BACKGROUND: In the literature the possibility of a pressure trauma to the hypoglossal nerve between the root of the tongue and the ligamentum stylohyoideum is discussed. A recent in vivo study examined the effects of forced reclination of the head on the nerve.
PATIENT: A left sided hypoglossal nerve paralysis occurred in a patient after an endoscopically controlled endonasal operation under general anesthesia with transoral intubation.
RESULTS: Clinically and electrophysiologically the palsy seemed to be transient due to neurapraxia. An X-ray of the neck performed after the operation reveal

OBJECTIVE: Subtotal cordectomy and posterior cordectomy have repeatedly been recommended as surgical interventions restoring the airway, for the treatment of bilateral vocal cord paralysis. The objective of this study was to assess the effectiveness of transoral laser cordectomy and posterior cordectomy as compared to laser arytenoidectomy and to compare the respiratory and phonatory results of these minimally invasive procedures.
MATERIAL AND METHODS: Forty patients with bilateral vocal cord paralysis were included in a prospective study and operated upon to improve their laryngeal airways.

Traumatic root avulsion brachial plexus injuries in 216 patients were treated with spinal accessory-musculocutaneous neurotization to restore elbow flexion. The average postoperative follow-up period was 6 years, with a minimum of 2 years for all patients. The percentage of satisfactory biceps recovery (MRC III or better) was 72.5%. The average interval between the operation and MRC III motor recovery was 17 months. The percentage of poor results increased from 25.5% to 62.5% in patients who underwent operation later than 9 months after injury. This method of neurotization produces a result co

Thirty-five cases of unilateral acoustic neurinomas were analyzed with special reference to the postoperative eighth cranial nerve function. An additional three cases of bilateral acoustic neurinomas associated with neurofibromatosis were also analyzed. Out of a total of 40 neurinomas in all, 38 cases were retrospectively reviewed. The thirty-five cases of unilateral acoustic neurinomas were summarized as follows. The patients' age ranged from 23 to 69 years old. The tumor size varied as follows; 7 cases were confined to the internal acoustic meatus, 4 cases were 20 mm or less in their maximum

Cerebrospinal fluid otorhinorrhea after surgery for cerebellopontine angle tumors may persist despite obliteration of the mastoid, middle ear, and tympanic orifice of the eustachian tube. In this study, histologic sections of 120 adult temporal bones were examined by light microscopy to determine the incidence of peritubal pneumatization and to demonstrate the frequency of tubal and tympanic openings of the peritubal cells. The results of this study suggest that the pathway for these persistent cerebrospinal fluid leaks may be via the peritubal cells that open directly into the eustachian tube

Horner's syndrome is described in three cats associated with wounds to the ventrolateral neck. In each case, ipsilateral laryngeal hemiplegia was observed on laryngoscopy. This finding provided strong evidence to support a diagnosis of second order Horner's syndrome due to disruption of the cervical sympathetic trunk, as motor fibres innervating laryngeal abductors also traverse the neck; both as descending fibres within the contiguous cervical vagus and as ascending fibres within the recurrent laryngeal nerve. Notably, the ability to vocalise was unimpaired in all cases and, in two cats, neck

Bilateral traumatic abducens nerve palsy is a rare condition. We report a case associated with cervical spine flexion injury. This may be the first such case report, as no similar case was found in our review of the literature. The mechanisms of injury in this case are relevant to theories that explain hyperextension injuries.

Observations of rhythmic or semirhythmic myoclonus due to a peripheral nerve lesion are exceptional. We report on a patient with thorax trauma with multiple bilateral hematomas of the paravertebral musculature. Eight years later he developed rhythmic myoclonus of both trapezius muscles and thoracic pain. Infiltration of a paramedially located scar at the level of D5-6 with a local anesthetic agent led to an intermittent relief of the myoclonus as did anesthetic blockade of the left accessory nerve. Surgical excision of the scar, which contained multiple dystrophic axons on histological examina

During the years 1985 to 1992, we encountered 59 patients with meningiomas involving the space of the cavernous sinus. In 29 of these patients, meningiomas were primarily located within the space of the cavernous sinus and were operated on without mortality and with low morbidity. A small subtemporal surgical approach was favored, which allowed initial tumor resection from the posterior aspect, where the Parkinson's triangle is wide, thus avoiding the additional morbidity of large-scale approaches. According to the relationships of the all-important cranial nerves passing within the lateral wa

Most lesions of the spinal accessory nerve are of traumatic origin. If the proximal part is sectioned next to its exit from the cranial base, the reconstruction might be difficult. In such a case, one option is intracranial identification of the spinal accessory nerve and transdural interposition of a graft to its distal stump. Cerebrospinal fluid leaks or infections, caudal nerve palsies, or even spinal neurological deficits are possible complications. From more than 70 patients who underwent selective peripheral denervations for the treatment of spasmodic torticollis in our department, we ha

PURPOSE: To delineate the relationship between the optic nerves and the posterior paranasal sinuses using CT data.
METHODS: Direct coronal sinus CT scans of 150 consecutive patients with chronic inflammatory sinus disease were reviewed by two radiologists. Axial oblique reconstructions along the course of the optic nerve were obtained for the first 100 patients. The direct relationship between the optic nerve and the posterior ethmoid and sphenoidal sinuses was recorded, as were identations into the sinus wall, course of the nerve through the sinus region, pneumatization of the anterior clino

PURPOSE: Two cases illustrate the symptoms, signs, etiologies, and treatment of ocular neuromyotonia (ONM).
METHODS: The histories, neuroradiologic tests, and/or biopsy revealed the etiologies of ONM in both patients. Clinical observations, videotaping, and electronic eye movement recordings documented the eye movements.
RESULTS: A 72-year-old man with chronic arachnoiditis following myelography with thorium dioxide (Thorotrast) developed intermittent diplopia and a partial right third nerve palsy. Left gaze induced spasm of the right medial rectus. Right gaze produced right lateral rectus s

Intratemporal enhancement of (Gd-DTPA) was investigated by an interleaved-overlapping magnetic resonance imaging (MRI) technique in 35 cases of acute facial palsy. In a reference group (normal facial function), enhancement was localized from the ganglion geniculi to the stylomastoid foramen. In cases of acute palsy, the facial nerve enhanced in the meatal fundus independent of etiology (idiopathic, herpetic, or traumatic). Exposure of the facial nerve from the brainstem to the parotid can be accomplished without injury to the nerve, tympanic membrane, external auditory canal, ossicular chain, inner ear or structures within the cerebello-pontine angle. The procedure has reliably provided good results for patients who have had the proper indications with facial paralysis from Bell's palsy, herpes zoster oticus, infection, hemi-facial spasm, temporal bone fracture and tumors. The current technique for exposure through the mastoid, middle cranial fossa and retrolabyrinthine combined approaches are described. This te

Bilateral acoustic neurinomas are found in almost all patients with neurofibromatosis 2 (NF2). With better knowledge of the disease and development of diagnostic procedures, an increasing number of patients have been detected at an early stage of the disease. Preservation of hearing in these cases has become more feasible. Between 1979 and 1992, 110 acoustic neurinomas in 74 NF2 patients have been resected in this department. All tumours but 11 were completely removed. In 11 cases deliberate subtotal tumour removal was performed: bilaterally in one deaf patient for brainstem decompression, and

The authors report a retrospective study of 351 parotidectomies observed during a 28 years period. The average follow-up is 28 months. Preoperative and post-operative facial nerve function, type of parotidectomy, surgical management of facial nerve and histologic diagnosis according to the revised WHO classification (1990) are presented. Type of parotidectomy and degree of tumor malignancy are statistically analyzed. It seems that occurrence of post-operative facial nerve dysfunction depends on radical parotidectomy, whereas long term dysfunction is determined by tumor malignancy. Malignant ep

Optochiasmic arachnoiditis is a complex symptomatological ocular and neuroendocrine disease. We present a 32-years-old patient who was hospitalized in Ophthalmological Clinic from Iaşi because a slow visual acuity diminishing, visual field troubles and neurological symptoms which appeared after a cranial contusion. Ophthalmological and neurosurgical exams established "optochiasmic arachnoiditis" diagnosis, which was confirmed peroperator. The exceptional postoperator evolution was, after a while, eclipsed by final functional ocular result.

265 carotid endarterectomies in 210 patients (all vein patch in the carotid bifurcation) were investigated postoperatively for the presence and reversibility of cranial nerve palsy especially for N. facialis, N. hypoglossus, and N. vagus paresis. Additionally, the anatomical relationship between the affected cranial nerve and the carotid artery and the influence of surgical case load on the incidence of cranial nerve paresis were investigated. Altogether 28 (10.5%) cranial nerve injuries were registered. In 6 (2.2%) cases the hypoglossus nerve was injured, in 13 (4.9%) the vagal nerve (N. lary

Primary internal ophthalmoplegia is a rare ocular syndrome after head injury. Its pathogenesis remains unclear and it is only a hypothesis that the pupillomotor fibres on the ventromedial aspect of the oculomotor nerve are slightly contused against the petroclinoid ligament at the moment of impact. We observed a patient after head injury who presented with a fixed, dilated pupil and died within 15 hours of the accident. The postmortem examination revealed an isolated group of fibres on the medial aspect of her oculomotor nerve, which had been torn out of the midbrain. This suggests an alternat

Neurological consequences of extractions of the third inferior molar can be severe. Progress in imaging techniques has made it possible to identify molars << at risk >>. Extraction by wear without removal of the apex is proposed as a means of avoiding damage to the alveolar nerve. If no surgical prophylaxy is used the risk is major sequellae: painful anaesthesia of the area involved. There is no really effective treatment for such sequellae, creating persistent psychological problems throughout life.
