A 35-year-old man developed a pupil sparing palsy of the oculomotor nerve after a standard Le Fort I osteotomy during which there had been more bleeding than usual. No definite cause was found but we suggest that it was caused by ischaemia of the nerve secondary to local injury by haematoma or instrumentation. The anatomical basis for this is discussed.

This article describes the treatment of oral complications that arose after surgery for the removal of an acoustic brain tumor. The authors contend that a thorough oral examination can pinpoint the cranial nerves that have been affected by the tumor or the surgery, and that dental appliances can offer special protection after surgery.

451 arthroscopies of the temporomandibular joint (TMJ) have been performed on 373 patients during 10 years of clinical experience. The complication rate was low (1.77%). Most complications were transient, consisting mainly of temporary deficits of the 5th and 7th cranial nerves. No patient required arthrotomy because of complications, but one patient had to undergo angiography and embolization of a traumatic aneurysm of the superficial temporal artery secondary to arthroscopy.

The authors give an account of the results of suture and decompression of the n. laryngeus recurrence in 15 patients with a mean follow up period of 17,6 months. A total of 12 nerves were sutured, one of them by means of an autograft from the n. suralis. A total of 11 nerves were decompressed. One patients after suture of the nerve and contralateral decompression did not attend a check-up examination. Of 11 sutured and checked nerves the results were very good in ten instances when a remarkable improvement of the voice was recorded as well as of the adduction movement and phonation closure of

A trauma victim with locked-in syndrome demonstrated severely decreased bowel sounds, intact response to suppository, and elevated, but unchanging pulse. Absent cardiac response to tracheal suctioning, high gastric residual volumes, and pulmonary edema in response to a urecholine challenge demonstrated dysfunction in the automatic system. Symptoms persisted for 2 1/2 years until death. At autopsy, asymmetric bilateral involvement of the dorsal motor nucleus of the vagus and of the nerve tract in the medulla were demonstrated. In contrast, a control subject with locked-in syndrome caused by a s

The authors present a case of 34-years-old man with retrobulbar hematoma caused by blunt injury accompanied by partial palsy of the oculomotor nerve and ischemia of the optic nerve and retina. Fractures of the orbit were excluded using computer tomography. Surgical decompression of the orbit was performed. The authors obtained quick and marked improvement with normal visual acuity and limitation of adduction.

We identified retinal ganglion cells undergoing apoptosis, a form of programmed cell death, in an eye of a 70-year-old man with anterior ischemic optic neuropathy. The TUNEL (terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate-biotin end nick labeling) staining and the presence of condensed, fragmented nuclear bodies were used to identify apoptotic cells. Examination of TUNEL-stained retinal sections revealed occasional cells in the ganglion cell layer with pyknotic nuclei and brown reaction product, representing positive staining for chromosomal DNA breaks. Positive cells

Injury of the recurrent laryngeal nerve (RLN) is one of the most frequent complications in thyroid surgery. It leads to a significant morbidity of up to 20%, depending on the type of surgery being performed. We present a new device for intraoperative monitoring of the RLN. The new monitoring system was evaluated in piglets. The system is reliable and easy to use. For the first time even imminent injury to the RLN can undoubtedly be demonstrated by signal changes. It will soon be available for use in humans.

Intraoperative facial nerve monitoring has emerged as a powerful tool for facilitation of surgery involving the facial nerve. This tool must be properly applied and maintained, however, in order to avoid untoward results and to maximize its potential benefits. Facial nerve monitoring is best suited for prevention of iatrogenic injury. Once injury has become established, its value may be severely limited, especially for moderate to severe injury, in which visual assessment of injury is still the most effective means to determine the need for repair. The most valuable derivative of facial nerve

We investigated the effect of increased middle ear pressure on blood flow to the inner ear, middle ear, and facial nerve in guinea pigs using a nonradioactive microsphere technique. The elevation of middle ear pressure significantly reduced blood flow to the middle ear. Blood flow to the facial nerve also decreased due to the elevation of the middle ear pressure but the reduction was not significant. Blood flow to the inner ear did not change even after the middle ear pressure was increased. Findings are discussed in relationship to aural barotrauma, alternobaric vertigo and facial baroparesis

Techniques to repair cerebrospinal fluid (CSF) leak through the eustachian tube (ET) include temporary or permanent CSF diversion; middle fossa craniotomy and packing of the ET from above; and packing of the ET from behind, through the middle ear. We report a case of endoscopic closure of the ET in the nasopharynx (the front). A 26-year-old woman underwent a translabyrinthine removal of a 4.5-cm vestibular schwannoma. One year later, she developed CSF rhinorrhea and meningitis. Attempts at control of this leak included traditional approaches mentioned previously (shunting, middle fossa, middle

Our 24-year experience with acoustic neuroma surgery is reported. Three approaches were used in 34 patients: translabyrinthine (11 cases), transtemporal (2 cases), and retrosigmoid (21 cases). Prolonged facial paralysis occurred in 27% of patients with the translabyrinthine approach and in 15% with the retrosigmoid approach. Facial nerve monitoring was used in all operations with the retrosigmoid approach. In the translabyrinthine approach, 5 patients had CSF leak, 2 required secondary surgery, and 2 developed meningitis. With the retrosigmoid approach, 5 patients had CSF leak, 2 required seco

The laryngeal nerves are at risk during thyroid surgery, and several techniques have been described for their intraoperative identification to minimize potential damage. Nerve protection is based on the electromyographic recording from the muscles innervated by the laryngeal nerves, and that electrical activity is picked up by various techniques. We evaluated an electrode attachment to the endotracheal tube that provides a stable method for continuous recording of the laryngeal electromyogram. In addition, we tested various modalities of electrical stimulation in the region where the nerves ar

OBJECTIVE: To describe the causes and characteristics of hypoglossal nerve palsy.
DESIGN: A review of 26 years of personal experience in a large public hospital.
RESULTS: Twelfth-nerve palsies usually appear as signs rather than symptoms. Tumors, predominantly malignant, produced nearly half of the palsies (49 cases), while gunshot wounds made trauma (12) the second most common cause. Stroke (6), hysteria (6), multiple sclerosis (6), surgery (5), Guillain-Barré neuropathy (4), and infection (4) together accounted for about one third of the patients.
CONCLUSION: Twelfth-nerve palsy proved to

Over a period extending from 1984 to 1993, 16 children ranging in ages from 2 months to 12 years with posterior fossa ependymoma were treated. Four of these tumors were classified as lateral ependymomas because of their configuration and suspected site of origin. These 4 patients' ages ranged from 2 months to 5 years. Signs and symptoms of increased intracranial pressure and cranial nerve dysfunction were the hallmark of their presentation. Their imaging evaluation included a preoperative and postoperative CT san and/or MRI. A gross total surgical resection' was completed in the 4 children. At

PURPOSE: The purpose of this paper is to describe the damage caused to the retina and the axons of the optic nerve by acute ischemia-reperfusion injury and the extent to which optic nerve damage correlates with the duration if ischemia due to high intraocular pressure (IOP).
METHODS: Acute ischemia in the retina and optic disc was induced in albino rats by increasing the IOP to 110 mmHg for a period of 45-120 min. Thereafter, the eyes were reperfused at normal IOP after 7 days. The retina and optic nerve were examined by light and electron microscopy, and morphometrical counts of the optic ne
