Phonatory characteristics of patients undergoing thyroidectomy without laryngeal nerve injury.

Complications that arise after thyroid surgery may be associated with infection, hemorrhage, hormonal problems, and laryngeal nerve injury. Voice alteration after thyroidectomy is usually caused by recurrent or superior laryngeal nerve injury. This voice dysfunction may also be associated with laryngotracheal fixation with impairment of vertical movement or by temporary malfunction of the strap muscles after surgery. In this study, we evaluated the voice function phonetically before and after thyroidectomy in 54 patients, although function of the recurrent and superior laryngeal nerves was nor

Mechanisms of auditory impairment during acoustic neuroma surgery.

Hearing loss during removal of acoustic neuroma (AN) may be due to labyrinthine and/or neural and/or vascular damage. Surgical maneuvers relating to perioperative and postoperative hearing may give rise to mechanisms of auditory impairment. Recording action potentials from the intracranial portion of the cochlear nerve (CN) has proven particularly useful for identifying the mechanisms of iatrogenic auditory injury. In this paper intraoperative and postoperative auditory impairments are investigated in relation to surgical steps in a group of 47 subjects with AN (size ranging from 5 to 25 mm) u

Cranial nerve II-VII injuries in fatal closed head trauma.

PURPOSE: To study the distribution and mechanism of traumatic injuries to the nerves supplying the eye and muscles protecting the visual apparatus. METHODS: Brain autopsy was carried out in 12 consecutive patients who died within three days after closed head injury. A segment of the brainstem with the entire intracranial portion of nerves II-VII was dissected out in each case and fixed in formalin. The specimens were stripped of the leptomeninges and inspected thoroughly under magnification. RESULTS: Injuries to the nerves were seen in nine subjects. The oculomotor nerve was completely torn

Circadian blood pressure rhythm in patients with higher and lower spinal cord injury: simultaneous evaluation of autonomic nervous activity and physical activity.

OBJECTIVE: To examine the relationships among the circadian rhythms of blood pressure, autonomic nervous function, and physical activity of patients with varying levels of spinal cord injury. DESIGN AND METHODS: We studied 19 patients with spinal cord injury [10 tetraplegic patients with cervical cord injury (C4-C7), and nine paraplegic patients with thoracic cord injury (Th6-Th12)] compared with 16 control subjects. A new multibiomedical recorder was used to measure blood pressure (every 30 min), cardiac vagal activity (hourly frequency of R-R50), and physical activity (integrated accelerati

Continuous intraoperative electromyographic monitoring of cranial nerves during resection of fourth ventricular tumors in children.

The authors reviewed the results of continuous intraoperative electromyographic (EMG) monitoring of muscles innervated by cranial nerves in 17 children whose preoperative imaging studies showed compression or infiltration of the fourth ventricular floor by tumor to determine how intraoperative EMG activity correlated with postoperative cranial nerve morbidity. Bilateral lateral rectus (sixth) and facial (seventh) nerve musculatures were monitored in all children. Cranial nerve function was documented immediately postoperatively and at 1 year. Of the 68 nerves monitored, nine new neuropathies o

[Hypoglossal paralysis after septum correction with intubation anesthesia].

BACKGROUND: We report a case of irreversible unilateral hypoglossal nerve palsy in connection with nasal septum surgery in intubation anaesthesia. On account of the spatial distance, there is no danger of injuring the hypoglossal nerve during nasal surgery. However, in the otorhinolaryngological and anaesthesiological literature, some cases of hypoglossal palsy following endotracheal intubation, use of the laryngeal mask airway, bronchoscopy and laryngoscopy are reported. CASE REPORT: One day after uneventful nasal septoplasty in intubation anaesthesia in a 45-year-old male patient, an unilat

An immunohistochemical study of TNF-alpha in optic nerves from AIDS patients.

PURPOSE: Both in vitro and in vivo studies have implicated a role for tumor necrosis factor (TNF-alpha) in the pathology of demyelinating diseases. The purpose of this study was to address the hypothesis that TNF-alpha is a mediator of AIDS-related optic nerve injury and to determine the cell types involved in the proliferation of TNF-alpha in the AIDS optic nerve. METHODS: Ten optic nerves from seven patients with AIDS, and three from persons who were HIV negative were stained, using the indirect immunoperoxidase method. Six of the ten AIDS optic nerves were positive for cytomegalovirus (CMV

Chronic electrical stimulation of the gasserian ganglion for the relief of pain in a series of 34 patients.

The use of an implanted system for chronic electrical stimulation of the gasserian ganglion for relief of facial pain was described in 1980 by Meyerson and Håkansson. Between 1982 and 1995, the senior author (R.R.T.) performed gasserian ganglion stimulation in 34 patients for the relief of chronic medically intractable facial pain. The etiology of pain was peripheral damage to the trigeminal nerve in 22 patients (65%), central (stroke) damage in seven (21%), postherpetic neuralgia in four (12%), and unclassifiable cause in one (3%). All patients received a trial of transcutaneous stimulation (

Neurologic assessment and cardiac surgery.

Cerebral injury remains a significant complication of cardiac surgery. This complication is evaluated by clinical means that include a neurologic examination. In this report, the most important components of this type of evaluation are described. The neurologic complications of cardiac surgery can be determined by comparing structured preoperative and postoperative clinical evaluations. The neurologic examination must include a mental state examination, examination of cranial nerves, motor, sensory, and cerebellar systems, examination of gait and station, and deep tendon and primitive reflexes

Cerebro-vasculopathy and malignancy: catastrophic complications of radiotherapy for optic nerve glioma in a von Recklinghausen neurofibromatosis patient.

The authors report a unique case of a patient with intraorbital optic nerve glioma and von Recklinghausen neurofibromatosis who developed cerebro-vasculopathy and malignant transformation in the orbit 18 months after radiotherapy treatment. The case is an important reminder of the possibly increased susceptibility of von Recklinghausen neurofibromatosis patients to the complications of radiotherapy.

Meningiomas of the cerebellopontine angle.

Meningiomas of the cerebellopontine angle (CPA) represent a clinically and surgically interesting entity. The opportunity of complete surgical excision and the incidence of impairment of nerval structures largely depend on the tumour biology that either leads to displacement of surrounding structures by an expansive type of growth or to an enveloping of nerval and vascular structures by an en plaque type of growth. As the origin and the direction of growth are very variable, the exact tumour extension in relation to the nerval structures and the tumour origin can be identified sometimes only a

[Incidence and regeneration time of disorders of the inferior alveolar nerve in osteosynthesis of mandibular fractures--a prospective clinical study].

In a prospective study posttraumatic and postoperative sensitivity disturbances after mandible fractures in the region of osseous course of the inferioralveolar nerve were assessed. The study contains 28 patients with 31 mandible fractures. For assessment somato sensoric evoked potentials and computer assisted pain and thermal sensitivity testing was used. A rate of 32% posttraumatic and 52% nerve disturbances resulting from osteosynthesis was found. In 81% the sensitivity disturbances lasted only 6 months post-operatively. In 19%, mainly cases of fracture dislocation of 5 mm minimum, anesthes

[Presymptomatic injury of the recurrent laryngeal nerve in benign thyroid disease: contribution of electromyography].

Diagnosis of recurrent laryngeal nerve palsy is usually possible through a clinical, laryngoscopical and electromyographical approach, but at a critical stage of the nerve injury. We observed four cases of benign thyroïd tumoral processes with a preoperative electromyographic examination showing neurogenic abnormalities in the thyroarytenoid muscle without any clinical symptoms. We presume that only laryngeal electromyography permits the diagnosis of mild, even asymptomatic laryngeal recurrent nerve injury. A recurrent laryngeal nerve palsy occurs in thyroid tumors, most often in malignant con

Differential effect of ischemia/reperfusion on pigmented and albino rabbit retina.

The purpose of the study was to compare the retinal sensitivity of pigmented and albino rabbits to ischemia/reperfusion-induced electroretinogram (ERG) alterations and optic nerve morphological changes. High intraocular pressure (HIOP) was induced by applying a suction-cup on the eye and a depression with an ophthalmodynamometer. HIOP was maintained for lengths of time (30-75 min). Flash ERGs were recorded in dark-adapted animals for ischemia and 2 h reperfusion periods. Two weeks later, histological examination of the retinas and optic nerves was done. Albino rabbits submitted to 45 min HIOP

Severe functional laryngeal stenoses treated by endoscopic arytenoidectomy.

During the 15-year period from 1978 to 1993 we treated 17 cannulated patients suffering from dyspnea caused by bilateral recurrent laryngeal nerve palsies. The causes of palsy were struma surgery (13 cases), prolonged intubation (3 cases) and unknown (1 case). All cases were operated on by the classical endolaryngeal arytenoidectomy technique developed by Kleinsasser. All patients were able to be decanulated after arytenoidectomy: 14 cases within 1 month of surgery and 3 delayed. Four patients required replacements of their tracheotomies during the follow-up period, but this proved to be only

[Surgical treatment of benign recurrent goiter with pre-existing unilateral recurrent laryngeal nerve paralysis--a report of experiences].

Operations for recurrent goiter are considered to range among the most difficult procedures in thyroid surgery, because the risk of a permanent recurrent nerve palsy increases to 10 or 30%. In case of pre-existing unilateral lesion of the nerve the danger of bilateral paralysis of the vocal chord will become even larger. The results from 29 patients with an intracapsular resection (nearly total removement of the thyroid tissue without the preparation of the recurrent nerve) are presented and compared with those found in 4 patients with an extracapsular approach. All four patients, where the op

F-wave recordings from nasal muscle for intraoperative monitoring of facial nerve function.

The generation of the F-wave in muscle after peripheral motor nerve stimulation depends upon the functional integrity of the entire motoaxon between its axon hillock and the motor endplates. Cerebellopontine angle (CPA) surgery bears the risk of damaging the facial nerve in its root exit zone; functional degradation should therefore be reflected by changes of the nasal muscle F-wave. Constant current pulses supraliminal for direct (M) muscular responses were applied subcutaneously to the zygomatic branch of the facial nerve ipsilateral to the operated side, EEG needle electrodes were placed in
