Bilateral blindness was diagnosed in a 5-year-old Hanoverian gelding presented for evaluation of a corneal opacity in one eye. The intensity of clinical manifestations (rubbing of the oro-facial area with skin injury), bioelectric activity of the somatosensory cortex and microcirculatory disturbances were studied for 6 weeks in Wistar rats with partial infraorbital nerve compression. A 41-year-old man presented with a unilateral atrophy and paresis of his tongue which were not explained by any pathological finding in the brainstem or at the base of the skull. The diagnosis of right internal carotid artery dissection was suggested by magnetic resonance imaging and ultrasound examination and confirmed by arterial angiography. Because of the diminishing number of patients diagnosed with otosclerosis, the adequacy of residency training in stapedectomy techniques remains controversial. A prospective study, conducted from 1986 to 1991, assessed whether or not surgical outcome obtained by residents, under close supervision by otologic faculty, could be improved using a single stapedectomy technique. The authors describe the usefulness of split silicone tubing to protect the cranial nerves and arteries during microneurosurgery. The inner diameter of the tube varied from 1.0 to 3.3 mm with a thickness of 0.125 mm. Application of the tube protects the nerves and arteries from mechanical trauma, electrical injury, and dryness.

Sacrifice of the facial nerve during surgery for parotid malignancy is rarely necessary, but the sequelae of facial nerve paralysis necessitates some form of facial re-animation. The technique which achieves the best results is that of immediate nerve grafting. This paper records our experience with 14 cases of extra temporal reconstruction of the facial nerve, utilizing the cutaneous branch of the radial nerve as an autograft.

We report a 66-year-old man with progressive spinal paraplegia. He was well until June of 1991 when he had an onset of backache and right chest pain. Intraoperative facial nerve monitoring has become an indispensable tool during cerebellopontine angle surgery. Ninety-seven patients underwent surgical removal of an acoustic neurinoma with the use of a pneumatic sensor monitoring between January 1, 1986, and December 31, 1990. Between 1974 and 1991, hearing preservation surgery was attempted on 161 of 476 patients with a variety of cerebellopontine (CPA) tumors, at New York University School of Medicine. This included 146 unilateral acoustic neuromas, seven meningiomas, and six cases of neurofibromatosis. The suboccipital/retrosigmoid approach was used almost exclusively. A report is given on first experiences with motor cortex stimulation in 10 patients with different forms of neuropathic pain. Three of them had central pain as sequelae of cerebrovascular disease. In none of them did the stimulation provide pain relief. Two patients had pain from peripheral nerve injuries. One did not respond, but the other obtained about 50% pain relief.
