Fracture of the occipital condyle is a rare injury that can be easily overlooked. Palsies of lower cranial nerves can be the only symptom of the fracture. We report a patient with isolated post-traumatic hypoglossal nerve palsy who developed hypoglossal nerve palsy within 2 months after a car accident, indicating that the acute trauma itself did not damage the hypoglossal nerve. Most likely the palsy is caused by pressure to the nerve prior to the entry or within the hypoglossal canal. Since, in the present case, the fracture was stable and the patient showed only moderate neurological deficit

Intraoperative facial nerve monitoring (IFNM) is a suitable technique for intraoperative facial nerve identification and dissection, especially in large vestibular schwannomas (VS) (acoustic neuroma). To evaluate its feasibility for estimating functional nerve outcome after VS resection 60 patients underwent surgery using IFNM. Out of this group the last 40 patients were included in a prospective study evaluating the prognostic value of various IFNM parameters (proximal and distal absolute EMG amplitude, stimulation threshold, and proximal-to-distal amplitude ratio) for prediction of initial p

Wegener's granulomatosis, characterized by necrotizing granulomas and vasculitis of the respiratory tract and kidney, frequently first presents with otologic symptoms. We report a case of primary otologic Wegener's granulomatosis in a patient who presented with symptoms of acute otomastoiditis and associated facial nerve palsy. The patient subsequently developed neuropathies of various cranial nerves. The patient underwent urgent mastoidectomy with facial nerve decompression. Nonspecific inflammatory disease of the mastoid mucosa delayed the correct diagnosis of Wegener's granulomatosis, which

Bilateral sagittal split osteotomy may be associated with postoperative sensory deficiency in the area innervated by the inferior alveolar nerve. The aim of this study was to assess the neurosensory response of the inferior alveolar nerve after such surgery. Fifty consecutive patients receiving mandibular setback or advancement were investigated. Four different neurosensory tests were used: light touch, pin prick, static two-point discrimination, and vibration thresholds. These tests were performed preoperatively, 2 days, as well as 3 months and 12 months postoperatively. The methodologic erro

The authors report 17 cases of a right non-recurrent inferior laryngeal n. (NRILN) observed during 15 years of practice of thyroid and parathyroid surgery. In their last two cases, the existence of an aberrant right subclavian a., constantly associated with NRILN, was confirmed by MRI angiography. On the basis of the literature and their own experience, the authors review the incidence of this double anomaly, its embryologic explanation and its anatomic and surgical importance. They stress the diagnostic factors and the therapeutic implications, very different in children and adults, of a part

OBJECTIVE: Cranial nerve deficit, either transient or permanent, is a common postoperative complication after cranial base surgery. Frequently, this occurs because intracranial nerves are directly involved in the cranial base lesion. However, any cranial nerve adjacent to the lesion can be damaged because of direct or indirect manipulation during surgery, leading to severe morbidity. We describe a new technique in which the adjacent intracranial nerves are protected from surgical trauma by coating the nerves with a biological two-component fibrin glue (Tissucol; Immuno A.G., Vienna, Austria).

In the period of 1 January 1990 to 31 December 1996 the thyroidectomy cases we performed were immediately followed by vocal cord evaluation using a flexible bronchoscope while the patient was still on the operating table. If an obvious cord paralysis was discovered, an exploration of the recurrent laryngeal nerve, to the level of the larynx, was performed. If the nerve was found to be intact, no further measures were taken. A severed nerve underwent suture repair. If an otolaryngologist diagnosed a vocal cord paralysis 1-5 days after surgery, a reoperation was recommended except in the cases w

Hypoglossal nerve palsy (HNP) may follow fracture of the base of the skull. Of the various aetiologies described, trauma is one of the least frequent. HNP can easily be missed at the time of injury and awareness of this rare complication may assist in early diagnosis. A case of HNP after a head injury associated with the loss of taste sensation on the paralysed side of the tongue and aerocoele is reported.

BACKGROUND: Ipsilateral mydriasis is known to accompany signs of cerebral ischemia in unilaterally compromised carotid blood flow. Mydriasis as the presenting sign of common carotid artery (CCA) dissection has not been reported thus far.
CASE DESCRIPTION: We report the case of a patient who presented with a mydriatic pupil after intraoperative injury of the ipsilateral CCA. Mydriasis preceded complete third-nerve palsy and symptoms of cerebral ischemia for 12 hours. Cerebral angiography revealed occlusion of the CCA with slow collateral flow to the internal carotid artery and fetal origin of

In a consecutive series of 1,143 first operations for benign nodular goiter with 1,928 "nerves at risk", the incidence of postoperative recurrent laryngeal nerve palsy (RLNP) was analyzed related to the extent of the operative procedure (node-excision up to lobectomy) and manipulation of the nerve (identification, mobilization, non-identification). The incidence of early RLNP increased with the extent of the operation (1.2% up to 4.8% in lobectomy) and with the extent of nerve manipulation (up to 3.1% in extensive mobilization). However, the rate of permanent RLNP was higher after non-identifi

A new "all in one" sensing device for tracing and continuous intraoperative monitoring of the recurrent laryngeal nerve during thyroid surgery is described. The system, based on a double ballooned endotracheal tube, is atraumatic, easy to use and sensitive even to imminent trauma to the nerves. The most striking feature of this instrument is that it operates outside the sterile operating field and truly monitors continuously throughout the operation.

We present a simple and reliable method for identification of the recurrent nerve by intraoperative stimulation. An electromyographic record from the vocal muscle is obtained using a bipolar needle electrode inserted through the cricothyroid membrane. The recurrent nerve could be easily identified in 176 cases out of 181 nerves at risk (97.2%). We have not seen any cases of recurrent nerve paralysis and discuss the advantages of intraoperative neuromonitoring as a routine procedure.

To study the main prognostic factors and significance of facial nerve decompression of facial paralysis in temporal bone fracture, the main relative prognostic factors of 64 patients with facial paralysis were analyzed. Experimental model of facial paralysis was made. Expansion rate of facial nerve in facial canal opening group and non-opening group was measured and observed under electron microscope. The main factors affecting the prognosis were facial nerve decompression and selection of surgery time. Expansion rate of facial nerve in opening group was significantly higher than that of non-o

Trauma to the cranial base can complicate craniofacial injuries and lead to significant neurological morbidity, related to brain and/or cranial nerve injury. The optimal management involves a multidisciplinary effort. This article provides the neurosurgeon's perspective in management of such trauma using a 5-year retrospective analysis of patients sustaining skull-base trauma. The salient features of anterior and middle skull-base (temporal bone) trauma are summarized, and the importance of frontal basilar trauma as well as brain injury is evident. With these injuries, all cranial nerves (exce
