Closed versus open operative treatment of nondisplaced diacapitular (Class VI) fractures.

PURPOSE: The purpose of the study was assessment of whether open reduction and internal fixation of high nondisplaced, nondislocated diacapitular fractures (Class VI according to Spiessl and Schroll) have better 1-year results compared with closed treatment. PATIENTS AND METHODS: Twenty-two patients treated 2001 to 2005 with 26 (4 double) Class VI fractures prospectively entered this evaluation; in randomized fashion 9 (41%) patients had open reduction and internal fixation, 13 (59%) had closed treatment. Facial symmetry, nerve function, scarring, pain, and interincisal maximum distance were

Tactile dysgeusia: a new clinical observation of middle ear and skull base surgery.

OBJECTIVE: To describe a new clinical observation of "tactile dysgeusia," a phenomenon associated with otologic and skull base surgery likely caused by injury to the chorda tympani nerve (CTN) or the nervus intermedius (NI) with subsequent aberrant cross-innervation with somatosensory fibers in the surgical field. STUDY DESIGN: Descriptive case series. SETTING: Tertiary university referral center. METHOD: Eight patients described a sensation of touch-evoked dysgeusia after surgery. Seven patients underwent a variety of middle ear procedures including stapedotomies and mastoidectomies. One p

Identification and monitoring of the recurrent laryngeal nerve during thyroidectomy.

Careful dissection of the recurrent laryngeal nerve (RLN) represents perhaps the most critical component of thyroidectomy. It long has been established that routine identification of the nerve reduces the risk of iatrogenic injury. In recent years, much attention has been paid to the role that functional monitoring plays in identification and preservation of the RLN. This article explores methods for detecting and identifying the RLN. It then examines the evolution of functional RLN monitoring, its potential advantages and disadvantages, statistical validity, and its role in the current medico

Laryngeal reinnervation with nerve-nerve anastomosis versus laryngeal framework surgery alone: a comparison of safety.

OBJECTIVE: To characterize any additional risks and rate of complication associated with laryngeal reinnervation in comparison to traditional laryngeal framework surgery. DESIGN: Retrospective chart review over a 5-year period. RESULTS: A total of 16 reinnervation procedures were performed to rehabilitate a paralyzed vocal fold. Of the 16 procedures, 12 (75%) were combined with arytenoid adduction. One reinnervation was aborted due to presence of scar and vascular malformation in the tracheal-esophageal groove. There were no major complications (airway obstruction or death). There were no ca

The use of polydimethylsiloxane for injection laryngoplasty.

BACKGROUND: Injuries of the recurrent laryngeal nerve with consecutive vocal cord paralysis is a typical complication in chest, esophageal, thyroideal, and neck surgery. Glottic insufficiency secondary to such a lesion can be treated by endolaryngeal vocal cord augmentation (injection laryngoplasty). Many different substances have been used, often showing complications or disadvantages. This study reports on the use of injectable polydimethylsiloxane (PDMS), with special regard to the long-term results. METHODS: In this prospective study, 21 patients with unilateral vocal cord paralysis under

The mechanism of recurrent laryngeal nerve injury during thyroid surgery--the application of intraoperative neuromonitoring.

BACKGROUND: Identification of recurrent laryngeal nerve (RLN) has decreased the rates of permanent RLN palsy during thyroid operations; however, unexpected RLN palsy still occurs, even though the visual integrity was assured and most nerve injuries were not recognized intraoperatively. The aim of this study is to determine the causes of RLN palsy and to identify potentially reversible causes of RLN injury during the operation with the application of intraoperative neuromonitoring (IONM). METHODS: One hundred and thirteen patients with 173 nerves at risk were enrolled in this study. All operat

Mini-retromandibular approach to condylar fractures.

INTRODUCTION: Among maxillofacial surgeons, a general agreement exists that the therapeutic strategy for intracapsular condylar fractures is conservative, while the treatment of extracapsular fractures of the mandibular condyle is extremely controversial. The indications and choice of treatment are less than uniform, often relying on the surgeon's personal experience and beliefs. The literature increasingly suggests that the surgical management of these fractures is superior to conservative management in functional terms. Nonetheless, the indications for surgically treating condylar fractures

A prospective, randomized trial for use of prednisolone in patients with facial nerve paralysis after parotidectomy.

BACKGROUND: A high rate of facial nerve paralysis (FNP) develops after parotid surgery, but there have been few clinical trials on treatments to improve recovery times. This study evaluated the efficacy of prednisolone in these patients. METHODS: A total of 123 patients who underwent parotidectomy without injury to the facial nerve were assessed prospectively for postoperative FNP. These patients were assigned randomly to treatment with prednisolone or placebo for 10 days. FNP was evaluated by House-Brackmann grades. Recovery rates and adverse effects were monitored regularly for 6 months aft

Prevention of complications in revision endocrine surgery of the head & neck.

Revision cervical endocrine surgery increases risks for injury to important adjacent structures, possibly resulting in hoarseness and postoperative hypocalcemia. With anatomic knowledge and appropriate use of meticulous dissection techniques, the surgeon strives to minimize morbidity. Technologic advances have provided tools to aid in these difficult cases and maintain the thoroughness of resection while reducing morbidity. The judicious use of preoperative imaging can assist the surgeon in identifying the precise location of disease, thus reducing unnecessary dissection and risk to surroundin

The subclassification of schizencephaly and its clinical characterization.

We subclassified schizencephaly based on the association with optic nerve hypoplasia (ONH) and the absence of the septum pellucidum (ASP), and then characterized their clinical presentation and prognosis. The subjects of our study consisted of 10 cases with a mean age at the final evaluation of 10 years 3 months (range, 7 months to 25 years). The subclassification of schizencephaly consisted of the septo-optic dysplasia (SOD) group (n=3), with ONH and ASP; the optic hypoplasia (OHP) group (n=2), with ONH and without ASP, and; the classical group (n=5), without ONH. The subjects with an open-li

Investigation of auditory dysfunction in Leber hereditary optic neuropathy.

PURPOSE: To investigate the possibility of auditory dysfunction in patients with Leber hereditary optic neuropathy (LHON). METHODS: We prospectively recruited 10 affected patients from the north-east of England harbouring one of the three primary mitochondrial LHON mutations (3460G>A n = 3, 11778G>A n = 5 and 14484T>C n = 2). A detailed auditory history was taken and they were asked to complete a validated hearing questionnaire. Each patient then underwent a comprehensive topographic neuroauditory assessment to evaluate both middle- and inner-ear functions and the integrity of the brainstem a

Increased vagal tone accounts for the observed immune paralysis in patients with traumatic brain injury.

Traumatic brain injury (TBI) is a leading cause of death and disability, especially in the younger population. In the acute phase after TBI, patients are more vulnerable to infection, associated with a decreased immune response in vitro. The cause of this immune paralysis is poorly understood. Apart from other neurologic dysfunction, TBI also results in an increase in vagal activity. Recently, the vagus nerve has been demonstrated to exert an anti-inflammatory effect, termed the cholinergic anti-inflammatory pathway. The anti-inflammatory effects of the vagus nerve are mediated by the alpha 7

Isolated chorda tympani injury following petrous temporal fracture.

Petrous temporal fractures account for nearly a fifth of all skull base fractures but are associated with taste disturbance in only 2% of cases. Chorda tympani injuries almost invariably occur in association with other nerve injuries. We present a rare case of an isolated chorda tympani injury associated with a petrous temporal fracture. Our observations suggest that the gustatory fibres in this nerve are segregated into groups subserving different taste modalities.
