Teflon granuloma presenting as an enlarging, gadolinium enhancing, posterior fossa mass with progressive hearing loss following microvascular decompression.

Deleterious effects of Teflon strand placement during microvascular decompression (MVD) for hemifacial spasm are rare. In this report, a patient who had previously undergone suboccipital MVD for hemifacial spasm presented 3 years postoperatively with a progressive asymmetric sensorineural hearing loss and magnetic resonance imaging evidence of an enlarging ipsilateral gadolinium enhancing 1-cm cerebellopontine angle lesion. At surgery a granuloma was found displacing the structures of the internal auditory canal. Histologically, evidence of a Teflon fiber-induced giant cell granuloma was ident

Pneumocephalus caused by fistulas of the mastoid air cells treated with a temporoparietal fascial flap.

Using a temporoparietal fascial flap and hydroxyapatite ceramics, we treated a patient for complications after a neurosurgical operation for glossopharyngeal neuralgia. These consisted of pneumocephalus and cerebrospinal fluid rhinorrhea resulting from fistulas of mastoid air cells associated with a subcutaneous dead space. By means of the temporoparietal fascial flap, we were able to fill the dead space and reinforce the repaired dural and mastoid lesion as well. Hydroxyapatite ceramics were also useful for closing the mastoid air cell fistulas.

Protracted posttraumatic optic disc swelling.

BACKGROUND: Optic disc swelling is a rare sequela of blunt ocular trauma. METHODS: The authors examined three young patients who had an unusual post-traumatic optic neuropathy in which protracted swelling of the optic nerve head was the salient clinical feature. RESULTS: Associated choroidal ruptures in two patients suggested a contrecoup mechanism of injury to the optic nerve at its junction with the globe. All patients had partial recovery of vision over months, concurrent with resolution of the nerve head swelling and development of optic disc pallor. CONCLUSION: Despite its protracted c

Evaluation of the pathogenesis of vagus indigestion in cows with traumatic reticuloperitonitis.

OBJECTIVE--To evaluate the functional importance of impaired reticular contractions attributable to inflammatory adhesions in the pathogenesis of vagus indigestion in cows with traumatic reticuloperitonitis (TRP). DESIGN--Case control study. ANIMALS--42 dairy cows with TRP were compared with 10 healthy control cows. Cows with TRP were assigned to 3 groups: cows with uncomplicated TRP (n = 15), cows with TRP and a disturbance of digesta passage through the reticular-omasal orifice (TRP-AD; n = 17), and cows with TRP and a disturbance of digesta passage through the pylorus (TRP-PD; n = 10). Superior orbital fissure syndrome: current management concepts.

The superior orbital fissure syndrome is an uncommon complication of craniofacial fractures: middle-third facial fractures and lesions of the retrobulbar space. This article reviews the anatomy and etiology of the superior orbital fissure as it relates to pathophysiology and physical findings. Cases reported in the literature are reviewed, emphasizing diagnosis and established treatment options. Two cases are presented and their management discussed, including the use of pre- and postoperative steroids as an adjunct to standard fracture reduction and stabilization therapy.

Late effects of radiation on the eye and ocular adnexa.

A clinically useful classification system is suggested that can be used in prospective trials to evaluate the effects of radiation on the visual system. We review radiation-induced pathophysiological and clinical changes of the various ocular structures as well as dose-response data and management of ocular complications. The rationale for the classification scheme chosen is also discussed.

Optic nerve fiber count and diameter of the retrobulbar optic nerve in normal and glaucomatous eyes.

BACKGROUND: The caliber of the retrobulbar optic nerve and the count of optic nerve fibers vary considerably in normals. The diameter of the retrobulbar optic nerve also decreases with optic nerve atrophy. This study aimed to determine the relationship between the caliber of the optic nerve and the optic nerve fiber count. METHODS: We counted the optic nerve fibers and measured the diameter of histological cross sections of the optic nerve for 56 normal subjects and 23 patients with absolute glaucoma. RESULTS: The optic nerve fiber count increased significantly (P < 0.0001) by 777,000 fibers

[Unilateral laryngeal paresis as an early symptom of recurrent nerve compression caused by tumor or trauma].

Out of 19 patients with unilateral laryngeal paresis, 8 patients developed the paresis early after strumectomy, 4 had lung tumors, 2 tumors of the esophagus, 2 stab cervical wounds, 2 aortal arch dilatation, 1 patient had retrosternal goiter. Peripheral paralysis of the recurrent nerve is caused most frequently by the nerve compression in the thoracic cavity. The disease may be diagnosed accurately by ascertaining the causes underlying the nerve compression in the thoracic cavity using x-ray, ultrasound and computed tomography. Early diagnosis of mediastinal and pulmonary tumors enable conserv

Facial nerve palsy after mandibular fracture.

A 19-year-old man sustained a right parasymphyseal fracture and bilateral condylar neck fractures in a motor vehicle accident. The parasymphyseal fracture was treated by open reduction and internal fixation, and the subcondylar fractures were treated with closed reduction and maxillomandibular fixation. Three days postoperatively, a near-complete left facial nerve palsy developed. Facial nerve recovery was not full. The literature is reviewed, and possible mechanisms of this rare and devastating complication are discussed.

Magnetic resonance imaging of intratemporal facial nerve lesions in the animal model.

The facial nerve is often injured in head trauma. Computed tomography (CT) is the diagnostic study most frequently used to evaluate temporal bone fractures, but it does not demonstrate all soft tissues well. Recently, magnetic resonance imaging (MRI) has been used to evaluate certain soft tissues, including cranial nerves. Experience with MRI evaluation of temporal bone trauma is limited and consists primarily of anecdotal reports. This study assesses the accuracy of MRI in evaluating experimentally induced acute intratemporal facial nerve lesions. The tympanic segments of the right facial ner

Transposition of the facial nerve in congenital aural atresia.

It is generally recognized that surgery for congenital aural atresia is difficult. The success or failure of the operation is often directly related to the degree of development of the middle ear. In poorly developed middle ears, the facial nerve may overhang and conceal the oval window niche, making this area inaccessible to inspection, let alone manipulation. The criteria for transposing the facial nerve are; (1) the atresia must be bilateral, (2) there must be preoperative imaging evidence of a stapes and/or patent oval window, (3) there must be no large blood vessels feeding or draining th

[Branchiogenic anomalies (with special reference to severe and complicated outcome)].

Lateral cervical fistulae are usually described as examples for inborn branchiogenic anomalies. They result from evolutionary disorders of the first and second branchiogenic arch. Four patients were seen with cervical fistulae, three of those having a recurrence after surgical treatment. After a short review of the evolution and the topographic anatomy of such cysts and ducts, four patients are presented with all the details regarding their partially very complicated history.

Identification of the external branch of the superior laryngeal nerve (EBSLN) in large goiters.

BACKGROUND: Intraoperative injury to the external branch of the superior laryngeal nerve (EBSLN) can result in significant postoperative voice problems. This injury can be avoided by intraoperative nerve identification. The EBSLN has a close anatomic relationship with the superior thyroid pedicle. According to the previous anatomic classification, the type 2b nerve, which crosses the vessels below the superior thyroid pole and is considered high risk, is found in 14% to 20% of persons with normal or slightly enlarged thyroid glands. OBJECTIVE: To analyze the frequency of this type 2b nerve in

[Identification of the recurrent nerve in thyroid surgery. Technical note].

The authors report an experience of 1800 surgical operations for thyroid diseases executed, with identification of the recurrent laryngeal nerve in all cases, from 1988 to 1992, and analyse the various causes of recurrent laryngeal nerve injury. This complication of thyroid surgery is not completely avoidable, in spite of an extremely accurate surgical technique. In most instances of injury to one of the recurrent laryngeal nerves during thyroidectomy, the surgeon can confirm that the recurrent laryngeal nerve was not sectioned during the operation. This fact demonstrates that often the recurr

Role of vagal feedback from the lung in hypoxic-induced tachycardia in humans.

We assessed the cardiovascular responses to systemic normocapnic hypoxia in five normal subjects, five double lung transplant patients with lung denervation and intact hearts, and five patients with denervated hearts. Progressive normocapnic hypoxia was induced over 10-15 min and maintained for 2-3 min each at 90, 87, 84, and 80% arterial O2 saturation (SaO2). Normal subjects showed the most pronounced mean increase in heart rate (dHR/dSaO2 = 0.86 +/- 0.13 beat/min per 1% SaO2). Three lung-denervated subjects had normal tachycardiac responses (1.6, 0.9, and 0.69 beats/min per 1%), whereas the

A case of isolated pure trigeminal motor neuropathy.

Isolated trigeminal neuropathy after cranial trauma is exceptional. A 40-year-old woman was found to have unilateral trigeminal motor paralysis with muscle atrophy, unaccompanied by sensory involvement and without affecting other cranial nerves. She developed this deficit immediately after head trauma.

Visual loss after neurosurgical repair of paraclinoid aneurysms.

OBJECTIVE: To describe three cases of unexplained visual loss after technically successful clipping of paraclinoid aneurysms. DESIGN: Three case reports are presented. RESULTS: Profound blindness occurred in all three patients. Only one patient had marked visual recovery. Two clinical patterns were observed. A fulminant orbital syndrome presumably from compromise of large draining veins of the orbit was observed in one patient. A retrobulbar optic neuropathy that might have resulted from either direct injury or damage to small dural vessels of the posterior optic nerve was observed in the ot

Traumatic facial nerve injuries: review of diagnosis and treatment.

Both blunt and penetrating craniofacial trauma may lead to severe facial nerve injury and sequelae of facial paralysis. Initial evaluation involves quantitation of motor deficits using a clinical grading system, such as the House-Brackmann scale. High resolution computed tomography is used for localization of nerve injury in suspected cases of temporal bone trauma. In the absence of gross radiographic abnormalities, electrophysiologic testing helps predict the likelihood of spontaneous recovery. In patients with deteriorating facial nerve injuries by electroneuronography, surgical exploration
