Patients with localized damage to the taste system often experience no subjective change in real-world taste experience. In an effort to understand this, eight patients who recently underwent acoustic neuroma removal were evaluated for taste loss. Localized taste testing showed that taste intensities decreased in the distribution of cranial nerve VII ipsilateral to tumor removal as expected, but asymmetries occurred for IX. Intensities were greater on the side contralateral to the tumor removal. In addition, palatal taste, also thought to be mediated by VII, was not totally abolished. It is co

BACKGROUND: The modified Harada-Ito procedure has been reported to be an effective treatment for correction of cyclotorsion in bilateral superior oblique palsy. However, there are no reports regarding its use in intraoperative adjustable suture surgery.
METHODS: The authors performed a retrospective study of 12 patients with traumatic bilateral superior oblique palsy who were classified as having either symmetric or asymmetric palsy according to the symmetry of the alternate hyperdeviation on side gazes. Cyclotorsion and vertical and horizontal deviation in the nine diagnostic positions were

We studied two young males who had visual field defects consistent with optic chiasmal injury after blunt frontal head trauma. One patient also had a unilateral optic neuropathy. Long-term follow-up disclosed complete bi-temporal hemianopsias in these patients. Both patients had midline basilar skull fractures that traversed the midclivus through the sella turcica floor, dorsum sellae, and sphenoid sinus. Magnetic resonance images did not identify intrachiasmal hemorrhage as the cause of the visual field defect. We believe this fracture pattern, in conjunction with the magnetic resonance image

Petrous bone fractures (PBF) in children are relatively frequent. They are mostly diagnosed after collisions and falls. The complications typically associated with PBF were different types of hearing disorders in 69.1% of the patients who had audiometry, liquorrhea in 16.5%, palsy of cranial nerves in 10.8% (facial nerve palsy in 9.4%), bacterial meningitis, stenosis of the external ear canal, and posttraumatic cholesteatoma in 0.7% of the fractures each. Most complications were transient; 8.6% of the patients underwent surgery because of PBF-related complications and 9.4% suffered from severe

BACKGROUND: The proper management of neurogenic visual loss after blunt head trauma is controversial. Non-treatment, corticosteroids, and surgical decompression of the optic canal are all currently considered to be reasonable alternatives. The goal of this study was to identify factors affecting improvement in patients treated with canal decompression.
METHODS: A retrospective analysis of 31 cases in which transethmoidal decompression of the optic canal had been performed for neurogenic visual loss after closed head trauma was conducted. Each patient was alert and free of injury to the globe

Iatrogenic injury of the recurrent laryngeal nerve is doubtless one of the main problems in thyroid surgery. 56 consecutive patients, which have undergone surgical treatment for thyroid disease over the past two years are analysed. The recurrent laryngeal nerve is routinely exposed during thyroid surgery in order to decrease the incidence of recurrent nerve lesions. Four patients had an unilateral paralysis prior to the operation. Six patients showed a paralysis after the surgical intervention, two of whom had a carcinoma, where the nerve had to be resected. The residual four cases had only a

Coronal and axial high resolution computed tomography of the mandible extends facilities in dentoalveolar surgery. Preoperatively the relationship between the mandibular canal and the roots of deeply displaced third molars can be determined precisely. After surgical removal of lower wisdom teeth iatrogenic defects of the lingual cortical bone can be detected. The additional information obtained when compared with conventional radiographs is demonstrated in case descriptions. Indications for this technique, radiation exposure and implications for surgery are discussed.

The authors analyse problems related to laryngeal nerves in matters of thyroid surgery. As a matter of fact laryngeal nerve injury is still one of the most common complications for patients undergoing thyroid surgery. Although the sharp reduction in this drawback, due to a better anesthesiological technique and a wider surgical experience, the most representative case report show an incidence ranging from 3% to 5%. After a short introduction on surgical anatomy, attention is drawn to the "recurrent nerve risk" but also the superior laryngeal nerve too, often injured with modifications of the v

A case is reported of basilar fracture of the occipital bone passing through the left occipital condyle, ascending the clivus anteriorly, with marked medial displacement of the bony fragment. The injury produced ipsilateral palsies of nerves IX through XII (Collet-Sicard syndrome). A CT scan accurately delineated the extent of the fracture. Partial neurological recovery ensued following surgery.

The nonrecurrent laryngeal nerve is vulnerable during thyroid surgery. Pre-operative recognition of this nerve allows surgeons to avoid injuring the nerve. This anomaly depends upon the aortic arch anomaly and, therefore, in some cases, surgeons are able to predict the presence of this nerve pre-operatively by chest X-ray or barium swallow. Two patients with nonrecurrent laryngeal nerve are reported, one of whom, a 48-year-old man, had an abnormal shadow of the aortic arch, which led to confirmation of a right aberrant subclavian artery. Barium swallow plays a decisive role in the diagnosis of

Several surgical approaches to the cerebellopontine angle and internal auditory canal have been developed for the removal of acoustic neuromas. The choice of an approach may be influenced by hearing levels and tumor size. We reviewed the records of the primary translabyrinthine removal of 167 large (> or = 4 cm) acoustic neuromas performed between 1982 and 1990. Patients ranged in age from 15 to 83 years, with a mean of 43 years (male, 49%; female, 51%). Total removal was achieved in 95%. The facial nerve was preserved anatomically intact in 91%. At follow-up (mean, 2.1 yr), facial nerve funct

The manifestations of diabetic neuropathy are diverse. However, cases affecting the lower cranial nerves are rarely seen. We report three cases of previously unreported, bilateral, vocal-cord palsy associated with diabetes mellitus: a presumed relationship.

Injuries to the central or peripheral nerves caused by medical intervention are called iatrogenic nerve lesions. The caudal relationship between medical intervention and iatrogenic nerve injuries is frequently not very clear. Careful pre- and post-operative examination of the patient, however, will in most instances reveal a nerve lesion in a timely manner. Iatrogenic nerve lesions in the clinical cases available to us were predominantly due to indirect influences, such as pressure, insertion of a retractor or heat; we discovered that only in a few cases a nerve was partially or completely sev

Direct operative management of aneurysms of the internal carotid artery is associated with significant morbidity. Preauricular surgical approaches to this area can result in violation of the inner ear, temporomandibular joint, or mandibular ramus, and cranial nerves V, VII, IX, X, XI, and XII. A new technique for saphenous vein bypass of the infratemporal and petrous internal carotid artery, using a postauricular curvelinear groove drilled in the mastoid cortex, is described. Proximally, the reverse vein graft is anastomosed to either the internal or external carotid artery, whereas the distal

The aesthetic and functional morbidity associated with facial palsy is significant. Fortunately, corneal exposure and ulceration with the potential for the loss of visual acuity can be minimized by eyelid procedures such as gold weight implantation and horizontal eyelid shortening. Although primarily designed for restoration of eyelid function, these procedures also improve ocular cosmesis. However, uncertainty remains regarding the indications for eyelid surgery: When should surgery be performed, in whom, and which procedure(s)? On the basis of our series of 60 patients evaluated for facial p

Examination of the cranial nerves is a simple and valuable procedure following trauma to the head and neck. By understanding their courses, branches and distributions, the site of any damage may be localized. Once the site and nature of injury has been determined, correct management may be instigated. A case is presented in which there was some degree of trigeminal nerve damage following trauma. The site of damage was determined to be extracranial and therefore a conservative regimen adopted. The mechanism of injury is postulated.
