A 64-year-old woman with diabetes mellitus was admitted to our hospital with left hemiparesis of sudden onset. A brain MRI demonstrated a cerebral infarction in the ventral part of the right lower pons. When left hemiparesis worsened, she had dislocation of the temporo-mandibular joints repeatedly. Then, her lower jaw deviated to the right when she opened her mouth. Also, there was decreased contraction of the right masseter when she clenched her teeth. These findings suggest that there was trigeminal motor paralysis on the right side resulting from involvement of the intrapontine trigeminal m

PURPOSE: Ophthalmologists often are asked to evaluate the optic disc for evidence of acute increased intracranial pressure (ICP). The authors studied the incidence of papilledema in a population of patients with a documented acute increase in ICP.
METHODS: Included in this study were 37 patients with acute elevated ICP due to a spontaneous hemorrhage or craniocerebral trauma. In all patients, the ICP was monitored continuously. Fundus examination was performed twice daily on at least 7 consecutive days.
RESULTS: According to the level and duration of the ICP, the patients were divided into t

Carotid body paragangliomas are rare, and are generally benign tumours that grow slowly. Extirpation of carotid body paragangliomas is well documented as the best curative treatment, but can be accompanied by complications such as peroperative bleeding, stroke and injury to cranial nerves. Accurate preoperative diagnosis and evaluation are important, and preoperative carotide angiography is essential to confirm the diagnosis. In this case report, a patient with large carotid body paraganglioma underwent preoperative selective embolization of the major afferent arteries. The vascularity of the

Damage to the olfactory nerve during the pterional approach to the anterior communicating artery aneurysms has not previously been investigated in a quantified manner. In this retrospective study, 100 patients with anterior communicating artery aneurysms, for whom the pterional approach was employed, were observed from the point of view of postoperative olfactory nerve function. In the postoperative period only three cases suffered from the impaired sense of smell ipsilateral to the side of surgery. 15 patients objectively showed olfactory nerve distinctions. The functions of olfactory nerve c

We present a unique case in which a multichannel cochlear implant device was explanted and the same device was re-implanted into the contralateral ear. A patient with bilateral total deafness secondary to head injury received an implant in his left ear but developed severe facial nerve stimulation. Because this stimulation could not be eliminated effectively with change of programming, it was decided to implant the contralateral ear. Since the device itself was functioning well, it was explanted from the left ear and re-implanted successfully into the right ear. Facial nerve stimulation was mi

It was the purpose of this study to establish whether it is necessary to identify the recurrent laryngeal nerve intraoperatively when resecting the endemic nodular goiter. We prospectively formed two groups from 800 subtotal unilateral resections ("nerves at risk"). In group I (382 subtotal resections, 48.4%) the nerve was not identified intraoperatively. In group II (413 subtotal resections, 51.6%) the recurrent laryngeal nerve was routinely identified in all cases. The operative technique was standardized, giving special attention to the "anterior lamella". For all patients, preoperative and

OBJECTIVE AND IMPORTANCE: Percutaneous gangliolysis procedures may rarely be associated with vascular complications. There are three reported cases of carotid cavernous fistulas occurring after percutaneous microcompressive trigeminal gangliolysis. This is the only reported case of this complication associated with microcompression gangliolysis.
CLINICAL PRESENTATION: A 78-year-old woman was referred to our institution with a history of abrupt onset of left-sided bruit, proptosis, chemosis, and diplopia after a percutaneous retrogasserian microcompression.
INTERVENTION: Cerebral angiography

This study found that pretreatment with 4-(o-benzylphenoxy)-N- methylbuty-lamine hydrochloride (bifemelane hydrochloride, Celeport) reduced ischemia-reperfusion injury in rat eyes. Bifemelane (25 mg/kg) was injected intraperitoneally 30 minutes before an ischemic insult, then acute ischemia of the retina and optic disc was induced by increasing intraocular pressure to 110 mmHg for 45 minutes. After one week, the axonal count of the optic nerve was investigated using electron microscopy. The control group consisted of vehicle-treated eyes which received normal saline. The axon count was 93.4 +/

This report describes a patient with a headache and a bitemporal hemianopsia immediately following endoscopic ethmoid and sphenoid sinus surgery. Computed tomographic imaging revealed tension pneumocephalus caused by surgical interruption of the boundaries between the sinus(es) and the anterior cranial cavity. The delineation of the central chiasmal injury required follow-up magnetic resonance imaging.

With the increasing use of pedicle screws in instrumented spine surgery the neurological risk must be evaluated critically. Studies, which evaluated the accuracy of pedicle screw placement in scoliosis surgery, have not been published up to date to our knowledge. In 25 consecutive patients with idiopathic scoliosis, who underwent posterior instrumented curve correction and stabilization, the accuracy of pedicle screw placement was evaluated using axial computed tomography. There was a total of 178 screws between T5 and L4. The preoperative Cobb angle of the curve averaged 60.7 degrees, the mea

Tumors of the carotid body are relatively rare and may pose a difficult surgical problem because of their vascularity and compression of cranial nerves in the neck. This article reviews the physiology of the carotid body, its surgical history, and retrospectively reviews the management and outcome of 17 carotid body tumors occurring in 14 patients over an 18-year period at the University of Louisville Hospitals. The average age at presentation was 54.4 years. Three patients had bilateral tumors. Two patients (12%) had postoperative cranial nerve paralysis lasting greater than 6 months. One pat

PURPOSE: Sphenoidal electrode (SE) insertion can cause pain, for which local anesthesia with lidocaine or intravenous administration of fentanyl has been advocated by different epilepsy treatment centers. Transient facial palsies have been observed after SE insertion. Their frequency of occurrence, distribution, and duration have not been well characterized, however. We hypothesized that this complication is due to the effect of local anesthesia on the peripheral branches of the seventh cranial nerve. To test this hypothesis, we compared the incidence and characteristics of facial palsy during

BACKGROUND: Optic disc pits represent a rare congenital abnormality frequently associated with macular detachment. Several theories on the pathogenesis of the detachment have been proposed, but the issue still remains unclear. Treatment remains speculative as well; advocated treatments include eye-patching, bed rest, steroids, laser photocoagulation, pars plana vitrectomy with fluid-air exchange and optic nerve sheath decompression.
METHODS: The Authors report on the case of an optic pit with associated macular detachment which developed an inner layer macular hole and, after an apparently in

The anterior cervical approach is commonly used for access to the cervical spine. Vocal fold paralysis (VFP), a complication of this approach, is underrepresented in the literature. A review of the database of the Vanderbilt Voice Center revealed 289 patients with VFP, including 16 patients who developed paralysis as a result of an anterior cervical approach. The paralysis was on the right side in all but 1 patient. Compared to patients who developed VFP after thyroidectomy and carotid endarterectomy, patients with VFP after an anterior cervical approach have a higher incidence of aspiration a

The middle cranial fossa approach to lesions of the geniculate ganglion and internal auditory canal preserves cochlear function and affords access to the lateral internal auditory canal. The labyrinthine portion of the facial nerve tends to course near the basal turn of the cochlea, just beneath the middle cranial fossa floor, and is usually dissected in this approach. To determine the distance from the labyrinthine portion of the facial nerve to the basal turn of the cochlea, measurements were obtained in the temporal bones of 24 subjects (48 ears) 9 to 76 years of age. These subjects had no

Due to an increase in the numbers of function-preserving laryngectomies and the utilization of laser techniques in cancer surgery, more patients after supraglottic partial laryngectomies are experiencing dysphagia of different intensities. We therefore developed a functional therapy program that we use for patients after supraglottic partial laryngectomies following a special diagnostic evaluation including video-laryngoscopy and, if necessary, high-speed cine-radiography. This therapeutic treatment was developed from conventional treatment for patients with dysphagia caused by neurological di

Transcranial magnetic stimulation is a new method for objective evaluation of the facial nerve without constraints for the patient. By the first time, especially cortical lesions can be detected objectively. Using this method, a permanent disturbance of the ramus marginalis mandibularis after operative treatment of fractures of the mandibular condyle could be certainly excluded. Therefore, because of the clear presentation and the possibility of enlargement, the deep submandibular approach, using a correct operative technique, can be strictly recommended.

Superior laryngeal nerve paresis and paralysis are relatively common but often difficult to diagnose with certainty. They are most commonly caused by viral infections, though other etiologies must be considered. A thorough history and physical examination, including strobovideolaryngoscopy and laryngeal electromyography, are needed for definitive diagnosis. It is essential to establish the diagnosis accurately to differentiate an apparent superior laryngeal nerve paresis from other conditions, such as myasthenia gravis. Laryngeal electromyography is used to confirm clinical impressions, as a g
