Immediate post-traumatic pseudoaneurysm of the external carotid artery following condylar fracture: case report.

INTRODUCTION: Pseudoaneurysms following mandibular condylar fractures are extremely rare and usually present weeks after trauma. Acute life-threatening presentations are exceptional. MATERIALS AND METHODS: A 19-year-old man was admitted in hemorrhagic shock after high-energy facial trauma. Imaging revealed bilateral comminuted condylar fractures associated with an actively bleeding pseudoaneurysm of the distal external carotid artery. Emergency endovascular coil embolization achieved rapid hemorrhage control, followed by surgical fixation of the mandibular symphysis. Postoperatively, the pati

Medical AI, inductive risk and the communication of uncertainty: the case of disorders of consciousness.

Some patients, following brain injury, do not outwardly respond to spoken commands, yet show patterns of brain activity that indicate responsiveness. This is 'cognitive-motor dissociation' (CMD). Recent research has used machine learning to diagnose CMD from electroencephalogram recordings. These techniques have high false discovery rates, raising a serious problem of inductive risk. It is no solution to communicate the false discovery rates directly to the patient's family, because this information may confuse, alarm and mislead. Instead, we need a procedure for generating case-specific proba

Stigmasterol ameliorates neurological deficits and neuroinflammation in rats with traumatic brain injury by suppressing STAT3/NF-κB/LCN-2.

PURPOSE: TBI triggers intricate inflammatory processes that lead to secondary damage. Stig, a compound known for its cholesterol-lowering effects, demonstrates significant therapeutic potential, encompassing anti-cancer, antioxidant, and anti-atherosclerotic properties. It possesses an ability to penetrate the blood-brain barrier and accumulate in the brain, where it exerts potent anti-inflammatory, anti-apoptotic, and anti-oxidative stress properties, specifically in conditions impacting the central nervous system. METHODS: This study is a basic research combining in vitro and in vivo experi

Low or Low-Normal Insulin-Like Growth Factor 1 After Traumatic Brain Injury: Interpretation and Implications.

Growth hormone deficiency (GHD) is a prevalent consequence of traumatic brain injury (TBI) that can contribute to prolonged adverse symptoms and impaired quality of life. The overlap between symptoms of GHD and chronic TBI complicates the clinical picture, highlighting the need for precise diagnostic tools. Insulin-like growth factor 1 (IGF-1), a key mediator of growth hormone (GH) activity, is frequently utilized as a surrogate marker of GH status. However, IGF-1 levels may be normal in patients with confirmed GHD. Furthermore, low or low-normal IGF-1 is not specific to GHD, and provocative t

Infant Abusive Head Trauma Induces Seizures and Cellular Stress in a Pre-Clinical Mouse Model.

Abusive head trauma (AHT) occurs when a child is grasped and shaken with such force that the brain moves within the skull. AHTs are not isolated events; repetition occurs in 30-50% of cases, with affected children experiencing ∼10 episodes. While many AHTs appear asymptomatic, there is often underlying cellular and neurological damage. Notably, seizures following AHT may be solely electrographic without overt behavioral signs and detectable only via electroencephalography (EEG). Thus, we examined electrographic seizures and indicators of cell stress in a mouse model of repeat AHT. On postnatal

Multimodal Novel Recurrent Laryngeal Nerve Monitoring Techniques in Anterior Cervical Spine Surgery: Initial Experience and Proof of Concept.

Injury to the recurrent laryngeal nerve (RLN) during anterior cervical spine surgery is an uncommon yet impactful sequela. This study evaluates the feasibility and intraoperative behavior of laryngeal adductor reflex (LAR) and motor-evoked potentials with vocalis recordings (MEP-VC) during anterior cervical spine surgery. LAR and MEP-VC were employed for 13 patients undergoing anterior cervical spine surgery. LAR alerts were issued when clinically correlated reductions in amplitude, typically ≥20% in previously stable, non-habituating recordings, and/or temporal dispersion of the LAR waveform

Differences in Reaction Time Variability on the Senaptec Sensory Station by Concussion Status in Special Operations Forces Service Members.

INTRODUCTION: Computerized cognitive tests are widely used in post-concussion assessments. The psychometric properties and clinical utility of employing central tendency measures have been questioned. This is true for many outcomes, including reaction time (RT). Cognitive dispersion metrics, such as reaction time variability (RTV) measured by intraindividual variability (IIV), may offer increased sensitivity to concussions. Evaluating IIV across different computerized cognitive platforms is key to determining the utility of central dispersion metrics. This study aimed to extract IIV from the S

Cognitive loss after brain trauma results from sex-specific activation of synaptic pruning processes.

Cognitive losses resulting from severe brain trauma have long been associated with the focal region of tissue damage, leading to devastating functional impairment. For decades, researchers have focused on the sequelae of cellular alterations that exist within the perilesional tissues; however, few pharmacological therapies are available to patients. To examine whether expansive global synaptic damage underlies cognitive losses associated with brain injury, we evaluated the influence of D-serine on synaptic damage in male and female wild-type mice as well as mice deficient in microglial serine

Influence of Demographic and Clinical Factors on Perceived Usability, Presence, Flow, Competence, Pleasant and Unpleasant Sensations, and Utility During Interaction with Virtual Reality Games for Motor and Cognitive Rehabilitation: An Observational Study in Patients with Stroke and Traumatic Brain Injury.

OBJECTIVE: User experience plays a crucial role in the interaction with virtual reality systems, especially in rehabilitation games for patients with brain injuries, who often face cognitive and motor challenges. User experience influences factors such as acceptance, engagement, and, importantly, the effectiveness of the intervention, all of which are key to achieving optimal recovery outcomes. While previous studies have examined the experiences of neurological populations, they have often focused on a single construct and used instruments that include irrelevant or confounding items. The obj

Risk Stratification for Traumatic SAH Enlargement and Surgical Intervention: Guides to Follow-Up Imaging in Patients with Trauma.

BACKGROUND AND PURPOSE: SAH is a frequent intracranial finding in patients with trauma and poses significant diagnostic and prognostic challenges. Identifying which patients need closer follow-up because of potential complications of posttraumatic SAH is important because the need for serial imaging is debated. The purpose of this study was to evaluate the rate of posttraumatic subarachnoid hemorrhage enlargement, identify predictors of hemorrhage progression and need for surgical intervention, and propose a risk-stratified approach to follow-up imaging in trauma patients. Battling the Conundrum of Bleeding and Clotting: Initiation of Venous Thromboembolism Prophylaxis in Traumatic Brain Injury after Neurosurgical Intervention.

BACKGROUND: There are no consensus guidelines demonstrating that early venous thromboembolism prophylaxis (VTEP) is safe in patients with traumatic brain injury (TBI) who undergo neurosurgical interventions. We hypothesized that early initiation of VTEP in patients with TBI would decrease the incidence of venous thromboembolism (VTE) and intracranial hemorrhage expansion (ICHE). STUDY DESIGN: This was a retrospective single-center study of adult patients with TBI who underwent neurosurgical intervention from 2012 to 2023 at a level 1 trauma center.
