Management of a Rare Combined Osseoligamentous Knee Injury: Tibial Plateau Fracture with High-Grade Posterior Cruciate Ligament Tear Treated by Arthroscopic Reconstruction and Osteosynthesis - A Case Report.

INTRODUCTION: Tibial plateau fractures (TPFs) are intra-articular injuries accounting for approximately 1% of all fractures and are frequently associated with soft-tissue and ligamentous injuries. Medial condyle fractures (Schatzker type IV) are uncommon but often result in instability, particularly when associated with posterior cruciate ligament (PCL) injury. Such combined injuries are complex and may be underdiagnosed, especially following delayed presentation. We report a rare case of a medial TPF with high-grade PCL injury managed using a combined arthroscopic and open surgical approach.

Hip Fractures: A Review.

IMPORTANCE: More than 14.2 million people worldwide and 280 000 in the US experience a hip fracture each year. The median 1-year mortality rate after a hip fracture is 22% and approximately 42% to 71% of patients regain their prefracture level of basic activities of daily living within 6 months. OBSERVATIONS: Hip fractures are classified as intracapsular or extracapsular and most commonly occur after a fall. Intracapsular hip fractures include femoral neck (34%) and femoral head (rare). Extracapsular fractures consist of intertrochanteric (48%) and subtrochanteric fractures (5.8%). In the US,

Neglected traumatic hip dislocation: a systematic review of case reports and case series on management, outcomes, and complications.

BACKGROUND: Neglected traumatic hip dislocation (NTHD) is a challenging orthopaedic condition predominantly encountered in developing countries, where delayed presentation is attributable to socioeconomic barriers, lack of awareness, and reliance on traditional bone setters. Unlike acute hip dislocations, which demand emergent reduction within 6 h, neglected cases present unique reconstructive challenges including soft-tissue contracture, acetabular fibrous tissue in-growth, neo-acetabulum formation, avascular necrosis (AVN), and limb-length discrepancy. The objective of this work was to syste

Long-term outcomes of the modified Dunn procedure in moderate and severe slipped capital femoral epiphysis: a prospective case series with 7-year follow-up.

INTRODUCTION: Slipped capital femoral epiphysis (SCFE) is the most common hip disorder in adolescents and may lead to femoroacetabular impingement, early osteoarthritis, and long-term functional disability if inadequately treated. While in situ pinning remains the standard treatment for mild slips, it fails to correct the deformity in moderate and severe cases, potentially predisposing to degenerative changes. The modified Dunn procedure (MDP) was developed to restore proximal femoral anatomy through surgical hip dislocation while preserving vascular supply. The aim of the study is to evaluate

Bilateral Electric Cataracts With Markedly Disparate Onset Times.

Ocular injuries secondary to high-voltage electrocution are uncommon and may lead to delayed manifestations, particularly cataract formation. In this article, we describe the case of a 21-year-old male diagnosed with a unilateral electric cataract. The patient had previously undergone cataract surgery in the left eye following a high-voltage electric shock sustained at the age of seven and presented years later with decreased vision in the right eye. Clinical examination revealed an anterior subcapsular cataract in the right eye, with no other associated lesions, particularly involving the ret

Image-guided puncture drainage as a first-line management for postoperative abscess following head and neck reconstruction: A case series.

OBJECTIVE: Postoperative abscess formation remains a clinically significant complication following head and neck reconstruction. It may prolong hospitalization, delay rehabilitation, and complicate postoperative recovery. With advancements in interventional imaging, image-guided puncture drainage (IGD) has emerged as a minimally invasive management option; however, its appropriate indications remain unclear. This study aimed to characterize postoperative abscesses and evaluate the clinical effectiveness of IGD as a first-line management strategy in selected patients undergoing head and neck re

Reconsidering Surgical Intervention for Infection After Cranioplasty in Older Adults: A Case Report.

Post-cranioplasty infection with scalp breakdown in older adults poses a difficult reconstructive problem, particularly when frailty and cognitive impairment may limit tolerance for staged procedures. Although debridement, infection control, and delayed cranioplasty are often considered standard management, the extent of reconstruction that should be pursued in elderly patients remains uncertain. We report the case of a woman in her late 80s who developed scalp loss with exposure of a synthetic cranial implant eight months after cranioplasty following surgery for acute subdural hematoma. [Endoscopic and Combined Skull Base Surgery - The Experience of Shaare Zedek Medical Center].

INTRODUCTION: The anterior skull base forms the bony floor of the anterior cranial fossa and serves as a barrier between the sinuses and nasal cavity and the brain. The most common pathologies in this area include tumors (benign and malignant), inflammatory and infectious diseases, congenital anomalies, and traumatic injuries. Anterior skull base surgeries are performed as part of a multidisciplinary team effort. In the past, these surgeries were performed using open approaches, but with the advancement of endoscopic technology, most surgeries are now performed using minimally invasive techniq

Skull base CSF leak repair in the outpatient Arena: Pushing the boundaries of modern endoscopic surgery.

BACKGROUND: Skull base defects with cerebrospinal fluid (CSF) leak have been a significant clinical challenge due to the risk of life-threatening complications such as meningitis, pneumocephalus and intracranial abscess formation. Traditionally, these defects have been managed in the operating room under general anesthesia, using either open craniotomy or endoscopic endonasal approaches. However, with advances in high-definition endoscopy, specialized instruments and biomaterials for reconstruction, there is a growing chance of performing selected repairs in an outpatient setting under local a

Acellular Dermal Substitutes for Lining the Exenterated Orbit-A Systematic Review and Meta-Analysis of Clinical Outcomes.

Orbital exenteration reconstruction remains a major reconstructive challenge because of the orbit's complex anatomy and its close relationship with the paranasal sinuses, which predispose patients to delayed healing. Traditional approaches, such as secondary intention healing or split-thickness skin grafting, may result in prolonged epithelialization and can compromise timely prosthetic rehabilitation. Acellular dermal matrices (ADM) have emerged as biological scaffolds that provide immediate coverage of the defect while preserving an open orbital cavity suitable for prosthetic fitting. A syst

Timing to restart direct oral anticoagulants after traumatic intracranial haemorrhage (RESTART tICrH): study protocol for a randomised controlled multicentre trial.

BACKGROUND: Head injury in older adults is increasingly common, often caused by low-impact falls. A significant proportion of these patients are taking oral anticoagulants (OACs) to prevent stroke or thromboembolism. When traumatic bleeding within the head occurs, OACs are usually stopped to reduce the risk of worsening haemorrhage; however, prolonged interruption increases the risk of stroke, systemic thromboembolism and death. There is currently no consensus on when it is safest to restart OACs. Direct oral anticoagulants (DOACs) are now widely prescribed and may offer advantages over warfar
