Phrenic nerve neurotization utilizing the spinal accessory nerve: technical note with potential application in patients with high cervical quadriplegia.

INTRODUCTION: High cervical quadriplegia is associated with high morbidity and mortality. Artificial respiration in these patients carries significant long-term risks such as infection, atelectasis, and respiratory failure. As phrenic nerve pacing has been proven to free many of these patients from ventilatory dependency, we hypothesized that neurotization of the phrenic nerve with the spinal accessory nerve (SAN) may offer one potential alternative to phrenic nerve stimulation via pacing and may be more efficacious and longer lasting without the complications of an implantable device. Necrosis of the skin of the chin: a possible complication of inferior alveolar nerve block injection.

BACKGROUND: The literature describes different local complications derived from a local anesthetic. The authors present a report of a case of such a complication-a complication of which they have identified only one other similar case in the literature. CASE DESCRIPTION: A 10-year-old girl received an inferior alveolar nerve block injection for a dental restoration on the right side. After the procedure, she developed pallor of the right side of the lower lip and chin that subsequently evolved into a surface ulceration. The lesion suddenly healed 15 days later. The authors ruled out self-infl

The usefulness of preoperative ultrasonographic identification of nonrecurrent inferior laryngeal nerve in neck surgery.

BACKGROUNDS AND AIMS: Nonrecurrent inferior laryngeal nerve (ILN) represents a risk factor for injury during neck surgery. It is associated to arterial abnormalities (absence of the brachiocephalic trunk and arteria lusoria) that can be identified by ultrasonography. The aim of the study was to verify the usefulness of preoperative ultrasonography in the research of nonrecurrent ILN by the means of identification of arterial abnormalities and the impact on ILN morbidity. PATIENTS AND METHODS: The study included 750 patients who underwent neck surgery with right-side ILN dissection. A preopera

Microsurgery for vestibular schwannoma after gamma knife radiosurgery.

BACKGROUND: We evaluated the clinical characteristics of microsurgery for vestibular schwannoma (VS) after failed gamma knife radiosurgery (GKS). METHOD: Twelve patients, 5 men and 7 women aged 19 to 70 years (mean 54.5 years), who underwent microsurgery after failed GKS for VS were studied retrospectively. FINDINGS: The median interval between GKS and microsurgery was 28.8 months (range, 6.6-120 months) and 4 patients had undergone previous microsurgery. The mean volume of tumour at GKS was 6.9 cm(3) (range, 0.5-19.7 cm(3)) and the mean prescription dose to the tumour margin was 12.3 Gy. Mi

Remote tentorium meningioma causing sudden sensorineural deafness.

BACKGROUND: Sudden sensorineural deafness is a well-known symptom mostly of unknown etiology. CASE DESCRIPTION: A case of sudden sensorineural deafness is reported to be caused by a small, remote, ipsilateral tentorial meningioma not compressing the vestibulocochlear nerve or auditory tract. Surgical resection of the meningioma immediately restored the patient's hearing. CONCLUSION: The authors hypothesize that the sudden sensorineural deafness resulted from a growing meningioma inducing a neurovascular compression of the vestibulocochlear nerve, the vertebral artery already being in close r

[Collet-Sicard syndrome after carotid artery dissection].

Collet-Sicard syndrome is a rare condition, defined as unilateral palsy of the last four cranial nerves. It differs from Villaret syndrome because of absence of sympathetic involvement. Collet-Sicard syndrome is most often caused by skull tumors, carotid artery dissections or head and neck trauma. We report the case of a 57-year-old man who presented palsy of the left lower cranial nerves IX-XII linked to carotid artery dissection after trivial neck injury.

Optic nerve avulsion following accidental penetrating orbital injury.

Traumatic enucleation with optic nerve avulsion following accidental penetrating orbital injury is a rare phenomenon. A 35 years old young active man was suddenly traumatized in his left orbit after falling down on a boat following collision of two running boats having no search light in a dark night. The patient was examined few hours after the event. The patient presented with severe pain around left periorbital region. He never lost consciousness. On examination right eye revealed no abnormality. There was left periorbital swelling with blood clots. Left eyeball was hanging from the orbit w

Stereotactic radiotherapy for unilateral orbital lymphoma and orbital pseudo-tumors: a planning study.

Orbital lymphoma and Grave's ophthalmopathy (GO) are successfully treated with radiation therapy. The lenses are blocked to prevent cataract formation. However, blocking of the lens by traditional methods can be difficult for tumors located anteriorly and extending into the retrobulbar space. We present a series of 3 patients with orbital lymphoma and 1 with GO treated with stereotactic intensity modulated radiation therapy (IMRT) to spare normal structures, including the lens. Three patients with orbital lymphomas and 1 with unilateral GO were treated with IMRT using a linac with stereotactic

Trigeminal schwannomas: experience with 68 cases.

Trigeminal schwannomas (TS) are rare. Only a couple of series involving a large number of cases have been reported. In the present study we aimed to analyse the clinical characteristics of TS, the surgical approaches used to treat TS, and the outcomes for patients undergoing surgical treatment for TS via retrospective analysis of departmental records. Data for 68 patients treated for TS in the Department of Neurosurgery at the All India Institute of Medical Sciences between January 1993 and December 2005 were analysed. Most patients were in the fourth decade of life, with the duration of sympt

Facial-zygomatic triangle: a relationship between the extracranial portion of facial nerve and the zygomatic arch.

BACKGROUND: This study was conducted to clarify the relationships between the extracranial portion of the facial nerve (EFN) and the zygomatic arch (ZA). METHOD: Four cadaveric heads (8 parotid regions), examined under 3-40x magnification, were dissected from lateral to medial to expose the EFN. FINDINGS: In a vertical plane just anterior to the tragus, the distance from the superior edge of the ZA to the facial nerve (FN) is, on average, 26.88 mm. The FN then courses superiorly and anteriorly, crossing the ZA 18.65 mm anterior to the tragus on average. Thus, three points can be used to depi

Techniques for dissection of the facial nerve in benign parotid surgery: a cross specialty survey of oral and maxillofacial and ear nose and throat surgeons in the UK.

We investigated techniques of dissection of the facial nerve currently being used in parotid surgery for benign disease in oral and maxillofacial (OMF) and ear, nose and throat (ENT) surgery. A postal questionnaire was sent to 300 OMF and 300 ENT consultants and 49% were returned(171(57%) OMF and 123 (41%) ENT. The antegrade technique was used routinely by 209 surgeons (87%), the retrograde technique by 9 (4%), and both techniques combined by 21 surgeons (9%). The antegrade technique was used by 135 surgeons (56%) for revision parotidectomy, by 193 (81%) for limited superficial parotidectomy,

Surgical approaches: postoperative care and complications "posterolateral-far lateral transcondylar approach to the ventral foramen magnum and upper cervical spinal canal".

BACKGROUND: Lesions that affect the lower clivus, foramen magnum, the craniocervical junction, and the upper cervical spinal canal that are anterolateral and at times intradural require access ventral to the cerebellum and spinal cord. The posterolateral transcondylar approach provides such a route. In addition, posterior craniocervical stabilization can be accomplished at the same time. The author has reviewed the technique as well as the surgical results here. MATERIALS AND METHODS: The posterolateral transcondylar approach to the craniocervical junction was utilized in children with schwan

The endonasal microscopic approach for pituitary adenomas and other parasellar tumors: a 10-year experience.

THE DIRECT ENDONASAL transsphenoidal approach to the sella with the operating microscope was initially described more than 20 years ago. Herein, we describe the technique, its evolution, and lessons learned over a 10-year period for treating pituitary adenomas and other parasellar pathology. From July 1998 to January 2008, 812 patients underwent a total of 881 operations for a pituitary adenoma (n = 605), Rathke's cleft cyst (n = 59), craniopharyngioma (n = 26), parasellar meningioma (n = 23), chordoma (n = 18), or other pathological condition (n = 81). Of these, 118 operations (13%) included

Trigeminal trophic syndrome with extensive ulceration following herpes zoster.

The trigeminal trophic syndrome is a rare complication following central or peripheral injury of the trigeminal nerve typically characterized by unilateral distribution of anaesthesia, paraesthesia and ulceration. In one third of cases it is preceded by an iatrogenic damage of the trigeminal nerve, in another third by a history of stroke. Other causes include head trauma, intracranial tumours, herpes virus infection, degenerative diseases of the CNS and idiopathic. Little is known about the pathogenesis. Contribution of neurotrophic factors and an altered sympathetic activity is assumed but a
