The relationship of the parotid duct to the buccal and zygomatic branches of the facial nerve; an anatomical study with parameters of clinical interest.

There have been studies concerning the protection of the facial nerve during plastic surgery intended for the parotid gland. The close relationship between the parotid duct and the buccal and zygomatic branches of the facial nerve is studied here. The dissections were performed on 10 fixed cadavers at the Anatomy Dissection Laboratory of Ankara University in 2004. The reference points used for surgery of this region were taken into consideration as the landmarks for morphometric measurements. In 7 of the cases the zygomatic branch was double and in 3 it was single. In 4 of the 7 cases with a d

Eye dryness sensations after refractive surgery: impaired tear secretion or "phantom" cornea?

The cornea is richly innervated by various functional types of sensory nerve fibers. When stimulated, these fibers evoke conscious sensations of different quality including ocular dryness, discomfort, and pain. Refractive surgery involves a variable degree of damage to corneal nerves. This leads to an altered expression of membrane ion channels at the injured and regenerating nerve fibers, giving rise to aberrant spontaneous and stimulus-evoked nerve impulse firing. It is speculated that these abnormal sensory discharges are read by the brain as ocular surface dryness. This would explain the h

Urinary bladder irritation alters efficacy of vagal stimulation on rostral medullary neurons in chronic T8 spinalized rats.

The presence of pelvic visceral inputs to neurons in the rostral medulla that are responsive to electrical stimulation of the abdominal branches of the vagus nerve (VAG-abd) was investigated in a complete chronic T8 spinal transection rat model. Using extracellular electrophysiological recordings from single medullary reticular formation (MRF) neurons, 371 neurons in 15 rats responsive to pinching the ear (search stimulus) were tested for somato-visceral and viscero-visceral convergent responses to stimulation of the following nerves/territories: VAG-abd, dorsal nerve of the penis, pelvic nerv

Dental pulp necrosis as a rare complication of reduction malarplasty.

Reduction malarplasty through the intraoral or the combined approach using intraoral and preauricular incisions is widely performed as an aesthetic surgical procedure in northeast Asia. The authors recently examined a patient who underwent this type of surgery and presented with discoloration of both upper canine teeth that was diagnosed as pulp necrosis. This case suggests the possibility that pulp necrosis occurs as a result of vibration and heat energy transmitted from a rotating bur to the anterior superior alveolar nerve branch located in the more lateral and superficial portion than in t

Lingual nerve injury II. Observations on sensory recovery after micro-neurosurgical reconstruction.

The aim of this study was to report on neurosensory recovery after micro-surgical lingual nerve repair, and to evaluate the effect on recovery of age, delay in repair and gender of the patient. Seventy-four patients entered the study. The micro-surgical repair performed was direct nerve suture (n=71), external neurolysis (n=2) and excision of neuroma without nerve coaptation (n=1). A standardized neurosensory examination was employed in all patients before surgery and during follow up. Recovery was significant for perception of all tested stimuli: feather light touch, pin prick, pointed/dull d

Intraoral approach for the treatment of submandibular salivary gland mixed tumors.

Pleomorphic adenoma represents a most common benign neoplasm of major salivary glands. Most occurrences of this benign tumor in the submandibular gland have been treated surgically without difficulty via transcervical approach. However, a few clinical problems in the transcervical approach have been mentioned, such as nerve injury or aesthetic scaring. In this study, we introduced the intraoral approach for the removal of the submandibular mixed tumor. In total, 12 cases of pleomorphic adenoma of the submandibular gland were treated via intraoral approach. The surgical technique and morbidity

Evolving concepts of laryngeal paralysis.

Accepted concepts of the pathophysiology and treatment of laryngeal paralysis have changed over the years. It has long been observed that symptoms of laryngeal paralysis vary greatly, both between patients and over time. There have been various theories to explain these differences. This article reviews how these ideas have changed over time as research has produced new information. Currently, the most popular view is that the laryngeal nerve regenerates after injury, albeit incompletely and inconsistently, and that variations in symptoms and laryngeal posture can be accounted for by muscle ac

Pterional keyhole approach to middle cerebral artery aneurysms through an outer canthal skin incision.

OBJECT: The supraorbital keyhole approach via an eyebrow skin incision provides a method for the minimally invasive clipping of aneurysms located in the circle of Willis, but has disadvantages for aneurysms located in the lateral Sylvian fissure. The pterional keyhole minicraniotomy via an outer canthal skin incision is proposed for the clipping of unruptured aneurysms of the middle cerebral artery (MCA). METHODS: The procedure consists of a 35-mm outer canthal skin incision, partial temporal muscle dissection restricted in the pterion, a 20-25-mm keyhole minicraniotomy, and a 15-20-mm dural

Altered control of the trapezius muscle in subjects with non-traumatic shoulder instability.

OBJECTIVE: Reflexes of shoulder girdle muscles such as trapezius are evoked from muscle afferents supplying the forearm and hand in healthy subjects. These reflexes are thought to aid the stability of the shoulder during use of the arm and hand [Alexander CM, Harrison PJ. Reflex connections from forearm and hand afferents to shoulder girdle muscles in humans. Exp Brain Res 2003;148: 277-282.]. With this in mind, the objective of this investigation was to examine this trapezius reflex in subjects with non-traumatic shoulder instability (NTSI). METHODS: The occurrence and alteration to this sup

Influence of different approaches to the cerebellopontine angle on the function of the intermediate nerve.

OBJECT: Surgery in the cerebellopontine angle (CPA) is a standard procedure at many centers. Outcome is focused mainly on preservation of hearing and facial function postoperatively. The nonmotor part of the facial nerve, the intermediate nerve, is nearly neglected nowadays. METHODS: A retrospective study was designed, including a questionnaire that was sent to 178 patients who had undergone surgery between 2000 and 2004. Data were obtained in 156 cases. The questionnaire was divided into five parts assessing the presence of increased tearing, reduced tearing, salivation disturbances, increas
