Delayed facial palsy in a patient with a bifid facial nerve lending support for viral theory of facial palsy.

OBJECTIVE: To describe the level of neurologic impairment in a case of delayed facial palsy occurring after cochlear implantation surgery. PATIENT: A 58-year-old man undergoing cochlear implantation who was found intraoperatively to have congenital bifurcation of the facial nerve just distal to the second genu. INTERVENTION: Cochlear implantation was performed through a facial recess approach. RESULTS: The lateral branch of the nerve impinged on the posterior tympanotomy slot and was uncovered during the procedure, rendering it vulnerable to direct thermal or mechanical injury or to the eff

Management of penetrating neck injuries: a new paradigm for civilian trauma.

PURPOSE: Improvements in imaging technology, particularly computed tomographic angiography (CTA), have altered the management of patients with penetrating neck injuries. Although some centers still advocate routine exploration for all zone 2 neck injuries penetrating the platysma, many civilian centers in the United States have adopted a policy of selective exploration based on clinical and radiographic examination. The purpose of this retrospective study is to evaluate our 5-year experience with the management of penetrating neck injuries, to further elucidate the role of CTA in clinical deci

Timing of trigeminal nerve microsurgery: a review of the literature.

Injury to peripheral branches of the trigeminal nerve is a known sequelae of oral and maxillofacial surgery procedures. The most often studied and reported branches have been the inferior alveolar and lingual nerves. Many questions still remain unanswered concerning the appropriate timing of surgical repair. The literature frequently mentions specific timing guidelines, however, there is scant scientific evidence to support these guidelines. In fact, several authors of clinical series state that although many of their surgical procedures occurred fairly late due to the timing of referral and o

Beneficial auditory and cognitive effects of auditory brainstem implantation in children.

CONCLUSION: This preliminary study demonstrates the development of hearing ability and shows that there is a significant improvement in some cognitive parameters related to selective visual/spatial attention and to fluid or multisensory reasoning, in children fitted with auditory brainstem implantation (ABI). The improvement in cognitive paramenters is due to several factors, among which there is certainly, as demonstrated in the literature on a cochlear implants (CIs), the activation of the auditory sensory canal, which was previously absent. The findings of the present study indicate that ch

Immunodeficiency impairs re-injury induced reversal of neuronal atrophy: relation to T cell subsets and microglia.

Following facial nerve resection in the mouse, a substantial number of neurons reside in an atrophied state (characterized by cell shrinkage and decreased ability to uptake Nissl stain), which can be reversed by re-injury. The mechanisms mediating the reversal of neuronal atrophy remain unclear. Although T cells have been shown to prevent neuronal loss following peripheral nerve injury, it was unknown whether T cells play a role in mediating the reversal of axotomy-induced neuronal atrophy. Thus, we used a facial nerve re-injury model to test the hypothesis that the reversal of neuronal atroph

Abducens nerve ocular neuromyotonia following non-sellar or parasellar tumors.

Ocular neuromyotonia is an uncommon disorder resulting from episodic involuntary discharge of ocular motor nerves producing sustained contraction of their respective ocular muscles. Ocular neuromyotonia manifests in brief spells of diplopia occurring spontaneously or after eccentric gaze holding. In most cases, ocular neuromyotonia follows months or years after radiotherapy to the sellar and parasellar region and involves the oculomotor nerve. We report two unusual cases of abducens nerve ocular neuromyotonia that followed radiation therapy of tumors in areas other than the sellar or parasella

The spinal accessory nerve in head and neck surgery.

PURPOSE OF REVIEW: To describe landmarks and tips used for minimizing surgical traumas to the spinal accessory nerve, and different options in case of its injury. RECENT FINDINGS: Modified radical and selective neck dissections reduce the prevalence of shoulder syndrome, a sequela of radical neck dissection. Impaired shoulder mobility and pain may be present even after nerve-sparing procedures, as shown using electromyography, particularly when dissection is extended to level V. In these cases physical therapy is mandatory to prevent shoulder pain and functional limitations. The issue of spin

Impairment of the brachial plexus after harvest of the latissimus dorsi muscle for reanimation of a paralysed face.

The one-stage neurovascular pedicled latissimus dorsi muscle transfer with a long thoracodorsal nerve for dynamic reanimation of long-standing facial paralysis offers several advantages over other options. However, extensive dissection of the thoracodorsal nerve risks injury to the brachial plexus. We reviewed 264 consecutive cases to assess the extent of the risk of impairment of the brachial plexus and to elicit factors that could possibly induce complications. Six patients had impairment of brachial plexus, but both sensory and motor disturbance disappeared about 11 months postoperatively.

Iatrogenic injury to the accessory nerve.

We describe two patients with uncommon causes of iatrogenic injuries and review the anatomy, presentation, possibilities of repair, and results. The incidence of such nerve injuries during lymph node biopsies is 3%-10%, but the diagnosis is often delayed. Symptoms are shoulder pain and inability to abduct the arm beyond the horizontal plane. Surgical repair may improve function and pain and should be performed early, preferably within six months, but prevention of nerve injury is most important.

Accessory nerve: anatomy and surgical identification.

The XIth cranial nerve or accessory nerve provides the motor supply to the sternocleidomastoid and trapezius muscles. It is frequently encountered during neck surgery, and as such is at risk of iatrogenic injury, resulting in the 'shoulder syndrome'. Historically, the nerve was sacrificed on oncological grounds during radical neck dissection. However, the basis for sacrifice is unfounded in the majority of cases, and accessory nerve sparing selective neck dissection has equal oncological efficacy. The path of the nerve in the neck is very variable, and there is not a wholly reliable landmark f

Intraoperative monitoring of lower cranial nerves in skull base surgery: technical report and review of 123 monitored cases.

The fundamental goal of skull base surgery is tumor removal with preservation of neurological function. Injury to the lower cranial nerves (LCN; CN 9-12) profoundly affects a patient's quality of life. Although intraoperative cranial nerve monitoring (IOM) is widely practiced for other cranial nerves, literature addressing the LCN is scant. We examined the utility of IOM of the LCN in a large patient series. One hundred twelve patients underwent 123 skull base operations with IOM between January 1994 to December 1999. The vagus nerve (n=37), spinal accessory nerve (n=118), and the hypoglossal

Cranial base fracture and rhino cerebrospinmal fluid leakage. A case report.

Oto cerebrospinal fluid leakage occurs frequently in skull base fractures but it is not always recognized which may produce potentially serious consequences on the prognosis. The aim of this study is to present a case of an extended skull base fracture with bad defined symptoms. A male in coma was admitted to our hospital following a road accident. Imaging revealed a fracture that transversally crossed the squamous occipital bone and petrous portions of temporal bone on the right, the sphenoid bone, and the left zygomatic bone. Ten days later the patient regained consciousness presenting sympt

Treatment of vestibular schwannomas. Why, when and how?

Sporadic vestibular schwannoma (VS) causes unilateral hearing loss, tinnitus, vertigo and unsteadiness. In many cases, the tumour size may remain unchanged for many years following diagnosis, which is typically made by MRI. In the majority of cases the tumour is small, leaving the clinician and patient with the options of either serial scanning or active treatment by gamma knife radiosurgery (GKR) or microneurosurgery. Despite the vast number of published treatment reports, comparative studies are few, and evidence is no better than class III (May, 2006). The predominant clinical endpoints of

Intraoperative brainstem auditory evoked potential pattern and perioperative vasoactive treatment for hearing preservation in vestibular schwannoma surgery.

OBJECTIVE: In vestibular schwannoma surgery, four different intraoperative brainstem auditory evoked potential (BAEP) patterns (stable BAEP, abrupt loss, irreversible progressive loss, reversible loss) can be identified and correlated with postoperative hearing outcome. Patients with reversible loss significantly benefit from postoperative vasoactive treatment consisting of hydroxyethyl starch and nimodipine. The present study investigates the treatment effect in the remaining three BAEP patterns. METHODS: A retrospective analysis was performed in 92 patients operated on for vestibular schwan

The ansa cervicalis revisited.

Recurrent laryngeal nerve paralysis represents a major complication in oesophageal cancer surgery. Nerve-muscle transplantation to the paraglottic space after resection of the recurrent laryngeal nerve with the ansa cervicalis (AC) has recently become the procedure of choice. The aim of this study was to investigate the anatomical variations of AC in order to avoid iatrogenic injuries and facilitate surgical procedures. We examined 100 adult human formalin-fixed cadavers. The ansa cervicalis showed a great degree of variation regarding origin and distribution. The origin of the superior root o
