Our experience with surgical dissection of the facial nerve in parotid gland tumours. (A preliminary report).

Conservative parotidectomy is a primary method in the surgical treatment of parotid gland tumours. The classic parotid gland surgery uses the styloid process as a landmark for dissection of the facial nerve. The anatomical variations of the styloid process, however, prevent it from being always used as a reference landmark in this operative approach. This requires finding reliable invariable anatomical landmarks to be applied in the facial nerve dissection. The author presents his experience from the surgical treatment of 37 patients (25 females and 12 males) with tumours of the parotid gland.

[Brainstem anesthesia with respiratory arrest after retrobulbar block--a case report with a review of literature].

BACKGROUND: This case shows the time course and typical clinical features of brain stem anaesthesia. It is presumed that it follows accidental injection of local anaesthetic into the subarachnoidal space via the optic nerve sheaths. HISTORY AND SIGNS: We report the case of a 70-year-old man, who became unconscious shortly after retrobulbar anaesthesia. The first symptoms developed after an interval of several minutes following the injection of the local anaesthetic. After the appearance of cranial nerves deficits the patient complained of difficulty breathing and became unconscious and apnoei

Acute and long-term complications of therapeutic radiation for skull base tumors.

Acute toxicity from radiation of skull base tumors typically resolves spontaneously within days to weeks after completion of therapy. Long-term complications have a latency of months to years after radiation and are usually irreversible. The need to keep the probability of chronic toxicity at an acceptable level has determined the upper limits of radiation doses for the treatment of tumors. The threat of chronic toxicity thus affects the ability to control skull base tumors with radiation. Use of conformal radiation techniques with image guidance, allowing minimization of normal tissue volume

Final report of a randomized trial on altered-fractionated radiotherapy in nasopharyngeal carcinoma prematurely terminated by significant increase in neurologic complications.

PURPOSE: The aim of the present study was to compare the survival, local control and complications of conventional/accelerated-hyperfractionated radiotherapy and conventional radiotherapy in nonmetastatic nasopharyngeal carcinoma (NPC). METHODS AND MATERIALS: From February 1993 to October 1995, 159 patients with newly diagnosed nonmetastatic (M0) NPC with N0 or 4 cm or less N1 disease (Ho's N-stage classification, 1978) were randomized to receive either conventional radiotherapy (Arm I, n = 82) or conventional/accelerated-hyperfractionated radiotherapy (Arm II, n = 77). Stratification was acc

Unusual occipital condyle fracture with multiple nerve palsies and Wallenberg syndrome.

A 52-year-old male presented with an extremely rare fracture of the occipital condyle involving the jugular foramen with marked medial rostrad displacement of the fragments. He had ipsilateral VII through XII nerves palsies and Wallenberg syndrome. Conservative treatment did not improve the cranial nerve palsies. A high-resolution CT-scan is essential to visualize these fractures.

Aberrant regeneration of the third nerve.

We describe three patients with aberrant regeneration of the third nerve secondary to traumatic brain injury. The full blown features of the syndrome include horizontal gaze-eyelid synkinesis, pseudo-Graefe sign, limitation of elevation and depression of the eye with retraction of the globe on attempted vertical movements, adduction of the involved eye on attempted elevation or depression, pseudo-Argyll Robertson pupil and absent vertical optokinetic response. The 'misdirection' incidence in our study is 15%.

Cranial nerve dysfunction following carotid endarterectomy.

BACKGROUND: Carotid endarterectomy (CEA) is the most common surgical procedure performed for the treatment of symptomatic carotid stenosis greater than 70%. Among the recognised complications, such as stroke and myocardial infarction, is injury to cranial nerves. METHODS: We report the incidence and follow-up of cranial nerve injury in 269 patients who underwent carotid endarterectomy between January 1994 and December 1997 at the Royal Liverpool University Hospital. RESULTS: Fifteen cranial nerve injuries were documented (5.6%). Seven patients (2.6%) had unilateral vocal cord paralysis, nine

Clinical evaluation of inferior alveolar nerve block by injection into the pterygomandibular space anterior to the mandibular foramen.

The conventional inferior alveolar nerve block (conventional technique) has potential risks of neural and vascular injuries. We studied a method of inferior alveolar nerve block by injecting a local anesthetic solution into the pterygomandibular space anterior to the mandibular foramen (anterior technique) with the purpose of avoiding such complications. The insertion angle of the anterior technique and the estimation of anesthesia in the anterior technique were examined. The predicted insertion angle measured on computed tomographic images was 60.1 +/- 7.1 degrees from the median, with the sy

Neuropathic complications of mandibular implant surgery: review and case presentations.

Injuries to trigeminal nerves during endosseous implant placement in the posterior mandible appear to occur acutely in approximately 5-15 of cases, with permanent neurosensory disorder resulting in approximately 8%. Nerve lateralization holds even higher risks from epineurial damage or ischaemic stretching. Neuropathy from implant compression and drill punctures can result in neuroma formation of all types, and in some cases precipitate centralized pain syndrome. Two patterns of clinical neuropathy are seen to result; hypoaesthesias with impaired sensory function, often seen with phantom pain,

[Recurrent laryngeal nerve and inferior thyroid artery. Anatomo-surgical considerations].

Among the diverse complications of thyroidectomy, it is mostly the cordial sequels from the recurrent laryngeal nerve damage. This is why anatomical and surgical considerations are made upon the vulnerable areas of this nerve and the circumstances in which it is damaged. The relationship between the recurrent laryngeal nerve and the inferior thyroid artery, location most frequently injured, was investigated on 55 adult formulated cadavers. It was found that on the right side in 54.5% the nerve passes behind the artery, in 38.1% it passes in front of it, and between its terminal branches in 7.2

Traumatic optic neuropathy. A case report.

A case of visual loss following cranio-maxillofacial trauma is reported. The patient had a sudden partial blindness associated with a fracture of the roof, medial and lateral orbital walls. Access to the orbit was achieved through a transethmoidal approach using the Howarth-Lynch medial incision and resecting the bone fragments which impinged on the optic nerve. The patient had total return of visual acuity, without surgical complications. The role of orbital and optic decompression in the management of patients with traumatic optic neuropathy is discussed. Its indications are controversial an

Airway complication after thyroid surgery: minimally invasive management of bilateral recurrent nerve injury.

OBJECTIVES: After bilateral vocal cord paralysis, the consequent paramedian position usually necessitates tracheostomy for at least 6 months, when the paralysis is potentially reversible. In the present study a reversible endoscopic vocal cord laterofixation procedure was used instead of tracheotomy. STUDY DESIGN: Prospective study of 15 consecutive patients aged 33 to 73 years who suffered bilateral recurrent laryngeal nerve paralysis after thyroid surgery. METHODS: The operation was performed endoscopically with a special endo-extralaryngeal needle carrier instrument. Two ends of a monofil

Vagus nerve stimulation: quality control in thyroid and parathyroid surgery.

This paper describes the use of the Neurosign 100 Nerve Monitor and vagus nerve stimulation in the identification and assessment of the integrity of the recurrent laryngeal nerve (RLN) during thyroid and parathyroid surgery. Vocal fold function was assessed pre- and post-operatively in all patients undergoing thyroid and parathyroid surgery. The nerve monitor, used in association with endotracheal electrodes, was used to confirm correct RLN identification and demonstrate its integrity at the completion of surgery. There were 21 unilateral and 19 bilateral neck explorations. In these 40 patient

Ultrastructural evidence of calcium involvement in experimental autoimmune gray matter disease.

Experimental studies have suggested that increased calcium and inappropriate calcium handling by motoneurons might have a significant role in motoneuron degeneration. To further define the involvement of calcium in motoneuron loss we used the oxalate-pyroantimonate technique for calcium fixation and monitored the ultrastructural distribution of calcium in spinal motoneurons in experimental autoimmune gray matter disease (EAGMD). In cervical and hypoglossal motoneurons from animals with relatively preserved upper extremity and bulbar function, increased calcium precipitates were present in the

[First continuous nerve monitoring in thyroid gland surgery].

UNLABELLED: A new "all in one" sensing device was developed for continuous transtracheal intraoperative monitoring and in situ detection of the recurrent laryngeal nerve (RLN) during thyroid surgery. PATIENTS AND METHODS: The new system is based on a double-balloon endotracheal tube with integrated atraumatic stimulating and tracing electrodes. The recurrent laryngeal nerve is stimulated transtracheally and compound action potentials are recorded from the laryngeal muscles. Fifty-five patients were introduced into a phase-one clinical trial. Thirty-five patients with primary thyroid operation

Recurrent laryngeal nerve monitoring during cricotracheal resection.

Cricotracheal resection has been advocated in the management of severe subglottic stenosis. One of the possible complications of this procedure is injury to the recurrent laryngeal nerve (RLN). We describe a new technique in which electrodes are placed directly through the thyroid cartilage to monitor the RLN intraoperatively. Nine cats' left vocal cords were monitored, and 3 cats had unilateral vocal cord injury postoperatively: 1 right cord and 2 left cords. Even though this technique was efficacious, our ability to monitor the RLNs was difficult secondary to difficulty interpreting the nerv
