Free flap versus pedicled flap reconstruction in head and neck surgery: A systematic review and meta-analysis.

BACKGROUND: Free and pedicled flaps remain the principal reconstructive options following head and neck oncologic surgery. Although free flaps are most frequently used, pedicled flaps have re-emerged with modern techniques offering comparable outcomes with reduced complexity. The aim was to systematically compare surgical, functional and economic outcomes between free and pedicled flaps in head and neck reconstruction. METHODS: A systematic search of PubMed and Embase was conducted from database inception to September 2025, following PRISMA guidelines. Comparative studies evaluating free vers

Independent Risk Factors for Lag Screw Cut-Out: The Role of Open Reduction in Intertrochanteric Fracture Fixation With the Proximal Femoral Nail.

BACKGROUND Intertrochanteric femoral fractures are common in older adults, and proximal femoral nail (PFN) fixation is widely preferred due to its biomechanical advantages. Lag screw cut-out, defined as the progressive migration of the lag screw through the femoral head with loss of fixation, is one of the most serious mechanical complications after PFN fixation. This complication can result in mechanical failure and require revision surgery. Therefore, this study aimed to identify independent risk factors for lag screw cut-out and evaluate lag screw stability in patients without cut-out. MATE

Ultrasound-guided pericapsular nerve group block versus intravenous morphine for pain management in older adults with hip fractures: a randomised controlled trial in the emergency department.

BACKGROUND: Hip fracture pain is severe and is commonly managed with intravenous opioids that cause adverse effects in older adults. Although the use of regional anaesthesia is increasing, high-quality evidence remains limited, with insufficient randomised controlled trial (RCT) evaluating the effectiveness of the pericapsular nerve group (PENG) block in the emergency department (ED). This study aimed to compare the analgesic efficacy of ultrasound-guided PENG block with intravenous morphine for acute hip fracture pain in older adults presenting to the ED. METHODS: This open label, 1:1 parall

Transoral endoscopic thyroidectomy vestibular approach versus open thyroidectomy in surgical resection of the thyroid: a systematic review and meta-analysis.

BACKGROUND: The increasing incidence of thyroid nodules and thyroid cancer, particularly among women, has driven the evolution of minimally invasive thyroidectomy techniques. The transoral endoscopic thyroidectomy vestibular approach (TOETVA) has emerged as a promising alternative to conventional open thyroidectomy (OT). This study aimed to compare operative time, blood loss, drainage volume, lymph node yield, and postoperative complications between TOETVA and OT. METHODS: A comprehensive search was conducted across PubMed, Embase, ClinicalTrials.gov, and Web of Science. Randomized controlled

Recurrent Cochlear Implant Electrode Exposure in a Smoker With Prior Canal Wall Mastoidectomy: A Case Report.

Cochlear implantation (CI) is an established and effective surgical intervention for patients with severe to profound sensorineural hearing loss. The procedure involves placement of an internal receiver-stimulator in a postauricular subperiosteal pocket with an electrode advanced through the mastoid and facial recess into the cochlea. Long-term device function depends on secure fixation and durable soft tissue coverage over both the implant and extracochlear electrode segment to prevent skin breakdown, infection, and electrode exposure. Although electrode exposure is uncommon, it represents a

Early post-traumatic avascular necrosis with secondary osteoarthritis following an acetabular fracture in a 19-year-old patient treated with total hip arthroplasty: a case report.

INTRODUCTION AND IMPORTANCE: Acetabular fractures are complex orthopedic injuries that can lead to severe long-term complications, including avascular necrosis (AVN) of the femoral head and secondary osteoarthritis. These sequelae may result in progressive joint destruction and may ultimately require total hip arthroplasty (THA), particularly in cases where joint preservation is no longer feasible. CASE PRESENTATION: We report a case of a 19-year-old male presenting with progressive left hip pain and a restricted range of motion 1 year after sustaining a posterior column acetabular fracture a

Operative time in acetabular fracture fixation: predictors and impact on postoperative outcomes.

BACKGROUND: Surgical fixation of acetabular fractures is technically demanding, and operative time is often treated as a surrogate for surgical complexity and a presumed driver of complications. Whether prolonged operative time reflects identifiable preoperative factors, and whether it independently worsens outcomes, remains unclear. We aimed to identify factors associated with prolonged operative time in acetabular fracture fixation and to determine whether longer operative time is associated with worse postoperative outcomes. METHODS: We retrospectively reviewed patients undergoing surgical

Open 4-Compartment Fasciotomy for Chronic Exertional Compartment Syndrome of the Lower Extremity.

BACKGROUND: Chronic exertional compartment syndrome (CECS) is an underrecognized cause of lower extremity dysfunction in athletes. Open 4-compartment fasciotomy remains the gold standard for surgical management. INDICATIONS: Operative intervention is indicated for patients with clinically and objectively confirmed CECS who fail to respond to conservative management. Contraindications include inadequate nonoperative treatment or unaddressed medical comorbidities. Proper diagnosis requires exclusion of vascular or neurologic causes that can mimic CECS symptoms. TECHNIQUE DESCRIPTION: A 2-incis

Home-cage monitoring as a sensitive tool for detecting subtle behavioral alterations following mild traumatic brain injury.

Home-cage monitoring (HCM) has emerged as a powerful approach for detecting subtle and context-dependent behavioral alterations in rodent models. In this mini-review, we integrate clinical observations and preclinical data from the closed-head weight-drop model, emphasizing studies from our laboratory, to argue that mTBI should be conceptualized as a disorder of behavioral regulation rather than of basic behavioral capacity. We first outline the limitations of short, novelty-driven gold-standard assays (e.g., Open Field, Elevated Plus Maze, Rotarod), which often indicate intact motor function

Excellent functional outcomes and low complication rate following open reduction for severe pediatric radial neck fractures: A retrospective analysis.

BACKGROUND: Pediatric radial neck fractures, though rare, pose significant management challenges with several treatment options. Open reduction is typically used for more complex cases, where greater fracture severity may contribute to higher complication rates and poorer outcomes. This study aims to evaluate functional outcomes, complications, and risk factors in a large cohort of pediatric patients treated with open reduction, performed after unsuccessful closed reduction. METHODS: A retrospective cohort study was conducted on 53 pediatric patients (mean age 8 years) with radial neck fractu

Osteosynthesis versus arthroplasty in the management of radial head fractures: a retrospective cohort study.

BACKGROUND: Radial head fractures are among the most common elbow injuries in adults. The choice between osteosynthesis and radial head arthroplasty (RHA) remains controversial, particularly in comminuted or unstable fracture patterns. This study aims to compare functional and radiographic outcomes, complication rates, and patient satisfaction following osteosynthesis or arthroplasty in the surgical management of radial head fractures. METHODS: A retrospective cohort of 35 patients treated surgically for radial head fractures between 2018 and 2022 was analyzed. Fractures were classified using

Outcomes of Press-Fit Use of a Cementable Monoblock Radial Head Prosthesis in Complex Elbow Trauma: A Retrospective Observational Study.

BACKGROUND: Radial head arthroplasty is the preferred surgical strategy for unreconstructable radial head fractures associated with complex elbow injuries. Single-diameter monoblock prostheses remain widely used and are sometimes implanted off-label as press-fit, even when the manufacturer's instructions specify cementation. Outcome data on this practice are scarce. We aimed to describe the clinical, functional, and radiographic outcomes of a single-diameter monoblock partial radial head prosthesis implanted off-label as a press-fit for complex elbow trauma. METHODS: We retrospectively review

Beyond the Bleed: Characterization of Traumatic Head and Neck Pseudoaneurysms in the Facial Trauma Patient.

Traumatic head and neck pseudoaneurysms are rare, potentially life-threatening vascular injuries that may accompany craniomaxillofacial trauma. This retrospective observational cohort study characterizes the presentation, management, and outcomes of these injuries in civilian patients treated at academic level I to II urban trauma centers from January 2009 to May 2025. Patients were identified using ICD-9/10 codes, and variables included demographics, injury mechanism, vessel location, imaging, associated injuries, treatment modality, complications, and outcomes. The primary outcome was the ra

Ballistic head and neck injuries: management rationale and outcomes in a case series.

INTRODUCTION: Penetrating head and neck injuries with retained foreign bodies pose significant management challenges due to the anatomical density of vital structures. The optimal therapeutic strategy - ranging from surgical extraction to conservative observation - remains guided by individual patient and injury characteristics. METHODS: This case series retrospectively reviews four distinct presentations of retained metallic foreign bodies in the head and neck region. Diagnostic evaluation utilized advanced computed tomography (CT) for precise localization. Management strategies were individ

Open repairs with biceps rerouting does not impact retears in large to massive rotator cuff tears compared to conventional open repair: a randomized clinical trial.

BACKGROUND: The treatment of large to massive rotator cuff tears leads to a high failure rate and several techniques have been proposed to improve these results. Rerouting the long head of the biceps tendon (LHBT) appears to be a biological reinforcement to increase tendon healing. Therefore, this study aimed to compare the clinical and radiological outcomes of open repair for large and massive rotator cuff tears reinforced with the LHBT to conventional open repair. METHODS: A prospective randomized study was conducted with patients diagnosed with large to massive rotator posterosuperior rota
