Malignant spinal cord compression (MSCC) is a serious complication of spinal metastases and primary spinal tumours that can severely affect quality of life and overall survival. Clinical trials have used inconsistent definitions, criteria, and endpoints, limiting comparability. We conducted a Delphi consensus process following a systematic review of prospective MSCC trials from Jan 1, 2003, to Feb 26, 2024. From June 25, 2024, to Jan 11, 2025, experts from 20 countries participated in two survey rounds, 74 in round 1 and 71 in round 2. Consensus was defined as over 75% agreement. 49 statements

BACKGROUND: A 45-year-old woman with a history of schizoaffective disorder presented to the emergency department from a skilled nursing facility after being found unconscious in the bathroom with active scalp bleeding from a circumferential wound. The neurosurgery team was consulted due to concerns of intracranial infection.
OBSERVATIONS: CT of the head revealed band-like circumferential bony erosion of the calvarium traversing the superior sagittal sinus. Family members denied any history of prior neurosurgical intervention or cranial deformity; however, they noted a fall approximately 7 mon

INTRODUCTION: Pelvic ring fractures are an increasingly common injury in the aging population, carrying mortality comparable to hip fractures and substantial functional decline. Most geriatric fractures are managed nonoperatively, with operative fixation reserved for unstable patterns, but high-quality evidence comparing both treatment strategies is lacking. This study compared mortality and complications between operatively and nonoperatively managed pelvic ring fractures in older adults across multiple time points.
METHODS: This retrospective cohort study used the TriNetX database to identi

PURPOSE: Cervical myelopathy (CM) often presents with gait disturbances, which can lead to falls and spinal cord injury. As the presence of gait disturbances negatively affects postoperative improvement in patients with CM, early detection of gait disturbances is crucial. However, current gait analysis methods require specialisation, limiting their feasibility for screening. This study aimed to perform gait analysis in patients with CM using wearable in-shoe inertial measurement unit (IMU) sensors and to develop a screening system using machine learning. METHODS: This study included 26 patient

BACKGROUND: Risk factors for perineal wound complications have primarily been studied in women with obstetric anal sphincter injuries, whereas data in women with second-degree tears and episiotomy are limited.
OBJECTIVE: To identify risk factors for clinically relevant wound complications following second-degree perineal tear or episiotomy. We hypothesized that obstetric and repair-related factors would be associated with an increased risk of wound complications.
STUDY DESIGN: This cohort study was based on data from a randomized controlled trial conducted at a university hospital in the Cap

BACKGROUND: Custom unicompartmental knee arthroplasty (UKA) implants were developed to improve component fit and alignment compared to conventional off-the-shelf (OTS) implants. However, evidence supporting the clinical advantage of custom UKA remains limited. This study compared patient-reported outcome measures (PROMs) and complication rates following custom and OTS UKA.
METHODS: A single-institution database was queried for patients who underwent medial UKA. Patients who had at least two years of follow-up and completed pre- and postoperative PROMs were included. A total of 623 patients me

UNLABELLED: The primary objective of this study was to determine the differences in efficacy between denosumab and zoledronate in preventing secondary vertebral compression fractures (VCFs). Our findings demonstrate that twice-yearly administration of denosumab compared with conventional zoledronate significantly reduces the early risk of secondary VCFs among postmenopausal women following vertebroplasty. However, this advantage was confined to the first year and was not sustained at 24 months. Clinicians should consider denosumab as a preferential option over bisphosphonates when managing ost

Pediatric femoral neck fractures result primarily from high-energy trauma. Common treatment methods include screw fixation and the placement of a proximal femoral locking plate. However, there is limited biomechanical evidence favoring one method over another for the treatment of unstable fractures. This study aimed to evaluate the biomechanical properties of screws and proximal femoral locking plates for the treatment of unstable pediatric femoral neck fractures using a synthetic bone model. Fourteen synthetic composite femurs were divided into two groups that included screw fixation (S) and

The risk of fractures and osteoporosis is higher in patients on hemodialysis than in the general population. We examined the association between computed tomography (CT)-measured trabecular attenuation at the first lumbar vertebra (L1 CT-attenuation), bone mineral density (BMD), and osteoporosis in patients on hemodialysis. We retrospectively included Japanese patients on hemodialysis who underwent non-enhanced abdominal CT and dual-energy X-ray absorptiometry (DEXA) analyses as part of a medical checkup between April 2019 and March 2021. L1 CT-attenuation (Hounsfield units, HU) and BMD of the

Diabetes mellitus and osteoporosis are among the most frequent chronic diseases and therefore often occur in one and the same individual. As the prevalence of both diseases increases with age, in view of the age structure of our population their frequency will increase. Patients with diabetes have an elevated risk of fragility fractures. The pathophysiology is unclear and presumably multifactorial. Longitudinal studies have shown that the Fracture Risk Assessment Tool (FRAX) and the bone mineral density (BMD) measured by dual energy X‑ray absorptiometry (DXA, T‑score) and a possibly present tr

Post-stroke spasticity (PSS), characterized by hyperreflexia and spastic hypertonia, is a common and debilitating consequence of cerebrovascular injury that impedes motor recovery. Despite its prevalence, the neurophysiological mechanisms underlying PSS remain poorly understood, partly due to the lack of reliable animal models. A known clinical correlate of spasticity is weakened rate-dependent depression (RDD) of the Hoffman reflex (H-reflex), particularly in the upper limbs of chronic stroke patients. However, the mechanistic basis of RDD impairment and its relationship to brain lesion chara

Background/Objectives: Coronectomy is a nerve-preserving alternative for mandibular third molars at high risk of inferior alveolar nerve injury. However, coronectomy-related knowledge, clinical experience, treatment preferences, and self-reported decision-making confidence may differ between general dentists (GD group) and oral and maxillofacial surgeons (OMFS group). This study aimed to compare these factors and identify factors associated with coronectomy-related knowledge. Methods: This cross-sectional survey was conducted between June 2024 and January 2026 among 300 dentists in Turkey. Par

Sarcopenia, the progressive loss of muscle mass and function, is a significant health concern in middle-aged and older adults, potentially linked to adverse outcomes like falls and fractures. A cohort study was conducted utilizing participants aged ≥ 45 years from CHARLS. Sarcopenia was defined using established criteria. Primary outcomes were isolated falls, isolated hip fractures, and composite outcomes. Associations were assessed using multivariable Cox regression (adjusted for confounders), propensity score matching (PSM), and inverse probability weighting (IPW), reporting hazard ratios (H
