A Rare Pediatric Case of Anterior Radial Head Dislocation Associated With Volar Annular Ligament Avulsion Fracture.

An 11-year-old girl sustained an irreducible anterior radial head dislocation associated with a Salter-Harris type II proximal radial physeal injury and a volar annular ligament avulsion fracture after falling on an outstretched hand. Closed reduction was unsuccessful. During open reduction, the annular ligament was found interposed within the radiocapitellar joint, avulsed with a small bony fragment from the volar aspect of the radial notch. After ligament release, the radial head remained unstable, requiring anatomical repair using a pullout suture technique. At 1-year follow-up, the patient

Use of Endo GIA™ With Tri-staple™ and Local Muscle Flaps in Neopharyngeal Reconstruction Following Salvage Total Laryngectomy.

Salvage total laryngectomy (STLx) is associated with higher complication rates than primary procedures, particularly in patients with a history of radiation therapy, where delayed wound healing and pharyngocutaneous fistula remain major challenges. Commonly recommended approaches, such as pedicled or free tissue flaps, may reduce complications but often prolong operative time, enlarge the surgical field, and add morbidity. In this technical report, we describe a neopharyngeal reconstruction technique using Endo GIA™ with Tri-Staple™ Technology (Covidien, North Haven, Connecticut), reinforced w

The "Belt and Suspenders" Repair Approach to Intraoperative Sagittal Sinus Injury During Cranial Vault Surgery.

This case report detailed a successful surgical repair of a superior sagittal sinus (SSS) injury during a 16-month-old girl's craniosynostosis surgery. The patient presented with symptoms of increased intracranial pressure including irritability, stumbling, and sleep disturbances, stemming from metopic craniosynostosis. During the fronto-orbital advancement procedure, an iatrogenic SSS injury was sustained at the anterior fontanel. The surgical team used an innovative "belt and suspenders" repair technique. This approach involved immediate bleeding control with finger pressure, followed by Gel

Early Functional and Radiological Outcomes of Radial Head Replacement in the Management of Radial Head Fractures: A Prospective Study.

Background Radial head fractures, particularly Mason type III and IV, often require radial head replacement (RHR) to restore elbow stability and function when fractures are unreconstructable. This study evaluates the short-term functional and radiological outcomes of RHR in managing complex radial head fractures Methods A prospective study was conducted from January 2023 to January 2025 at Government Medical College, Amritsar, India, involving 25 patients with Mason type III (n=21, 84%) and type IV (n=4, 16%) radial head fractures treated with monopolar cemented RHR. Functional outcomes were a

Severe Craniocerebral Injury in a Child Caused by Repetitive Cleaver Strikes: Case Report and Review of the Literature.

Severe craniocerebral injuries are rare in the pediatric population, especially open craniocerebral injuries caused by repeated slashes with sharp instruments. Here, we report an exceptional case of a child with severe open craniocerebral injury caused by repeated chopping blows with a cleaver. The child was admitted with traumatic hemorrhagic shock, and a multidisciplinary team (MDT) was immediately organized for evaluation and treatment decisions. Preoperative imaging showed a comminuted skull fracture accompanied by extensive scalp laceration and brain tissue injury with active hemorrhage.

Reconstruction of the right side of the abdominal wall due to post-traumatic muscle avulsion.

Introduction: Traumatic detachment of the lateral abdominal muscle group from the iliac crest occurs as a result of the acceleration mechanism when the body impacts a seatbelt during a head-on collision of motor vehicles. Due to the rare and variable nature of such defects, a standard treatment protocol has not yet been developed. As a result, the surgical technique and timing of the procedure are always planned individually. The optimal surgical method and type of implant for such cases remain undetermined. Given the anatomical location of the defect, surgical intervention

Catastrophic iatrogenic abdominal aorta haemorrhage during anterior resection.

A woman in her 70s with hypertension, breast cancer and diverticulosis underwent laparoscopic anterior resection for a tubule-villous adenoma, converted to open Hartmann's with aorto-bi-iliac bypass due to a vascular injury. Intraoperative complications included haem-o-lok penetration of the calcified aorta, necessitating vascular team intervention. Postoperative issues included bilateral popliteal artery emboli requiring embolectomy and fasciotomy, and a parastomal abscess. Despite significant blood loss and complications, the patient recovered well and remains cancer-free. This case undersco

Redefining Bladder Dissection in Robotic Hysterectomy with Previous Cesarean Section: A "Head-On Approach".

Intraoperative adhesions, particularly between the bladder and cesarean scar, as well as between the uterus and anterior abdominal wall, are commonly encountered in patients with a history of cesarean sections. These adhesions substantially elevate the risk of severe intraoperative complications, including bowel or bladder injuries, during hysterectomy. The incidence of bladder complications is significantly lower in patients with one or two prior cesarean sections compared to those with three or more. Moreover, the likelihood of inadvertent cystotomy is several times higher in patients with t

Endovascular treatment of a traumatic aortic transection via implantation of self-expanding nitinol stent with open-cell design: a case report.

BACKGROUND: Traumatic aortic transection is a life-threatening injury with an acute mortality rate approaching 80%, typically resulting from high-energy blunt trauma, such as motor vehicle collisions. Without intervention, the overall mortality rate increases to 90-94%, primarily due to the rupture of pseudoaneurysms or expanding hematoma leading to exsanguination. Although delayed surgical repair may decrease immediate mortality, it is associated with significant perioperative risks. Endovascular techniques are becoming increasingly prevalent and have demonstrated lower morbidity and mortalit

Factors Associated With Persistent Symptoms Following Mild Traumatic Brain Injury.

IMPORTANCE: Traumatic brain injury (TBI) is a common presentation in which it is clinically difficult at presentation to identify patients at risk for persistent symptoms, defined as lasting longer than 30 days. Few studies report the time period from injury to presentation; thus, it may be useful to determine early findings that identify patients who are likely to have persistent 30-day symptoms. OBJECTIVE: To identify parameters associated with persistent 30-day symptoms in adult emergency department (ED) patients presenting shortly after a mild TBI (mTBI). DESIGN, SETTING, AND PARTICIPANT

A case of open skull fracture, subdural hematoma, and brain contusion complicated by traumatic brain injury-associated coagulopathy caused by a lawn trimmer; A case report.

Penetrating head injury is less common than blunt head injury in Japan but causes devastating complications and requires prompt surgical intervention. However, not all penetrating head injuries are apparent on arrival. We herein present a case of a 74-year-old male who lost his balance on a steep hillside and fell on an active lawn trimmer with rotating circular blades, which hit his face. On arrival, the patient was alert and conscious, but was unable to recall the accident. His vital signs were stable and motor functions were normal. The patient had a large laceration ranging from the right

Radial Head Fractures: Current Trends in Treatment and Outcomes, A Retrospective Review.

BACKGROUND: Radial head fractures are common elbow fractures in adults, and their treatment algorithm remains controversial. Particularly with displaced Mason III or IV radial head fractures, studies have shown mixed results when comparing outcomes between radial head arthroplasty (RHA) and open reduction internal fixation (ORIF). Although multifactorial, patient characteristics often play a large role in treatment modality. The purpose of this study was to analyze the current trend in the management of radial head fractures, complications, and need for repeat operation among these groups. MA

Clinical Outcomes of Fully Arthroscopic Versus Arthroscopically Assisted Latissimus Dorsi Transfer for Irreparable Subscapularis Tear.

BACKGROUND: Untreated chronic subscapularis (SSC) tears pose a challenging problem to treat owing to the resultant tendon retraction, atrophy, fatty infiltration, and changes in humeral head position, which complicate surgical options. Anterior latissimus dorsi (LD) transfer has shown effective results in treating these tears without glenohumeral arthritis. PURPOSE/HYPOTHESIS: This study introduces and evaluates fully arthroscopic and arthroscopically assisted anterior LD transfer techniques for reconstructing irreparable SSC tears. The authors hypothesize that both methods will yield promisi
