Delayed visual deterioration after pituitary surgery--a review introducing the concept of vascular compression of the optic pathways.

BACKGROUND: Delayed visual deterioration after pituitary surgery has been attributed to secondary empty sella syndrome and downward herniation of the optic nerves and chiasm, but the pathophysiological basis of this condition is still a matter of debate. REVIEW: According to the literature, prior radiation therapy, previous visual impairment and transcranial surgery constitute risk factors for delayed visual deterioration. Radiation-induced vascular changes and/or strangulation of the optic nerves or chiasm are thought to compromise local blood flow. Downward herniation of the optic pathways

Effects of transitory lingual nerve impairment on speech: an acoustic study of diphthong sounds.

PURPOSE: The effects of lingual nerve impairment on the phonetic quality of diphthongs were studied by analyzing changes in their main acoustic features when anesthetic was injected into the lingual nerve to partly block the normal neural feedback mechanisms in speech. PATIENTS AND METHODS: The speech material consisted of 8 diphthongs in word context produced by 7 male speakers. Every utterance was repeated 10 times using normal speech rate and intonation with and without the anesthesia (Ultracain D-Suprarenin, 0.8 mL; Aventis Pharma Deutscland GmpH, Frankfurt am Main, Germany). In addition,

The superior laryngeal nerve: function and dysfunction.

Despite long-standing clinical interest in SLN dysfunction, most aspects of this entity continue to require clarification. The replacement of the laryngeal mirror by flexible fiberoptic and rigid rod-lens laryngoscopy (including stroboscopy) and the resulting improvement in laryngeal visualization and documentation of examination has not resulted in a better definition of characteristic signs. Symptoms are often vague, and most are shared with other voice disorders. Under the circumstances, there is good reason to suppose that SLN dysfunction yields a clinical picture at least as heterogeneous

Sensory regeneration following intraoperatively verified trigeminal nerve injury.

OBJECTIVE: To follow recovery of sensory function mediated by both myelinated and unmyelinated axons in relation to the type of inferior alveolar nerve (IAN) injury. METHODS: The authors assessed the function of afferent Abeta-, Adelta-, and C-fibers of the IAN using neurophysiologic (mental nerve blink reflex, sensory nerve conduction [NCS] of the IAN) and quantitative sensory tests (QST; cold, warm, heat pain, and tactile modalities). The tests were done 2 weeks, 1, 3, 6, and 12 months postoperatively and compared to the preoperative baseline in 20 patients undergoing mandibular bilateral s

"Other" neurologic complications after cardiac surgery.

Compared to the neurologic morbidity of stroke and cognitive dysfunction, "other" neurologic complications involving injuries to the brachial plexus, phrenic nerve, cranial nerves, other peripheral nerves, as well as the visual pathways, have been disproportionately underrepresented in the cardiac surgery and anesthesiology literature. These injuries are often missed in the early postoperative period when attention is focused principally on recovery from the acute trespass of cardiac surgery and cardiopulmonary bypass. However, when these problems do become apparent, they can cause considerabl

Morphometric analysis of implant-related anatomy in Caucasian skulls.

BACKGROUND: Sequelae related to implant placement/advanced bone grafting procedures are a result of injury to surrounding anatomic structures. Damage may not necessarily lead to implant failure; however, it is the most common cause of legal action against the practitioner. This study aimed to describe morphological aspects and variations of the anatomy directly related to implant treatment. METHODS: Morphometric analyses were performed in 22 Caucasian skulls. Measurements of the mental foramen (MF) included height (MF-H), width (MF-W), and location in relation to other known anatomical landma

Treatment of vertebral artery dissecting aneurysms presenting with progressive myelopathy.

Two patients with vertebral artery dissecting aneurysm are presented in which the posterior inferior cerebellar artery (PICA) arose from the wall of the aneurysm. The patients presented with progressive myelopathy due to mass effect on the medulla. One patient was treated with proximal occlusion of the vertebral artery using Guglielmi detachable coils (GDCs). The other patient underwent complete excision of the aneurysm, with reimplantation of the PICA into the vertebral artery proximal to the dissecting aneurysm. We obtained good results with improvement of myelopathy in both patients, but th

Peripheral mechanisms for the initiation of pain following trigeminal nerve injury.

Injury to a branch of the trigeminal nerve may lead to the development of chronic pain in the affected area. The etiology of this condition is not clear, but there is strong evidence to suggest that spontaneous and mechanically induced neural discharge from the injury site plays a crucial role. In laboratory studies, we have characterized this discharge following injury to the inferior alveolar or lingual nerves and have shown a temporal association with the accumulation of neuropeptides in the damaged axons. Substance P, calcitonin gene-related peptide, and vasoactive intestinal polypeptide w

A case of Collet-Sicard syndrome associated with traumatic atlas fractures and congenital basilar invagination.

An 18 year old man with congenital basilar invagination developed multiple lower cranial nerve (CN) palsies including CN IX to XII after a traffic accident. Computed tomography of his skull base revealed a two part atlas Jefferson fracture. Normally, lower cranial nerves (CN IX-XII) pass through a space between the styloid process and the atlas transverse process. Atlas burst fractures rarely cause neurological deficits because of a greater transverse and sagittal diameter of the spinal canal at the atlas, and a tendency of the lateral masses to slide away from the cord after injury. However,

Intraoperative auditory monitoring in vestibular schwannoma surgery: new trends.

OBJECTIVE: To investigate the efficiency of a new method of brainstem auditory-evoked potential (BAEP) monitoring during complete vestibular schwannoma (VS) resection with attempted hearing preservation. MATERIAL AND METHODS: Dedicated software providing near real-time recording was developed using a rejection strategy of artifacts based on spectral analysis. A small sample number (maximum 200) is required and results are obtained within 10 s. Fourteen consecutive patients with hearing class A operated on for VS, in an attempt to preserve hearing, participated in the investigation. Postoperat

[Neurophysiologic intraoperative monitoring to preserve cranial nerve function in base of skull surgery].

BACKGROUND: Neurophysiologic intraoperative monitoring (NIM) has gone through a renaissance since the advent of computer technology. Currently, both motor and sensory cranial nerves, including the IInd and VIIIth cranial nerves, can be intraoperatively controlled by means of small and mobile systems. PATIENTS AND METHODS: In order to estimate the value of NIM using the new generation of computer systems, we analyzed the records of 379 patients who underwent skull base surgery since 1996. These comprised NIM of the IInd, VIIth, VIIIth cranial nerves in most cases and of the IXth, Xth, XIth, an

[Radiosurgery of vestibular schwannomas after microsurgery and combined radio-microsurgery].

Significant morbidity is expected after microsurgery for recurrent or growing residual vestibular schwannoma (VS). In order to avoid this procedure, radiosurgery appeared as an interesting alternative. The purpose of this study is to analyze the results of Gamma Knife radiosurgery in this indication. Between July 1992 and January 2002, 60 patients (including 12 NF2 patients) underwent radiosurgical treatment after one or more attempts of surgical resection (including 27 growing remnant VS and 19 recurrent VS) and could be followed with enough informations, out of 103 patients (10% of the VS po

[Subtemporal approach according to house versus Wigand in intrameatal CPA tumors type A].

BACKGROUND: The subtemporal approach is indicated in intrameatal CPA tumors type A in order to preserve hearing. The exploration of the inner auditory canal for tumor exposure varies. It reaches from a locally limited uncovering of the bony inner auditory canal to a complete removal of the surrounding bony bed with the circular skeletization (360 degrees ) according to Wigand of the 7 (th) and 8 (th) nerve. Concerning the preservation of the function of the cranial nerve as well as an avoidable hyperelevation of the temporal cerebral lobe with a possible consecutive organic brain syndrome, bot

Secondary end-to-end repair of extensive facial nerve defects: surgical technique and postoperative functional results.

BACKGROUND: Repair of the transected facial nerve is imperative for restoration of muscle function, including the ability to produce appropriate facial expressions. Injury might involve the main trunk and its several branches. Restoration of function presupposes meticulous repair of all injured nerve branches. METHODS: Here we report three cases of secondary tension-free end-to-end coaptation of a transected trunk and branches of the facial nerve by removal of the superficial part of the parotid gland. RESULTS: Facial tone and symmetry at rest and motion were achieved. In two patients, a sli

Auditory brainstem implant in posttraumatic cochlear nerve avulsion.

Patients aged over 12 years with neurofibromatosis type 2 are considered candidates for an auditory brainstem implant (ABI). This study extends the indication criteria of ABI to subjects with profound hearing loss due to damaged cochleas and/or cochlear nerves (CNs) following head injuries. In our department, over the period from April 1997 to November 2002, 32 patients, 23 adults and 9 children, were fitted with ABIs. Their ages ranged from 14 months to 70 years. These patients were suffering from a variety of tumor (13 subjects) and nontumor CN or cochlear diseases (19 subjects). Six patient
