Comparison of contrast-enhanced T1-weighted and 3D constructive interference in steady state images for predicting outcome after hearing-preservation surgery for vestibular schwannoma.

We compared contrast-enhanced T1-weighted and 3D constructive interference in steady state (CISS) sequences for demonstrating possible prognostic factors in hearing-preservation surgery for vestibular schwannoma. We studied 22 patients with vestibular schwannomas having hearing-preservation surgery. Postoperatively six (27%) had a facial palsy and eight (36%) had hearing loss. There was a significant correlation between the size of the tumour and facial palsy (r=-0.72). Both techniques adequately demonstrated all tumours. Involvement of the fundus of the internal auditory canal (IAC) and a sma

Neurorrhaphy of the recurrent laryngeal nerve.

BACKGROUND: It has been controversial to anastomose the severed recurrent laryngeal nerve after transection because when severed recurrent nerves are directly anastomosed, the vocal cords do not regain normal movements and are fixed in the median. The objective of this study was to learn whether direct neurorrhaphy is necessary if the recurrent laryngeal nerve is severed during thyroid or parathyroid surgery. STUDY DESIGN: From 1998 to 2001, 12 patients who had a complete recurrent laryngeal nerve injury during thyroid or parathyroid surgery were enrolled into this study. Eight had primary re

[Endoscopic optic nerve decompression in traumatic visual loss].

OBJECTIVE: To review our experience of treatment of optic nerve decompression in traumatic visual loss by using endoscopic technique. METHOD: Fourteen cases patients with traumatic visual loss were undergone endoscopic optic nerve decompression. All patients were followed up for 3-18 months. RESULT: Eight cases had improved visual acuity, and 6 cases did not. No complication had happened. CONCLUSION: Endoscopic optic nerve decompression is an appropriate technique for patients with traumatic visual loss. Even though high-dose steroid treatment has failed, optic nerve decompression may impro

[Exposure of distal internal carotid artery through mandibular vertical ramus osteotomy].

Exposure of the distal internal carotid artery at the level of the second cervical vertebra required manoeuvers such as division of digastric muscle or mandibular subluxation. These increase the exposure but may not provide adequate access and are associated with significant cranial nerves or temporal mandibular joint complications. Vertical Ramus Osteotomy (VRO) provided access of the internal carotid artery (ICA) up to the base of the skull, with low incidence of cranial nerve injury temporo-mandibular joint (TMJ) pain and no preincision preparation. We report two cases in which vertical div

[Taste disorders and recovery of the taste function after middle ear surgery].

BACKGROUND: The incidence and the scale of recovery of the taste function after middle ear surgery is usually determined by the use of anamnestic data and electrogustometry. METHODS: Taste tests (regional chemical taste test, electrogustometry and subjective evaluation) were performed preoperatively, 2 to 3 weeks and 6 to 9 months after middle ear surgery. These results were evaluated by taking the scale of chorda tympani nerve (CTN) manipulation into consideration. Three groups were chosen: group 1: no or small CTN manipulation, n = 54; group 2: strong CTN manipulation, n = 14; group 3: seve

[Cephalic tetanus and Bell's palsy in an elderly man].

Cephalic tetanus is a rare form of tetanus defined as trismus plus paralysis of one or more cranial nerves. The seventh cranial nerve is most commonly involved. The process can remain localized, or become generalized. We describe the case of an elderly Caucasian man who developed cephalic tetanus after head trauma with a facial wound. Bell's palsy was inaugural.

Possible mechanisms of influence of esophageal acid on airway hyperresponsiveness.

Airway hyperresponsiveness is among the defining phenomena in asthma. In this article, 3 mechanisms are reviewed to explain how gastroesophageal reflux (GER) may influence airway hyperresponsiveness. First, microaspiration may cause not only direct tissue injury, but may also trigger vagal reflexes. Second, acid infusion of the esophagus in a dog model and in humans has been shown to result in vagally mediated reflexes leading to bronchoconstriction. These reflexes have been studied using immunohistochemical techniques. Third, neuroinflammatory reflexes have been found to play a role in airway

Lingual nerve injury.

Lingual nerve injury is a common complication following dental and medical procedures. The clinical presentation of lingual nerve injury, its epidemiology, predisposing factors, and anatomy are explored in an attempt to identify those patients at risk for developing neuropathic pain. Nonsurgical and surgical therapies also are discussed.

Nerve damage and third molar removal.

The surgical removal of lower third molars endangers both the lingual and inferior alveolar nerves. Patients sustaining an injury to either of these nerves must be managed correctly, and this requires a diagnosis of the injury type and regular monitoring of the recovery of sensation. Surgical intervention for a damaged inferior alveolar nerve is not usually indicated but may be undertaken: if the nerve is completely divided and the severed ends are misaligned; if a bony fragment has compressed the mandibular canal; or if the patient suffers from persistent neuropathic pain. In contrast, after

Mandibular fracture as a complication of inferior alveolar nerve transposition and placement of endosseous implants: a case report.

Placement of endosseous implants and inferior alveolar nerve transposition is a treatment option for patients with an edentulous posterior mandible with inadequate bone height superior to the inferior alveolar canal. Complications associated with these procedures include infection, prolonged neurosensory disturbances, and/or pathologic fracture. This report presents the surgical management of a patient with a mandible fracture after inferior alveolar nerve transposition with concurrent placement of two endosseous implants.

Paediatric mononeuritis multiplex: a report of three cases and review of the literature.

Mononeuritis multiplex results from disease processes causing multifocal injury to the peripheral nerves, with or without involvement of one or more cranial nerves. Most reported cases of paediatric mononeuritis multiplex have been related to autoimmune disorders. We present clinical and neurophysiologic findings in three cases of multiple mononeuropathy occurring in adolescence, with onset at ages 16, 17 and 13 years. Two of these cases were related to systemic vasculitis, one possibly representing a paediatric case of non-systemic vasculitis. Sural nerve biopsy confirmed the diagnosis of vas

Earring lesions of the parotid tail.

BACKGROUND AND PURPOSE: Masses in the parotid tail can be a source of consternation to radiologists and clinicians; inaccurate localization may lead to significant iatrogenic complication. We sought to review the pertinent anatomic localizing features of the parotid tail, relevant facial nerve anatomy, and sources of clinical and radiologic confusion. To conclude, we review imaging features that are helpful in generating a diagnosis in this location. METHODS: We retrospectively reviewed the imaging and clinical features of 111 parotid tail masses in 103 patients (56 male, 45 female, two of un

[Near infrared stimulation of the equilibrium organ--first clinical experiences].

BACKGROUND: A new method for the stimulation of the organ of equilibrium by means of a broad-scale and monochromatic near infrared emission was developed. This method should be examined within the framework of a pilot study, evaluated and its clinical possible applications examined. PATIENTS AND METHODS: Healthy probands (n = 15), patients with a radical cave of the ear (n = 5), patients with a defect of the tympanic membrane (n = 5) and spontaneous nystagmus (n = 5) were examined. In healthy probands an irritation with broad-scale as well as monochromatic near infrared (NIR) was performed an

[Reconstructive surgery of facial nerve injuries].

The facial nerve is main motor nerve of the face and its injury leads to total ipsilateral paralysis. There are several surgical procedures in reconstruction of the facial nerve, and the most frequent one is hypoglosso-facial anastomosis. In this study were analysed a series of 69 patients operated on Institute of neurosurgery from 1981 to 2000 year. The most frequent cause of injury was the operation of cerebellopontine angle tumors, as well as the skull base fractures. Hypoglosso-facial anastomosis was done in 57 patients, in 5 cases we performed nerve grafting in the cerebellopontine angle,

Radiotherapy for non-functioning pituitary tumors--when and under what circumstances?

The role of pituitary radiotherapy (RT) in the management of clinically non-functioning pituitary tumors (NFTs) remains controversial. Observational studies suggest that RT is effective in preventing the regrowth of NFT remnants following initial surgical debulking. However, not all tumor remnants will regrow in the absence of pituitary RT. Furthermore there are concerns relating to potential complications of pituitary RT, particularly hypopituitarism and its associated excess mortality. In the absence of any clear consensus guidelines relating to the application of pituitary RT in this settin

[Use of instenon in complex treatment of patients with optic nerve atrophy caused by craniocerebral trauma].

Instenon, a combined nootropic agent, was used on the step-type basis within a complex treatment of optic nerve atrophy caused by craniocerebral trauma. Our study revealed that instenon, when used within a complex treatment of a partial optic nerve atrophy caused by craniocerebral trauma, improved the visual acuity in 85.7% of patients and it expanded the total visual field in 76.2% of patients, whereas the routine therapy ensured the related improvements in 58.9% and 61.5% of patients, respectively. Besides, the drug provided for a better retinal electric sensitivity and for a better optic ne

[Retinal hemorrhages as a case for shaking trauma. Case report].

An interesting shaking trauma case was reported. A detailed analysis of the pattern of injuries and their progress allowed to reconstruct the mechanism and time point of cerebral lesions in a 6 week old infant who survived the impact. The necessity of postmortem investigation of the fundus and cranial nerves in fatal cases of shaking trauma is stressed.
