The stapedial reflex in cephalic tetanus.

Three patients are presented with cephalic tetanus following injuries to the face. Two were adults and one a child. All three had bilateral VIIth cranial nerve involvement and one patient also presented with involvement of the IIIrd, IVth and VIth cranial nerves. The patients initially an ipsilateral VIIth cranial nerve weakness which later in the course of the illness developed into hyperactivity of the VIIth cranial nerve. The contralateral VIIth cranial nerve demonstrated a similar pattern. The stapedial reflex was tested serially. The stapedius muscle activity preceded that of the muscles

A retrospective analysis of 279 patients with isolated mandibular fractures treated with titanium miniplates.

PURPOSE: This article addresses the suitability of semirigid fixation for the treatment of mandibular fractures. MATERIALS AND METHODS: Between 1986 and 1991, 279 patients with 447 isolated mandibular fractures were treated with miniplate fixation using the tension-band principle of Champy et al. The time from trauma to treatment, etiology, number and location of the fractures, and the presence of preoperative infection and neurosensory disturbances were recorded. Postoperative complications such as infections, neurosensory disturbances, malocclusion, and nonunions also were recorded, as well

Intraoperative monitoring of the vagus nerve during intracranial glossopharyngeal and upper vagal rhizotomy: technical note.

Intracranial section of the glossopharyngeal and upper vagal rootlets for the treatment of vagoglossopharyngeal neuralgia may cause dysphagia or vocal cord paralysis from injury to the motor vagal rootlets in 10% to 20% of cases. To minimize this complication, we recently applied a technique of intraoperative monitoring of the vagus nerve (previously described by Lipton and McCaffery to monitor the recurrent laryngeal nerve during thyroid surgery) in a patient undergoing intracranial rhizotomy for vagoglossopharyngeal neuralgia. By inserting an electrode in the ipsilateral false vocal cord and

Functional superficial parotidectomy.

PURPOSE: To clinically test a new type of parotid surgery termed functional superficial parotidectomy (FSP) to preserve the function of the residual gland. PATIENTS AND METHODS: Sixteen human parotid specimens were dissected, and 19 patients underwent FSP. RESULTS: It was shown that 91.4% of the main parotid ducts were found deep to the surface of the facial nerve. Physical examination, sialography, and scintigraphy showed that function of the residual gland was well preserved by FSP. CONCLUSION: A comparison of the main complications between FSP and conservative superficial parotidectomy s

[New aspects in surgery of the thyroid gland with intraoperative monitoring of the recurrent laryngeal nerve].

A method for electrophysiological monitoring of the recurrent laryngeal nerve during thyroid surgery is described. An electromyographic record from the muscles of the vocal folds is obtained using two fine needle electrodes. The electrodes are placed endoscopically. The nerve is identified by electrical stimulation, and the electromyographic activity is registered using a Nerve Integrity Monitor NIM-2 (Xomed-Treace). Our experience with this method in 43 patients is described. The safety of the integrity of the recurrent laryngeal nerve is increased using intraoperative monitoring. This has le

[Detailed results of orbital and optic nerve decompression].

Orbital complications of various pathogenesis include impairment or loss of vision inhibition of globe motility and exophthalmos, and are treated best by an interdisciplinary team. Preoperative strategy should include information about underlying diseases, since success of treatment is related to diagnosis. To analyse our postoperative results we examined four groups of cases: injuries, tumors, infections and mucoceles. Diseases were located in the orbit or paranasal sinuses and produced similar symptoms. But prognosis and results of surgical treatment produced significant differences. The bes

Facial nerve stimulation after successful multichannel cochlear implantation.

Postoperative rehabilitation after cochlear implantation may be difficult and frustrating for both patient and audiologist. The occurrence of facial nerve stimulation with associated twitching and discomfort can interfere with the rehabilitative process and be a cause of great concern to the patient. Such stimulation can occur at the time of initial "tune-up" or can be delayed by months or years. The incidence of postoperative facial nerve stimulation 109 consecutive patients operated by the same surgeon over a period of 7 years is reviewed. Possible causes for immediate and delayed stimulatio

Adult expression of DeMorsier syndrome following head trauma.

DeMorsier syndrome is a well-described entity, which includes optic nerve hypoplasia and absence of the septum pellucidum with or without pituitary abnormalities. Patients with all three aspects of this syndrome are diagnosed in childhood due to their neuroendocrine dysfunction. We present a review of the literature and a case report of an adult diagnosed with DeMorsier syndrome when he developed neuroendocrine abnormalities after head trauma.

Translabyrinthine and transotic surgery for acoustic neuroma.

The results and complications of translabyrinthine and transotic surgery for petrous apex lesions between 1980 and 1992 are presented. An acoustic neuroma was found in 52 patients. In 1988, the translabyrinthine approach was modified into the transotic approach and replaced the former technique. There was no mortality in this series, but two patients had mild brainstem infarcts and there was post-operative bleeding into the cerebellopontine angle in one. Cerebrospinal fluid (CSF) leakage was seen in six patients and meningitis in two. Three suffered deep vein thrombosis in their legs. There wa

Clinical application of the blink reflex with stimulation of the mental nerve in lesions of the inferior alveolar nerve.

We evaluated the clinical usefulness of a new modification of the blink reflex (BR) test with electrical stimulation of the innervation area of the mental nerve (MN). We studied the MN BR in 28 patients with traumatic lesions of the inferior alveolar nerve and compared the results with the clinical sensory examination. Within 3 months of the trauma, the MN BR was abnormal in 18 of 20 nerves. In lesions older than 3 months, the test was often normal (in 9 of 11 nerves) despite subjective sensory symptoms and mild clinical signs. Anatomic obstacles to nerve regeneration also gave rise to abnorma

Reflex sympathetic dystrophy of the head: case report and discussion of diagnostic criteria.

The case of a patient who was involved in a car crash is presented. On the right side her wrist and zygomatic arch were fractured. At both localizations she developed a painful syndrome. The diagnosis of reflex sympathetic dystrophy of her arm was made within 3 weeks after the injury and therapy instituted. The pain in her face and skull were accompanied by vasomotor changes which increased after exercise and by hypesthesia, hyperpathia, paresis, and hyperhidrosis. Disorders of the cranial nerves were excluded by physical examination and electromyography and structural disorders by CT scanning

Blindness after maxillofacial blunt trauma. Evaluation of candidates for optic nerve decompression surgery.

Clinical and computerized tomography (CT) findings in 10 patients with post-traumatic blindness after maxillofacial blunt trauma were evaluated as potential candidates for optic nerve decompression surgery. The total number of blind eyes was 14. All patients suffered from midfacial fractures (isolated zygomatic fracture in 4 and Le Fort fracture in 6 cases) predominantly caused by road traffic accidents. CT scanning and clinical findings allowed location of damage to the optic nerve region in all patients. The most common CT finding was swelling of the optic nerve. Fracture in the optic canal

Progressive infraorbital nerve hypesthesia as a primary indication for blow-out fracture repair.

Traumatic blow-out fractures of the orbital floor are a common injury that can lead to significant morbidity. Accepted indications for surgical repair include displaced fractures consisting of a defect > 50% of the orbital floor, extraocular muscle entrapment, and clinically significant enophthalmos. Although infraorbital nerve hypesthesia has been reported as an indication for repair of fractures of the zygomatic complex and is often encountered as an associated finding in fractures of both the orbital floor and inferior orbital rim, it has not been generally regarded as a primary indication

Trigeminal sensory loss in orbital disease.

A study was undertaken to estimate the incidence of periorbital trigeminal sensory loss with different types of orbital disease and to determine whether the area of loss is related to the location or nature of the underlying condition. A retrospective review was carried out of the clinical case notes of patients identified from the orbital diagnostic database held at Moorfields Eye Hospital. A total of 103 patients (51 male, 52 female), aged between 11 and 84 years (median 51 years), had signs of periorbital trigeminal sensory loss at the time of investigation for orbital disease and in 14/103

[Ophthalmological symptoms of visual tract lesions in craniocerebral injuries].

Craniocerebral injuries are known to involve the visual tract in 2-5% of cases. Fifty-nine patients aged 5 to 68 with visual tract involvement in craniocerebral injury were examined in N. N. Burdenko Institute of Neurosurgery of the Russian Academy of Medical Sciences. Unilateral optic nerve involvement was found in 48 patients, involvement of the chiasm and/or both optic nerves in 11. Involvement of a single optic nerve was associated with vision acuity reduction, 30 patients becoming blind or virtually blind, and with various defects of the visual field. Traumatic injury to the chiasm manife

Unilateral palatal adhesion for paralysis after high vagal injury.

Tumor involvement or sacrifice of the vagus nerve at the level of the jugular foramen is a cause of significant morbidity in skull base surgery. Management of the resulting dysphagia and dysphonia has been significantly improved by the development of vocal cord medialization. However, rehabilitation of palatal paralysis with the resulting velopharyngeal incompetence has not been widely addressed. In an effort to alleviate the symptoms of hypernasality and nasal reflux, unilateral adhesion of the palate to the posterior wall of the nasopharynx is proposed. This report summarizes the operative a

Primary surgical management for rehabilitation of the paralyzed eye.

Protection of the eye is the primary concern in managing the patient with facial paralysis; however, the aesthetic consequences cannot be overlooked. Lagophthalmos has traditionally been treated with frequent ocular lubrication, medical eye care, and tarsorrhaphy. This approach is fraught with the difficulties of poor patient compliance and unacceptable cosmesis. We suggest a protocol for the complete ocular management with emphasis on immediate rather than delayed surgical rehabilitation as a primary therapy in facial paralysis. Gold weight implantation to the upper lid and tightening of the
