Facial Artery Pseudoaneurysms: A Systematic Review.

INTRODUCTION: Facial artery pseudoaneurysm is a rare but serious condition that can develop after traumatic or iatrogenic injury. If undetected or untreated, it risks rupture, massive bleeding, and damage to nearby structures, potentially leading to life-threatening complications. Prompt diagnosis and treatment are essential to prevent these risks, with options including surgical repair or endovascular embolization. METHODS: This systematic review followed PRISMA guidelines and conducted comprehensive searches of MEDLINE, Embase, Web of Science, Cochrane Central, and ClinicalTrials.gov for st

Cervicothoracic screwdriver impalement causing traumatic aortic injury managed with TEVAR.

Penetrating neck injuries are uncommon but carry high mortality due to the proximity of major vessels and the aerodigestive tract. We report a woman in her 80s who presented with a 40-cm flat-head screwdriver impaled in the right lower neck at the thoracic inlet. Computed tomographic angiography (CTA) demonstrated a distal thoracic aortic arch pseudoaneurysm with mediastinal haemorrhage. Although initially haemodynamically stable, she developed airway compromise and underwent emergent open tracheostomy in a hybrid operating room. Thoracic endovascular aortic repair excluded the pseudoaneurysm

Biceps tenodesis with 360° suture anchor versus self-locking tenodesis in the absence of full-thickness rotator cuff tears: protocol for a non-inferiority randomised controlled trial (BLAST-2).

INTRODUCTION: Pathology of the long head of the biceps tendon (LHBT) is commonly treated with either tenotomy or tenodesis, which provide comparable clinical outcomes but have distinct disadvantages. A novel self-locking biceps tenodesis aims to combine the benefits of both techniques. Comparative evidence in patients without full-thickness rotator cuff tears is limited. This study aims to compare the clinical outcomes and complications of self-locking biceps tenodesis and biceps tenodesis with 360° suture anchor in patients without full-thickness rotator cuff tears. METHODS AND ANALYSIS: Thi

Risk factors for post-traumatic osteoarthritis following surgical treatment of acetabular posterior wall fractures: a retrospective study.

Posterior wall fractures, commonly caused by high-energy mechanisms like motor vehicle collisions or falls, are frequently associated with hip dislocation and complex intra-articular injuries. Despite surgical advances, post-traumatic osteoarthritis (PTOA) remains a prevalent complication, often leading to significant functional impairment and eventual joint replacement. Identifying potentially modifiable risk factors may help improve long-term outcomes. This study aimed to identify independent risk factors associated with the development of post-traumatic osteoarthritis (PTOA) following surgi

Data from a UK-based multicentre randomised feasibility study investigating the management of post-traumatic benign paroxysmal positional vertigo: mixed-methods analyses.

OBJECTIVES: Traumatic brain injury commonly causes dizziness and balance problems. Benign paroxysmal positional vertigo (BPPV) is the most frequent cause of inner-ear related post-traumatic vestibular dysfunction. However, optimal assessment and treatment practices are poorly evidenced. Using a mixed methods approach, we aimed to explore the feasibility of managing post-traumatic BPPV. DESIGN: A mixed-methods randomised feasibility study. In this paper, quantitative and qualitative data relating to key feasibility targets around recruitment, randomisation and assessment and treatment were sel

Combined ipsilateral fracture of the tibial shaft, lateral malleolus, and talar neck: A case report.

BACKGROUND: Talar fractures, accounting for approximately 2% of all lower extremity injuries and 5%-7% of foot injuries, are common and typically involve the talar head and neck. Avascular necrosis of the talus is one of the most challenging complications encountered in post-traumatic hindfoot reconstruction. CASE PRESENTATION: A 21-year-old man presented to the emergency department with an open wound, deformity, and multiple traumatic injuries to the right lower limb following a motorcycle accident. Examination of the right lower extremity revealed a 3-cm open wound on the distal third of th

Subcapital Realignment of the Displaced Pediatric Radial Head-Technical Description and Early Clinical Results.

Background: Displaced pediatric radial neck fractures are challenging injuries. Adequate reduction can be difficult to achieve via closed methods. Open reduction has been associated with avascular necrosis and growth arrest due to soft tissue stripping during surgery. We describe a modified technique that adopts lessons learned from subcapital realignment procedures of the proximal femur, prioritising preservation of the periosteal sleeve with the aim of reducing complications associated with open reduction. Methods: The modified technique includes visualisation of the radial neck fracture thr

Single-Stage Fixation of Bilateral Open Medial Hoffa Fractures Associated with Bilateral Patella Fracture and Bilateral Distal Radius Fractures: A Case Report.

INTRODUCTION: Posterior cruciate ligament (PCL) avulsion fractures, lateral collateral ligament (LCL)-biceps femoris complex avulsion injuries are relatively uncommon injuries, and associated diaphyseal tibia fracture further is the rarest combination. It is challenging to diagnose such complex injuries, and missed injuries, particularly at the knee can lead to significant instability, increasing patients' morbidity. CASE REPORT: A 25-year-old male patient was diagnosed with PCL avulsion fracture, LCL - biceps femoris complex injury at the fibular side with Type I open mid-shaft both-bone leg

Femoral head fractures: anatomy, diagnosis and management.

Femoral head fractures are complex and severe injuries, usually associated with hip dislocation. They typically result from high-energy trauma. Therefore, a low index of suspicion is required for diagnosis in these contexts. Initial presentation can vary depending on coexisting injuries but is typically an emergency and requires immediate reduction of the joint. Delays result in worse outcomes for patients. Pelvic radiographs are recommended before and after joint reduction, with Judet, inlet and outlet views to identify any associated acetabular fracture and pelvic ring injury. Computed tomog

Distal radius T-shaped locking plate with suture anchors for humeral greater tuberosity fracture with rotator cuff injury: A case report.

Greater tuberosity (GT) fractures with rotator cuff injuries are challenging due to risks of nonunion and functional loss. We report a case treated with a distal radius locking plate combined with suture anchors. A female patient presented with shoulder pain and immobility 4 weeks after trauma, diagnosed with humeral head dislocation and rotator cuff tear. She underwent minimally invasive fixation using this combined technique, which provides stable fracture reduction while repairing the rotator cuff. At the 18-month follow-up, shoulder function was fully restored with no complications. This c

Temporary Cement Prosthesis as a Bridge After Control of Axillary Artery Hemorrhage in Proximal Humeral Fracture-Dislocation: A Case Report.

CASE: A 64-year-old woman underwent open reduction and internal fixation for four-part proximal humeral fracture-dislocation with palpable distal pulses. Upon dislocated head removal, life-threatening hemorrhage occurred from posterior circumflex humeral artery tear. Bleeding was controlled through packing, resuscitation, and open vascular repair. Owing to unavailable shoulder prosthesis and prolonged operative time, a temporary cement prosthesis with Steinmann pins was used. At the 3-year follow-up, partial function was restored with no loosening. CONCLUSION: This case highlights axillary br

Pediatric Phalangeal Neck Fractures: To Fix or Not to Fix?

Pediatric phalangeal neck fractures occur distal to the collateral ligament recess, often with apex volar angulation. These fractures are at an increased risk for various complications including malunion, nonunion, stiffness, and osteonecrosis. The Al-Qattan classification system categorizes these fractures into three types: type I fractures are nondisplaced, type II fractures are displaced while maintaining cortical contact, and type III fractures lack cortical contact with associated malrotation or partial amputation. Type I fractures are typically managed nonsurgically. Recent reports have

Proximal humerus fractures treated with single or dual-plate constructs: a comparison of clinical outcomes.

BACKGROUND: Proximal humerus fractures (PHFs) are common injuries in the adult population. Open reduction and internal fixation (ORIF) still represents the most common treatment for reconstructable injuries. Despite the continued prevalence of ORIF, studies have demonstrated fixation failure ranging from 11% to 18% and a reoperation rate of up to 27%. Dual plating is an emerging technique that has been noted to enhance fracture fixation. There is only one comparative study examining the outcomes of isolated locked plating vs. dual plating. Therefore, our objective was to compare the effect of

A retrospective cohort study comparing combined TiRobot navigation and percutaneous minimally invasive FNS fixation versus conventional open reduction FNS for femoral neck fractures.

Femoral neck fractures are prone to mechanical failure and severe complications due to high inter-fragmentary shear forces. The Femoral Neck System (FNS) provides angular-stable biomechanical fixation, where precise intra-osseous implant positioning directly determines long-term clinical outcomes. Ti-Robot orthopedic navigation enables preoperative three-dimensional trajectory planning and intra-operative guided screw placement, which may improve implant accuracy while reducing surgical soft tissue trauma. A critical methodological caveat applies to this comparison: the two surgical arms diffe

Traumatic Bilateral Asymmetric Hip Dislocation With a Rotated Pipkin Type II Femoral Head Fracture.

Bilateral asymmetric hip dislocation is a rare injury with a complex mechanism, and treatment of delayed cases is difficult with unpredictable outcomes. We report a case of a 25-year-old man with bilateral asymmetric hip joint dislocation (right anterior, left posterior) and a Pipkin type II left femoral head fracture. His dislocations were reduced under sedation within 4 hours of the accident, but the surgical fixation of his fracture was delayed for 4 days. Hip dislocation should be reduced within 6 hours of injury. Postreduction CT in high-energy trauma is especially crucial for detecting a

Unexpected Sequela of Shoulder Dystocia: Neonatal Humerus Fracture.

Shoulder dystocia is an obstetric emergency defined by failure of the fetal shoulders to deliver after expulsion of the head, most often due to impaction of the anterior shoulder behind the pubic symphysis. Although uncommon and considered largely unpredictable, shoulder dystocia carries the risk of significant neonatal morbidity. While brachial plexus injury is the most frequently reported complication, skeletal injuries such as humeral fractures are less commonly described. A 28-year-old primigravid woman with obesity and anemia presented at 38 weeks and 6 days of gestation with spontaneous
