OBJECTIVE: The internal branch of the superior laryngeal nerve (ibSLN) conveys impulses for the laryngeal cough reflex, which protects the laryngeal aditus and prevents the development of aspiration pneumonia. The purpose of this study was to determine the effect of bilateral anesthesia of the ibSLN on the cough reflex after inhalation of a nebulized chemoirritant solution of tartaric acid.
DESIGN: Prospective, clinical investigation.
SETTING: Outpatient.
PARTICIPANTS: Nine healthy volunteers.
INTERVENTIONS: Bilateral injections of 2% lidocaine solution without epinephrine into the paraglo

There is a need for an uncomplicated, consistent method to predict facial nerve function after acoustic neuroma surgery. A prospective study with a 2-year follow-up of 35 patients undergoing acoustic neuroma surgery was performed assessing how well intraoperative facial nerve monitor electrophysiological thresholds and facial function postsurgery can predict ultimate nerve function. Tumour size was a strong predictor of immediate (P-value < 0.0005) and long-term facial nerve function (P-value = 0.004). Immediate facial nerve function was strongly predicted by stimulus intensity (P-value = 0.00

We describe a patient with an arachnoid cyst, possibly of traumatic origin, at the hypoglossal canal producing atrophy of the tongue. An arachnoid cyst should be considered in the differential diagnosis of any patient with a localized cystic mass around the cranial nerves at the base of the skull.

In this retrospective study the functional results of microsurgical repair of iatrogenic recurrent nerve lesions (10 patients) faced during years 1983 through 1995 have been analysed. Different parameters were considered (thyroid disease, previous thyroid surgery, microsurgical reconstruction technique, time between surgical damage and microsurgery). Furthermore, a thorough review has been made on the medical literature on subjects such as surgical anatomy of the cervical region, microsurgical nerve repair techniques, and clinical results of microsurgery in different authors experiences.

We present two cases of mutism observed after resection of tumors of the cerebellum, in two children of the feminine sex, being in the first case of medulloblastoma and in the second of juvenile astrocytoma. In both patients there was pre-operative lesion of low cranial nerves. The pathophysiology of the mutism involves anatomical, vascular and emotional factors, being its essential characteristics discussed with base in revision of the literature.

INTRODUCTION: The outcome of cases of near-drowning and initially poor prognostic signs are usually discouraging because of the severity of the consequent encephalopathy in most survivors. However, good recovery has been described, in spite of bad prognostic factors initially. It is difficult to establish the predictors of poor outcome which would enable one to decide when to establish and maintain advanced cardio-pulmonary resuscitation measures (CPR), since each case of near-drowning is different.
CLINICAL CASE: A four year old boy survived near-drowning in cold water without sequelas but w

Neurological complications following the administration of a local anaesthetic can be alarming. By reading reports of such incidents, dentists who find themselves in similar situations will be able to reassure their patients and act accordingly. The authors have reviewed the literature for those neurological complications that have been reported and offer an explanation of their aetiology. Examples of complications covered are facial nerve palsy, transient amaurosis, post-injection paraesthesia, Horner's syndrome, transient paralysis of combined cranial nerves III, IV and VI, sudden unilateral

PURPOSE: To assess the prognosis for recovery of vision in patients with blindness due to head injury, and to analyse the predictive value of visual evoked potential (VEP).
METHODS: One hundred consecutive patients with unilateral/bilateral blindness as a result of minor head injury were studied with regard to their visual status, CT scan, MRI scan and serial VEPs. Steroids were given to those presenting within one month of injury, 5 patients among them received methyl prednisolone. Transethmoidal decompression was done in 6 patients.
RESULTS: Visual improvement was recorded in 23 patients.

Neurofibromatosis type 1 is a common autosomal dominant genetic disorder associated with numerous physical anomalies and an increased incidence of neuropsychological impairment. Tumors of the CNS occur in approximately 15% of children with neurofibromatosis, presenting additional risk for cognitive impairment. This study examines the impact of an additional diagnosis of brain tumor on the cognitive profile of children with neurofibromatosis. A comprehensive battery of neuropsychological tests was administered to 149 children with neurofibromatosis. Thirty-six of these children had a codiagnosi

OBJECTIVE: In order to investigate the opportunity of repair and prognosis of recurrent laryngeal nerve injuries after thyroidectomy.
METHODS: Twelve cases with recurrent laryngeal nerve injuries after thyroidectomy were immediately and delayed operated on nerve repair and reinnervation. In immediate operation, 5 cases were repaired by direct recurrent laryngeal nerve suture, and 1 case was treated by transposition of the phrenic nerve to the recurrent laryngeal nerve and sutured the adductor branch to the branch of ansa cervicalis. In delayed operation, 3 cases were treated by anastomosis th

Trigeminal neuralgia is an example of an extreme form of neuropathic pain and continues to be a real therapeutic challenge. Although the pathophysiology of the disorder is uncertain, vascular compression of the trigeminal root resulting in damage to primary afferent neurons is thought to play a major role in the generation of pain. In the present study, we have used a recently developed rat model of trigeminal neuropathic pain, where the neuropathy is produced by a chronic constriction injury of the infraorbital branch of the trigeminal nerve (CCI-ION), and for the first time studied the effec

We report the case of a traumatic Foix-Chavany-Marie syndrome (SFMC) which is the cortico-subcortical type of suprabulbar palsy. A 10-year-old boy was brain injured in a traffic accident in August 1996. He was found comatous (initial GCS = 6) without any focal neurological deficit. The hemodynamic situation was stable even though he presented two wounds of the scalp and a hemoperitoneum that required intensive perfusions. The initial CT scan elicited a frontal fracture, ischemo hemorrhagic lesions of the right frontopolar and anterior temporal cortex. On the second day, he developed on the lef

OBJECTIVE: To examine the relation of the facial nerve to the only identifiable surgical landmark in the external auditory canal.
INSTITUTION: Community-based teaching hospital.
STUDY DESIGN: Examination of formalin-fixed human temporal bones.
BACKGROUND: The transcanal approach is often used in tympanoplasty, canaloplasty, hypotympanotomy, and removal of tumors of the external auditory canal (EAC), such as exostoses and osteomas. Surgery of the EAC places the facial nerve at risk for injury as the nerve courses vertically in the posterior canal wall. Few articles have described the relatio

The treatment of vocal fold paralysis by type I thyroplasty in the pediatric age group has not been reported. From 1990 to 1998, 12 type I thyroplasty procedures were performed on 8 patients between 2 and 17 years of age. The most common cause of vocal fold paralysis was neurologic, followed by vagal injury from a cardiac procedure. The most common indications for the procedure were aspiration and dysphonia. In our early thyroplasty experience, adult techniques and measurements adapted after Isshiki or Netterville were used. Postoperative laryngoscopy showed that in most cases, the placement o

This study analysed the use of the commercially available Neurosign 100(R) Nerve Monitor during thyroidectomy. Consecutive patients undergoing thyroidectomy were monitored. The nerve response prior to and after thyroidectomy were compared as were the relative benefits of mono and bipolar electrodes. Twenty-one consecutive patients over 9 months were assessed. The threshold for stimulation of the recurrent laryngeal nerve was never more than 0.5 mA (range 0.2-0.5 mA) for the bipolar and 1.5 mA (range 0.2-1.5 mA) for the unipolar electrode. The threshold for the superior laryngeal nerve was 1 mA

The most common complications of cochlear implantation are flap necrosis, compressed or improperly placed electrodes, and facial nerve injury, paresis, or stimulation. We report on three recent complications experienced at our institution: 1) a flap ischemia and partial-thickness necrosis without infection or device exposure, 2) facial nerve stimulation, which was managed with a new programming technique, and 3) a postimplant cholesteatoma in a patient with chronic eustachian tube dysfunction that required reoperation and explanation. We discuss the pathophysiology, likely etiology, and recomm

BACKGROUND: Injury to the recurrent laryngeal nerve (RLN) is an unwelcome and not infrequent complication of operations on or near the upper thoracic or cervical esophagus. Because anatomic information useful to the surgeon is difficult to come by, the aim of this study was to reinvestigate and display the RLNs and superior laryngeal nerves in humans.
METHODS: Postmortem en bloc specimens were studied: 23 by macroscopic dissection and 11 by large field serial histology. The nerves and their branches and supply areas were photographed at each step of dissection from the lateral (the surgeon's
