Neurological episodes after minor head injury and trigeminovascular activation.

Children appear particularly susceptible to severe but reversible neurological symptoms and/or signs after minor head injury; these include headache, confusion, drowsiness, vomiting, hemiparesis, cortical blindness, or seizures. Significantly, these neurological episodes are not associated with any identifiable structural brain abnormality on neuro-imaging. We propose that the cause of this condition is a reactive hyperaemia, a 'benign hyperaemic encephalopathy' mediated via activation of the trigeminovascular system.

[Rehabilitation of the eye in patients with facial paralyses: indications and results of gold weight implantation].

BACKGROUND: Upper-lid gold weight insertions in patients with long-standing facial palsy allow closure of the eye through an increased gravitational pull on the relaxed eyelid and complete opening without restriction of the peripheral visual field. MATERIALS AND METHODS: Between 1990 and 1995, 45 patients underwent eyelid rehabilitation for facial nerve paralysis at the New York Eye & Ear Infirmary. Their charts and outcomes were reviewed retrospectively. Twelve patients were followed prospectively at the University Hospital Zurich since December 1995. RESULTS: One gold weight extruded twice

Recurrent laryngeal nerve dysfunction following carotid endarterectomy.

Recurrent laryngeal nerve dysfunction is a significant complication of carotid endarterectomy and vocal cord paralysis is a major source of morbidity. This study prospectively assessed patients undergoing carotid endarterectomy to determine the nature and frequency of vocal cord damage and attempt to identify avoidable factors. Fifty consecutive patients undergoing carotid endarterectomy for symptomatic disease were studied. A standardized surgical technique was used emphasizing identification of the vagus nerve and minimal disturbance of the surrounding tissues. All patients underwent pre-ope

Protection of the lingual nerve during operations on the mandibular third molar: a simple method.

The object of the study was to assess the incidence of lingual nerve sensory loss during removal of impacted mandibular third molar teeth, and the effect of retention of the lingual plate on the incidence. The subjects were 395 patients, of whom 362 completed the study. Removal of 504 impacted wisdom teeth with retention of the lingual plate was performed. Results showed that 381 (76%) of the teeth were partially erupted and the remaining 123 were unerupted. In 497 (99%) bone had to be removed, and of these 376 (76%) required division of the tooth before removal (75% of the entire series). The

Inferior alveolar nerve damage following removal of mandibular third molar teeth. A prospective study using panoramic radiography.

Permanent alteration of sensation in the lip after the removal of mandibular third molar teeth is an unusual but important complication. Studies have been performed to assess the risk of nerve damage but most of these have been retrospective and poorly controlled. This prospective trial predicted the outcome of altered sensation prior to surgery based on assessment of a panoramic radiograph and correlated this with the result postoperatively in the consecutive removal of 479 third molar teeth. Results indicated that 5.2 per cent had transient alteration in sensation but only one patient (0.2 p

Intracranial coexistence of neurinoma with epidermoid cyst or cholesterol granuloma. Report of 2 cases.

The authors present 2 cases of a rare association of intracranial tumors of different cell types: neurinoma with epidermoid cyst, and neurinoma with cholesterol granuloma. The presenting symptoms resulted from neurinomas arising from the V and VIII cranial nerves, respectively. The diagnoses were achieved using Magnetic Resonance Images (MRI). The association of these rare lesions is discussed using recent literature pertaining to the coexistence of multiple brain tumors.

Surgical exposure of the petrous internal carotid artery: practical application for skull base surgery.

When exposing the horizontal petrous carotid artery in preparation for intrapetrous carotid bypass, the surgeon has no definite landmarks to localize the perimeter of the cochlea. The results of this study provide a practical, consistent, and safe method to maximize carotid artery exposure while minimizing cochlear injury. We measured the carotid-cochlea distance (mean, 4.3 mm) and the carotid-cochlear angle (mean, 10.8 degrees) in 33 temporal bones in which the extended middle fossa approach had been performed. We correlated this distance to the width of a Sheehy weapon knife, which can be ea

[Electromyography study and follow-up of bilateral recurrent laryngeal nerve paralysis after thyroid gland operations].

Bilateral recurrent laryngeal nerve (RLN) palsies result in dyspnea and often require further surgery to secure an adequate airway for safe respiration. The present study was devised to determine a predictive value of laryngeal electromyography (LEMG) and if it has therapeutic consequences. In a retrospective study of 63 patients with bilateral recurrent laryngeal nerve palsies after thyroid gland surgery we compared the results of LEMG from the thyroarytenoid muscles with the spontaneous recovery rate of vocal cord mobility. A tracheostomy had to be performed in 18 patients because of unsafe

[The lumbar ligamentum flavum. Anatomic characteristics with reference to microdiscotomy].

One important step in lumbar discotomy is the safe penetration and exact partial removal of the Lig. flavum. This is a crucial prerequisite for the selective and gentle removal of prolapsed and sequestrated intervertebral disc tissue. Extensive lancing of the Lig. flavum should be avoided, to minimize intraoperative and postoperative complications, such as injuries to the nerves and the Dura mater, instabilities, bleeding and scarring. As there has up to now been insufficient information on the anatomy of the Lig. flavum especially regarding microdiscotomy, an analysis of the Ligg. flava L2/L3

Characterization of heat-hyperalgesia in an experimental trigeminal neuropathy in rats.

Secondary trigeminal neuralgia (STN) follows an injury to the trigeminal nerve or one of its branches. Although rare, this condition results in great suffering and it is notoriously difficult to treat. The experimental analysis of painful neuropathy due to damage to the innervation of the limbs (e.g., the sciatic nerve) has progressed rapidly in recent years, but very few reports have appeared concerning experimental neuropathy in the trigemenial region. We report here an experimental rat model of trigeminal neuropathic pain produced by a chronic constriction injury to the infraorbital nerve (

Chorda tympani trauma--how much does it affect taste?

The chorda tympani nerve runs just beneath the tympanic membrane, and is often traumatised or sacrificed during middle ear surgery. There is varied opinion as to whether surgeons should preserve this nerve, risking trauma to it in the process, or cleanly sacrifice it, which some authors claim produces less trauma and less troubling dysgeusia. We studied 31 patients retrospectively to document their taste change following middle ear surgery, and found that in many instances, taste change caused by the operation recovered in the course of time. However, when the chorda was sacrificed, there was

Alloxan diabetes abolishes the increased negativity of interstitial fluid pressure in rat trachea induced by vagal nerve stimulation.

Increased negativity of interstitial fluid pressure (Pif) occurs concomitantly with oedema formation in acute airway inflammation. This observation is principally important because the loose connective tissues become 'active' and provide the driving force for the rapid oedema formation via Pif. The present study reports Pif in acute airway inflammation in alloxan diabetic rats. The basis for the study was, firstly, that inflammation is important in the pathogenesis of asthma. Secondly, that clinically there is almost a mutual exclusion between diabetes and asthma and, lastly, that the inflamma

[A case of epidermoid tumor inside the Meckel's cave].

An epidermoid tumor inside the Meckel's cave is rare. The symptoms caused by this tumor include trigeminal neuralgia, facial hypesthesia and paresis of the 3rd, 4th and 6th nerves. A case of epidermoid tumor inside Meckel's cave was presented. A 54-year-old female who had complained of 3rd nerve palsy with right facial hypesthesia since 3 years before was referred to our clinic. Magnetic resonance imaging (MRI) showed the tumor at Meckel's cave. The tumor removal was performed using the orbito-zygomatic approach. To avoid injury of the internal carotid artery and nerves inside the cavernous si

Childhood optic chiasm gliomas: radiographic response following radiotherapy and long-term clinical outcome.

PURPOSE: In children with chiasmal gliomas, radiation therapy can arrest progressive visual and neurologic impairment. We examined the radiographic response and clinical outcomes after irradiation. METHODS AND MATERIALS: Forty-two children (median age at diagnosis, 6.6 years) with chiasmal gliomas were managed as follows: 11 asymptomatic patients with neurofibromatosis-1 (NF-1) were observed only; 2 patients, less than 3 years old, underwent surgery and chemotherapy to delay irradiation; and 29 patients with progressive disease received radiation with or without prior surgery or chemotherapy.
