The blood supply of the intracavernous cranial nerves: an anatomic study.

Cranial nerve deficits are the most common complications of cavernous sinus surgery. Often the deficit occurs despite anatomic preservation of the nerve, and ischemic injury is thought to be the cause. A better understanding of the blood supply of these nerves may help to prevent such complications. The authors performed a cadaveric microsurgical study of the intracavernous cranial nerves and their blood supply in 20 cavernous sinuses. The oculomotor nerve received branches from the inferolateral trunk or its equivalent in all specimens (100%). The proximal trochlear nerve received branches fr

Ocular and optic nerve hemorrhages in abused infants with intracranial injuries.

PURPOSE: To ascertain whether the pathology of the eye and optic nerve might be useful in determining the manner of death in infants who died after sustaining apparent nonaccidental intracranial injury. METHODS: Complete autopsies were performed on 13 infants who died of acute intracranial injuries after nonaccidental trauma. The infants were divided into two groups: nine with physical evidence of blunt trauma to the head, and four without detectable scalp or skull injury whose intracranial injuries were attributed to violent shaking (shaken baby syndrome). Six infants with no intracranial in

Vestibular nerve section.

INTRODUCTION: Vestibular nerve section is considered to be the most effective surgical procedure for control of intractable symptoms secondary to labyrinthine and eighth nerve function. This study was developed to retrospectively evaluate the efficacy of vestibular nerve section in patients treated for disabling labyrinthine dysfunction. METHODS: A retrospective review of hospital and office records was carried out on 39 patients who underwent vestibular nerve section. All patients received a comprehensive questionnaire to subjectively evaluate efficacy. RESULTS: Questionnaires were returned

Wound botulism.

All published cases of wound botulism were reviewed to describe the epidemiology, clinical manifestations, diagnosis, and treatment of this rare infection. The MEDLINE data base of English-language literature was searched from 1966 to 1992, using the keywords "wound botulism". Cases published during this period were identified, and the bibliographies of these articles were used to identify cases published before MEDLINE's search limit of 1966. Because of the limited number of published cases, all were reviewed. Data related to epidemiology, clinical manifestations, diagnosis and treatment were

Injuries to the inferior alveolar and lingual nerves.

Both the inferior alveolar and the lingual nerve are at risk for damage following various surgical procedures. Sensory nerve function following potential damage should be tested and evaluated for treatment, with microneurosurgery considered under certain conditions. Repair between three and six months following injury has the best results. A study of 43 patients who underwent microneurosurgery for various types of injuries, and have been followed for at least one year, found four (9.3 percent) with essentially complete return of sensation; five (11.6 percent) with good return; 19 (44.2 percent

[Facial nerve decompression for Bell's palsy].

Fifty-six cases of Bell's palsy that were examined and diagnosed with neuro-otologic and electrodiagnostic methods were treated with facial nerve decompression. Transmastoid posterior tympanotomy decompression of the facial nerve from the geniculate ganglion to the stylomastoid foramen was performed. The operative results of the 56 cases were as follows: satisfactory 23, fair 30 and moderate 3 respectively. 3 cases of mild high frequency hearing loss and 2 cases of eardrum perforation were found during follow-up. Factors correlated with curative effect, indication and precautions of operation

Syringobulbia: a surgical review.

Syringobulbia is a term which is applied to brain stem symptoms or signs in patients with syringomyelia or hindbrain herniation. Syringobulbia clefts due to a dissection of the CSF under pressure from the fourth ventricle should be differentiated from the ascending syringobulbia which may occur from upward impulsive fluid movements in a syringomyelia. Clinical analysis suggests that the commonest correlation of bulbar features is with neither of the above mechanisms but with pressure differences acting downward upon the hindbrain with distortion of the cerebellum and stem, traction on the cran

[Radionecrosis of the optic chiasma. A clinical and radiological study of 3 cases].

The paper reports three cases of optic chiasmal radionecrosis due to radiotherapy of the sellar region. These three cases are compared with 34 ones found in literature. The use of M.R.I. is shown to dramatically improve diagnosis. Corticoids are useful in the treatment. The gravity of the complication, which implies visual prognosis, focuses on a strict use of radiotherapy. For benign or slowly evoluting lesions, surgery and/or regular controls by modern imaging techniques would often be more eligible than systematic earliest irradiation.

Post-traumatic and postoperative peripheral paralysis of the facial nerve.

Forty-five patients with postoperative or post-traumatic paresis of the facial nerve were studied and it was found that in only 2.6% of the postoperative cases was there some temporary facial nerve palsy, while in 11% of the cases with facial lacerations the paralysis was permanent. The preauricular area was the most hazardous, while in the submental area no paresis was recorded. It is our opinion that (excluding the preauricular area) when designing a surgical approach to the face, no particular attention need be paid to the preservation of terminal branches of the facial nerve as no permanen

Traumatic optic neuropathy in children: a prospective study.

Fifty children with indirect optic nerve injury were prospectively studied over an 8-year period. They constituted 20% of all patients with optic nerve injury managed on a slightly different protocol. Half of these children were injured due to fall from a height, followed by road traffic accidents in 40% of the cases. Forty-three children had unilateral and 7 children had bilateral visual impairment. Five children had definite history of delayed-onset visual involvement, which ranged from 12 to 72 h following injury. Fifty percent of these children had fractured skulls and 14% had optic canal

Choroidal rupture and optic atrophy.

The association between post-traumatic optic disc pallor and traumatic choroidal rupture is poorly understood. To further define this relationship, nine cases of indirect traumatic choroidal rupture and post-traumatic optic disc pallor were compared with cases of indirect choroidal rupture without disc pallor in terms of severity of ocular injury, fundus findings, and visual outcome. The type and severity of the injury did not appear to influence the risk of optic disc pallor. Optic disc pallor was associated with a slightly poorer long term visual acuity than eyes without pallor (p = 0.059).

[Transcranial magnetic stimulation in perioperative damage to the facial nerve].

5 patients (3 men, 2 women, aged from 28 to 51 years) with unilateral facial palsy after surgery of a cerebellopontine angle tumour have been investigated by transcranial magnetic stimulation. The purpose was to evaluate the prognostic aspects of this method, which was compared with the electrical stimulation of the facial nerve and the elicitation of an orbicularis-oculi reflex. The components of the blink reflex were absent in all cases. In 3 patients electrical stimulation was possible (compound muscle action potentials were delayed). With transcranial magnetic stimulation ipsilateral short

[Pseudoglottis after laryngeal trauma with bilateral recurrent laryngeal nerve paralysis].

Voice is produced by a steady flow of air from the lungs, which is segmented at the laryngeal level into a series of air puffs. The segmentation is facilitated by periodic vocal fold vibrations under physiological conditions. In some cases, as in partial laryngectomy, segmentation of air stream is caused by vibrations of the false vocal folds. Here we report a case of an air stream segmentation mechanism at the laryngeal level producing an acceptable voice quality after fracture of the larynx and bilateral vocal fold paralysis. A lateral fixation of the left vocal fold was performed because of

Anastamoses and bifurcations of the recurrent laryngeal nerve--report of 1177 nerves visualized.

One thousand seventy-seven recurrent laryngeal nerves were observed in 719 patients. Seven hundred forty-seven nerves bifurcated or trifurcated over 0.5 cm inferior to the cricoid cartilage (63%). Of these, 170 patients had bilateral nerve bifurcations. Thirty-nine per cent of the patients had bilateral bifurcations when one side bifurcated. Eleven patients had direct laryngeal nerves, two of which bifurcated. Five of these 11 patients also had a recurrent and direct laryngeal nerve. Seven patients had recurrent laryngeal nerves, all on the right side, receiving branches directly from the vagu

[Intraoperative monitoring of the facial nerve in transtemporal surgery of acoustic neurinoma].

Acoustic neuroma ablation by the enlarged middle cranial fossa approach results in a low incidence of postoperative facial nerve dysfunction. In 25 consecutive patients with tumors in the cerebellopontine angle with a diameter up to 2.3 cm, intraoperative monitoring of the facial nerve was evaluated. The facial nerve was monitored electrophysiologically with recordings of the spontaneous EMG and electrically evoked potentials (CMAP) from at least three groups of facial muscles. In 4 cases monitoring was helpful for identification and delineation of the facial nerve. The mechanically elicited p

Muslin-induced optic arachnoiditis ("gauzoma"): findings on CT and MR.

We report a case with a paraophthalmic aneurysm that was wrapped with muslin because it was unclippable. Six months after wrapping, the patient developed gradual vision loss involving the ipsilateral eye. Both CT and MR studies demonstrated a loculated fluid collection with marked surrounding enhancement in the region of the previous surgery. Craniotomy revealed an abscess centered in the region of the aneurysmal wrapping. Pathological diagnosis was consistent with a foreign-body reaction with acute and chronic inflammation. Postoperatively the patient showed improvement in vision. We describe
