Facial Injury Severity Scale and Frontal-Segment Involvement as Risk Stratification Markers in Panfacial Fractures: A Single-Center Retrospective Study.

Panfacial fractures are high-energy injuries. We evaluated whether the Facial Injury Severity Scale (FISS) and frontal-segment involvement are associated with early surgical complications, length of stay, intensive care unit (ICU) admission, and craniocerebral injury. Thirty-four panfacial fracture patients treated between January 2016 and May 2026 were analyzed retrospectively at a single center. Fractures were classified with the Erdmann-Follmar four-segment model (≥3 of 4 segments); FISS used the Duke modified system. Because several tables were sparse, associations were estimated by Firth

Varicella zoster vasculopathy presenting as bilateral subdural haematoma in an infant: case report of a rare case and literature review.

BACKGROUND: Varicella-zoster virus (VZV) infection is typically benign in infants but may rarely cause neurological complications. The occurrence of chronic subdural haematoma following VZV infection in children has not previously been reported. CASE DESCRIPTION: We report the case of a 6-month-old male infant who developed chicken pox with fever, irritability, and generalized vesicular rash. His rash and fever settled over 2 weeks, but he had persistent irritability, decreased feeding, intermittent vomiting, regression of developmental milestones, and developed progressive weight loss (50th

Glass intraocular foreign body removal assisted by biological amniotic membrane: a case report.

BACKGROUND: The extraction of large, sharp-edged nonmagnetic intraocular foreign bodies carries risks of iatrogenic retinal injury with conventional instruments. Biological amniotic membrane is known to be safe for intraocular use, but its application as a protective wrap to remove a foreign body has not been reported to our knowledge. We describe this novel approach. CASE PRESENTATION: A man in his forties presented with blurred vision and a retained intraocular glass fragment (5 × 3 × 1 mm) in the left eye, with visual acuity of 20/32. During vitrectomy, attempts to mobilize the sharp fragm

Capillary stalling after traumatic brain injury: an emerging microcirculatory contributor to secondary brain injury.

Secondary brain injury after traumatic brain injury (TBI) has traditionally been understood and managed through the lens of intracranial pressure, cerebral perfusion pressure, and global cerebral blood flow. Yet a substantial proportion of patients continue to exhibit persistent tissue hypoxia and metabolic crisis even when these macrocirculatory parameters appear adequately controlled. Capillary stalling-the transient or sustained interruption of red blood cell transit within individual or focal capillary networks-may represent an under-recognized microcirculatory disorder that conventional m

Post-traumatic cognitive decline and neurodegenerative disease: a narrative review and proposed framework for etiologic assessment.

BACKGROUND: When a patient with a history of traumatic brain injury (TBI) develops progressive cognitive decline months to years after injury, clinicians must consider injury sequelae, an independent neurodegenerative disease, or both. Existing diagnostic frameworks each address a different aspect of this question, and none was constructed to apportion contribution between them. The distinction matters for management because AD-directed therapy should be considered only after biological Alzheimer's disease (AD) has been established, followed by separate assessment of treatment eligibility and

Endoscopic techniques for removal of intraorbital foreign bodies - a clinical series.

BACKGROUND: Management of intraorbital foreign bodies (IOFBs) is challenging due to the confined space and proximity of neurovascular and ocular structures. The increasing adoption of endoscopic techniques has expanded minimally invasive treatment options. We report operative corridors, indications, and outcomes of various endoscopic approaches in a consecutive series. METHOD: We analyzed 16 patients with IOFBs treated between 2015 and 2023 at a tertiary university center. All procedures were performed using exclusively endoscopic techniques. Eleven IOFBs were metallic and five organic. Surgi

Duration of single-agent perioperative antibiotic prophylaxis and postoperative meningitis after endoscopic endonasal skull base surgery: a systematic review and meta-analysis of 7,981 patients.

Postoperative meningitis is a rare but serious complication in endoscopic-endonasal skull-base surgery. Although prophylactic antibiotics are routinely administered, the optimal regimen and duration remain unclear. This systematic review and meta-analysis evaluated the incidence of meningitis and intraoperative cerebrospinal fluid (CSF) leakage associated with single-agent perioperative antibiotic prophylaxis. Following PRISMA 2020 guidelines, PubMed, EMBASE, and Web of Science were searched through February 2025. Eligible studies reported postoperative meningitis or intraoperative cerebrospin

Application of cervical epidural blood patch for spontaneous cerebrospinal fluid leaks: Two case reports.

Spontaneous cerebrospinal fluid (CSF) leaks are uncommon but significant causes of orthostatic headaches, often resulting from dural injuries. Although lumbar epidural blood patches (EBPs) are commonly used to treat such leaks, cervical EBPs are rarely performed due to the higher risk of complications, including spinal cord injury. This case series presents two patients with spontaneous cervical CSF leaks successfully treated with cervical EBPs under fluoroscopic guidance. The first case involved a 24-year-old female who presented with a two-month history of severe orthostatic headaches and ti

C/EBPβ Drives Ischemia-Induced Microglial Inflammatory Reprogramming Through Transcriptional Activation of ATF3 and Exacerbates Brain Injury.

Microglial activation contributes substantially to secondary brain injury after ischemic stroke, but the transcriptional mechanisms governing this response remain poorly defined. Here, we identified CCAAT/enhancer-binding protein β (C/EBPβ) as a key regulator of ischemia-induced microglial inflammation. RNA sequencing of microglia isolated from mice subjected to transient middle cerebral artery occlusion (tMCAO) revealed extensive transcriptional reprogramming, characterized by activation of inflammatory pathways, including chemokine, TNF, IL-17, and efferocytosis signaling, as well as cell-cy

Noninvasive ultrashort wave neuromodulation enhances glymphatic clearance to alleviate traumatic brain edema and Alzheimer's pathology.

The glymphatic system clears cerebral waste, yet strategies to enhance its function remain elusive. Here, we show that 40.68 MHz ultrashort wave (USW) rapidly enhances glymphatic bulk flow in healthy mice within 10 minutes. USW potentiates both glymphatic influx and efflux, concomitant with increased Delta wave neuronal synchronization, as confirmed by local field potentials and electroencephalograms. Chemogenetic silencing confirms that USW-driven glymphatic enhancement depends on neuronal activity, while pharmacological blockade of aquaporin-4 (AQP4) abolishes the effects, revealing dual rel

A Standardized Workflow for Multimodal Assessment of Right-to-Left Shunt-Related Ischemic Brain Injury.

Right-to-left shunt (RLS) has been associated with cryptogenic stroke, migraine, and other neurological conditions in which ischemic brain injury may be clinically relevant. In routine practice, however, assessment is often fragmented. Shunt presence may be documented, but shunt burden, provoked versus non-provoked patterns, anatomical complexity, and MRI-defined brain injury are not always integrated into a reproducible workflow. Here, we describe a standardized protocol for multimodal assessment of RLS-related ischemic brain injury in a clinically selected cohort. The workflow includes patie

Recommendations for the Management of Spontaneous Intracranial Hypotension With Spinal Longitudinal Epidural CSF Collection: A Delphi Study.

BACKGROUND AND OBJECTIVES: Spontaneous intracranial hypotension (SIH) results from spinal CSF leakage. Epidural blood patch (EBP) is an important first-line treatment. However, pre-EBP, intra-EBP, and post-EBP practice varies widely owing to limited consensus guidance. We aimed to develop expert consensus statements to support clinical decision-making in SIH management, including peri-EBP practices, focusing on patients with spinal longitudinal epidural CSF collection (SLEC) before definitive leak type classification. METHODS: An international multidisciplinary Delphi process was conducted be
