants unless symptomatic. P

Low-Molecular-Weight Heparin Versus Unfractionated Heparin for Venous Thromboembolism Prophylaxis in Adult Traumatic Brain Injury: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVES: Traumatic brain injury (TBI) is associated with a high risk of venous thromboembolism (VTE), yet the optimal strategy for thromboprophylaxis remains uncertain. Low-molecular-weight heparin (LMWH) and unfractionated heparin (UFH) are commonly used agents in clinical practice, but their comparative effects in adult TBI have not been formally synthesized. METHODS: Medical databases were searched through March 2026 for studies comparing LMWH and UFH for prophylaxis in adult patients with TBI. Randomized trials and cohort studies reporting at least one thromboembolic, he

Perioperative hyperbaric oxygen therapy: mechanisms, evidence, and clinical application.

Hyperbaric oxygen therapy delivers 100% oxygen at supra-atmospheric pressures, elevating plasma oxygen content and tissue oxygenation above normoxic levels. Once limited to decompression illness, hyperbaric oxygen therapy is gaining recognition as a perioperative adjunct with potential to mitigate complications through supra-physiologic tissue oxygenation. This review summarizes current evidence on the biological mechanisms, clinical applications, and considerations of perioperative hyperbaric oxygen therapy. Mechanistically, hyperbaric oxygen therapy has been shown to promote angiogenesis thr

Olecranon medial flip osteotomy for total elbow arthroplasty : Triceps-preserving approach to the elbow.

OBJECTIVE: The olecranon medial flip osteotomy offers adequate exposure to the posterior elbow, while preserving the continuity of the triceps extensor complex and sparing the innervation of the anconeus muscle. It avoids soft-tissue injury due to strain and aids in protecting the ulnar nerve by medially reflecting the long head of triceps. INDICATIONS: This surgical approach allows the exposure required for total elbow arthroplasty and concomitant radial head excision. CONTRAINDICATIONS: Patients who underwent previous surgical procedures to the triceps tendon insertion. SURGICAL TECHNIQUE

Clinical outcomes after calcaneonavicular coalition resection in adults: A systematic review and meta-analysis of preoperative to postoperative changes in pain and function.

Calcaneonavicular coalition is traditionally considered a pediatric condition; however, some patients remain symptomatic into adulthood, and evidence regarding surgical outcomes in this population remains limited. This study aimed to evaluate clinical improvement following calcaneonavicular coalition resection in adults through a systematic review and single-arm meta-analysis of preoperative to postoperative changes in pain and function. A systematic search of PubMed, EMBASE, and Scopus was performed to identify studies published between January 2016 and December 2025. Eligible studies include

Arthroscopic debridement with microfragmented adipose tissue for wrist and hand chondral lesions in professional athletes.

BACKGROUND: Focal chondral lesions of the wrist and hand are an underrecognized source of pain and functional limitation in athletes. Their management remains challenging due to the limited healing capacity of hyaline cartilage, the biomechanical demands of small joints, and the lack of standardized treatment algorithms. Arthroscopy allows accurate diagnosis and targeted treatment with minimal morbidity, while biologic augmentation using autologous adipose-derived products has emerged as a potential adjunct to improve clinical outcomes. OBJECTIVE: To evaluate the clinical outcomes of arthrosc
