Blunt head injury frequently results in visual impairment, the optimal treatment of which is still debated. Over a 5-year-period (1987-1991) 111 patients with indirect optic nerve injury resulting from closed head trauma have been treated. In each case loss of vision was the only neurological deficit. In group A, 66 patients were initially treated with large doses of prednisolone (80 mg/day) for 3 weeks. Twenty-seven patients improved on steroids alone. In the remaining 39 patients in whom either unsatisfactory or no improvement occurred a transethmoidal optic nerve decompression was performed

Recurrent laryngeal or vagus nerve injuries in the mediastinum are repaired rarely because of technical difficulties. Impairment in phonation is especially severe in patients with respiratory dysfunction. We performed a simple and less invasive reconstruction, ansa cervicalis-recurrent laryngeal nerve anastomosis in the neck, to improve phonation in 2 patients. Although the reinnervated vocal cord did not regain normal movement, both of the patients obtained excellent improvement in phonation.

The external rhinoplasty approach is a refinement of the well-recognized sublabial transseptal technique for transsphenoidal hypophysectomy first introduced by Cushing in 1910. This article relates our experience with 111 cases of transsphenoidal hypophysectomy performed during a 10-year period (1982-1992) and includes a detailed description of our use of the external technique. Fifty-one patients were male, and 60 were female. Ages ranged from 12 years to 80 years, with an average of 46 years. One hundred one patients had pituitary adenomas, four had craniopharyngiomas, two had inverting papi

Between 1980 and 1991, 13 of 1312 carotid artery reconstructions were performed for aneurysms of the extracranial internal carotid artery. The patients included 12 men and one woman, mean age 56 years. Postoperative aneurysms and aneurysms involving the common carotid artery were excluded from the study. In six cases, the upper part of the aneurysm was higher than a line joining the mandibular to the tip of the mastoid process. Six aneurysms were atherosclerotic, two post-traumatic, four dysplasic and one post-traumatic and atherosclerotic. Nine patients had focal neurological symptoms (seven

The recommended treatment of penetrating traumatic facial nerve injuries associated with immediate, total paralysis of the ipsilateral facial muscles generally includes facial nerve exploration and repair. We reviewed our experience with bullet injuries to the extratemporal facial nerve to determine the efficacy of this approach. Five patients with immediate, total facial nerve paralysis caused by bullet wounds near the extratemporal facial nerve were seen between July 1990 and December 1992. Of four patients who underwent surgical exploration, only one demonstrated complete transection of the

Intraoperative facial nerve monitoring is performed by electromyographic recording of motor action potentials generated by facial nerve stimulation or mechanical damage of the facial nerve, and helps the surgeon to localize the course of this nerve, reduce the possibility of its injury during surgery, and assure its anatomical and functional integrity. We report our experience on the intraoperative facial nerve monitoring in 35 patients, ten of which belong to absolute indications for monitoring, and to relative indications the remaining cases. In every case, we used the Nerve Integrity Monito

Referring to 22 cases of surgery after frontobasal fractures of different origin with loss of vision, the surgical procedure is discussed with special consideration of the approach, technique and timing of the operation. We conclude that the external rhinological approach through the ethmoid sinus is the best and easiest access in cases of loss of vision with verified fracture or haematoma. A number of patients were operated using CAS (Computer Assisted Surgery)-Systems, a CT-based tool for three-dimensional orientation. It proved to be very useful for the surgeon, saving time and reducing the

Several mechanisms have been postulated to explain the optic nerve damage that occurs in primary open angle glaucoma (POAG). No single mechanism can adequately explain the great variations in susceptibility to damage and the patterns of damage seen in this syndrome. The etiology of POAG is likely to be multifactorial. Mechanical, vascular and other factors may influence individual susceptibility to optic nerve damage. An enhanced understanding of the nature of the optic nerve damage in POAG and improved methods of study may result in earlier diagnosis or may allow us to distinguish among diffe

The surgical removal of intradural petroclival tumors of any size is demanding because of the restricted surgical exposure and relation of the tumor with the brain stem, cranial nerves, and vertebrobasilar artery and branches. When petroclival tumors are large or giant sized and involve multiple areas of the clivus, adequate surgical exposure is difficult to achieve, yet is critical for safe tumor removal. In 17 patients with extensive petroclival tumors, total removal of the petrous bone was performed to gain a wide and shallow surgical exposure. This report details the intraoperative and pos

Despite recent advances in surgery of the cavernous sinus, meningiomas in that area offer a formidable challenge. The rationale for aggressive surgical removal of cavernous sinus meningiomas is based on the presumption that the extent of removal is inversely related to the rate of recurrence. Over the past 10 years, 41 patients with histologically benign meningiomas involving the cavernous sinus underwent aggressive surgery. Total removal, as confirmed by intraoperative inspection and postoperative radiological studies, was achieved in 31 patients (76%). Twelve patients have been followed for

Endoscopic sclerotherapy is an effective method of controlling variceal hemorrhage, but complications frequently occur. Recent studies have shown that sclerotherapy may induce transient alterations in gastrointestinal motility. It has been suggested that these complications result from vagal nerve injury due to sclerotherapy. We have tested this hypothesis by measuring pancreatic polypeptide secretion in response to insulin-induced hypoglycemia (insulin 0.1 U/kg i.v.), a well-known stimulus for vagal nerve function. We studied six patients with cirrhosis and variceal bleeding (group A, mean ag

Recurrent laryngeal nerve (RLN) injury [without injury to the superior laryngeal nerve (SLN)] is the most common traumatic neurolaryngological lesion. The acute effects are immediate flaccidity of the ipsilateral vocal fold, loss of abduction and adduction, severe dysphonia to complete paralytic aphonia, and, frequently, aspiration of food and drink into the trachea. This article reviews the function of the four intrinsic laryngeal muscles innervated by the RLN and the individual effects of RLN transection on these muscles. Following acute denervation, the subsequent progression is either chro

Rhytidectomy is a safe and effective procedure for rejuvenation of the aging face. Reported complication rates vary between 2.5% and 28%, and with proper management, longterm sequelae are unusual. Injury to the spinal accessory nerve is a rare but potentially debilitating complication of rhytidectomy. Afflicted patients present with dull, constant pain in the shoulder region which can be serve in nature. This is the first case report on the diagnosis of spinal cord injury due to hemorrhage during the neonatal period using magnetic resonance imaging (MRI). Somatosensory evoked potentials are also helpful in the functional demonstration of this lesion. When discrepant signs, alert consciousness and intact cranial nerves are observed in newborn babies with flaccid extremities and respiratory disturbance, the immediate carrying out of MRI is mandatory in order to differentiate spinal cord injury from hypoxic-ischemic encephalopathy, cerebral hemorrhage, and neuromuscular disease.

The fracture of the Atlas is a serious trauma, associated with important diagnostic traps. Whether operative or noninvasive treatment should be used depends on the type of fracture. Paralysis of brain nerves are exceptionally rare. We present a case in which fracture of the atlas was associated with paralysis of the 9th through 12th brain nerves, to which differential diagnostic considerations are discussed.

A rare case, 32-year-old man, of cerebellar infarction with the occurrence of the 8th cranial nerve symptoms was reported. On the neuro-otological examination, hearing test and caloric test showed a severe hearing loss and no response on the right side, respectively, and the spontaneous horizontal nystagmus fixed to the left direction was observed. Magnetic resonance (MR) imaging showed the infarctions in the areas of anterior inferior cerebellar artery (AICA) and posterior inferior cerebellar artery (PICA) on the right side. On the 14th illness day, the 8th cranial nerve symptoms disappeared,

Neurootologists are fortunate in having a considerable number of surgical options available to them for the treatment of patients with disabling vertigo. Most surgery for vertigo is performed on patients suffering from the ravages of Meniere's disease. In addition, other forms of disabling peripheral vertigo may also be surgically managed. Over a 3 year period, the author performed surgery on 14 patients suffering from disabling non-Meniere's vertigo. During the same period, 60 patients with Meniere's disease underwent surgical treatment. There were three non-Meniere's conditions for which sur

The monitoring of auditory function by recording brain stem auditory evoked potentials in patients undergoing removal of acoustic tumors is hampered by the small amplitude of the brain stem auditory evoked potentials. Because several thousands of responses must be added, it takes several minutes to obtain an interpretable record. Recordings done directly from the exposed eighth nerve have much higher amplitudes, and, therefore, interpretable responses can be obtained after only a few responses have been added. However, it is difficult to place the recording electrode in an optimal position and

We report the surgical results of 13 accessory nerve neurotizations in branchial plexus birth palsy. The mean age at operation was 5.9 months. The accessory nerve was transferred to three C5 roots, to three C6 roots, to four posterior division of the middle trunks, to one musculocutaneous nerve, and to two suprascapular nerves. Sixty-seven percent of the cases acquired M4 or more in the deltoid muscle, 88% in the infraspinatus muscle, and 100% in the biceps brachii muscle. Twenty-five percent of the cases acquired M4 or more in the triceps brachii muscle and the wrist extensor muscles. These r

BACKGROUND: Congenital or acquired anomalous synkinetic movements are common in Neuro-Ophthalmology. Examples are the Stilling-Türk-Duane syndrome, the Marcus Gunn phenomenon or secondary oculomotor synkinesis in oculomotor nerve palsy.
PATIENT AND METHOD: A 29-year-old woman with N.V-, N.VI-, N.VII- and N.VIII-lesion after blunt cranio-cerebral trauma developed a left sided trigemino-abducens synkinesis. The abduction of the left eye by gaze effort was limited to 30 degrees before the midline. With tight jaw closure the eye reached the midline. Eye muscle surgery was performed (recess-resect
