Upper gastrointestinal dysmotility in heart-lung transplant recipients.

Recipient pneumonectomy and the necessity for meticulous hemostasis in heart-lung transplantation can result in injury to the vagus nerves as they course through the posterior mediastinum, with consequent delay in gastric emptying. This has been reported to lead to chronic aspiration and associated pulmonary sequelae. To study the association between delayed gastric emptying, bronchiectasis, and bronchiolitis obliterans after heart-lung transplantation, we performed esophageal manometry, 24-hour pH monitoring, and radioisotopic gastric emptying in 10 patients who underwent heart-lung transplan

[Ocular tissue after intravitreous silicone oil injection. Histologic and electron microscopy studies].

The intraocular tolerance of silicone oil is still a matter of ongoing controversy. To evaluate a possible migration of silicone oil into eye tissues we examined various tissues from eyes that were injected with silicone oil for a period of 6 months to 3.5 years. The histological and ultrastructural characteristics of these tissues are reported (cornea, 4 cases; iris, 3 cases; retina, 8 cases; retinal pigment epithelium and optic nerve, 1 case). In the iris and in corneas showing a perforating injury light microscopy revealed vacuoles presumably lying in the cytoplasm of macrophages. Using imm

Focal and diffuse visual field defects: their relationship to intraocular pressure.

The view that visual field loss in "low-tension" glaucoma (LTG) is more focal than in ordinary or "high-tension" glaucoma (HTG) is controversial. The basis of the controversy may be the inappropriate presupposition that an arbitrary intraocular pressure (IOP) level effectively differentiates these entities. We attempted to determine whether meaningful differences exist between glaucoma patients classified on the basis of pattern of visual field loss rather than IOP. One hundred consecutive glaucomatous visual fields were classified as focal or diffuse by two independent observers masked to all

Extracranial spinal accessory nerve injury.

Eighty-three consecutive patients with extracranial accessory nerve injury seen over a 12-year period are reviewed. The most common etiology was iatrogenic injury to the nerve at the time of previous surgery. Such operations were usually minor in nature and often related to lymph node or benign tumor removal. Examination usually distinguished winging due to trapezius weakness from that of serratus anterior palsy. Trapezius weakness was seen in all cases. Sternocleidomastoid weakness was unusual. Patients with accessory palsy were evaluated by both clinical and electromyographic studies. Patien

[Treatment of accessory nerve injury].

From 1975 through 1988, 11 cases with accessory nerve injury were admitted to our hospital. The etiology in 9 of these cases was inadvertent surgical injury during operation. In the other 2 cases it was injured in accidents. The typical clinical manifestations of paralysis of the trapezius muscle and disorder of the shoulder movement were all seen in our cases. The diagnostic features, indications for operation, surgical technics and criteria for efficacy assessment were discussed. The necessity of early operation was stressed, and either neurorrhaphy or neurolysis should be done.

Recovery of the infraorbital nerve after zygomatic complex fractures: a preliminary study of different treatment methods.

The infraorbital nerve (ION) is often involved in trauma to the zygomatic complex (ZC), resulting in sensory disturbance of the area innervated by it. The purpose of the study was to compare the incidence of persistent sensory disturbance after recovery from isolated simple fractures of the ZC, with four treatment methods: 1) closed reduction via subcutaneous approach without fixation; 2) open reduction via subcutaneous approach without fixation; 3) open reduction via oroantral approach and support of the complex by an intraantral Foley catheter; and 4) open reduction via Gillies approach with

Cranial nerve length predicts the risk of delayed facial and trigeminal neuropathies after acoustic tumor stereotactic radiosurgery.

PURPOSE: To test the hypothesis that length of cranial nerve irradiated is a major factor predicting the risk of cranial nerve injury following radiosurgery and to identify any other significant related treatment factors. METHODS AND MATERIALS: Ninety-two patients (93 acoustic tumors) were treated with a 201 source Cobalt-60 gamma unit from 1987 to 1990 and prospectively followed. The range of minimum tumor dose was 12-20 Gy and maximum dose 24-50 Gy. Univariate and multivariate analyses were used to evaluate any correlations between tumor measurements and treatment factors, with the developm

Ocular tilt reaction resulting from vestibuloacoustic nerve surgery.

The ocular tilt reaction (OTR) is a triad of head-eye synkinesis composed of head tilt, conjugate ocular torsion in the direction of head tilt, and skew deviation. The OTR represents a normal compensatory response to lateral head tilts and is produced by activation of the utricle of the lowermost ear. A pathological OTR results when otolith activity is unopposed as the result of injury to the opposite utricle or its nerve. Vertical diplopia may be the only symptom of OTR in patients who have undergone surgery involving the vestibuloacoustic nerve. We report a series of patients with OTR after

Posterior fossa intraoperative monitoring.

Monitoring of brainstem structures is requested of the neurophysiologist for both intra-axial and extra-axial surgeries of the posterior fossa. A variety of techniques to include upper extremities somatosensory evoked potentials, short latency auditory evoked potentials, spontaneous and evoked electromyographic activity, and recordings from the cochlea and the eighth nerve are available. The indication, implementation, and interpretation of each of these modalities are delineated. The efficacy and utility of these techniques are also discussed.

Intraoperative facial nerve monitoring in acoustic neuroma surgery.

Intraoperative facial nerve monitoring simultaneously using electromyography and mechanical pressure sensors is being used in retrosigmoid and translabyrinthine approaches for acoustic neuroma resection. Insulated electrified microsurgical instruments and air drills are used to stimulate the facial nerve with a pulsed, constant current through bone and tumor, before the facial nerve is visually encountered. Electrical stimulation is used to help locate the facial nerve, map the course of the facial nerve within tumor, warn the surgeon of unexpected facial nerve locations, and help predict faci

Tolerance of cranial nerves of the cavernous sinus to radiosurgery.

PURPOSE: Stereotactic radiosurgery is becoming a more accepted treatment option for benign, deep seated intracranial lesions. However, little is known about the effects of large single fractions of radiation on cranial nerves. This study was undertaken to assess the effect of radiosurgery on the cranial nerves of the cavernous sinus. METHODS AND MATERIALS: We examined the tolerance of cranial nerves (II-VI) following radiosurgery for 62 patients (42/62 with meningiomas) treated for lesions within or near the cavernous sinus. Twenty-nine patients were treated with a modified 6 MV linear accele

Fourth nerve palsy: historical review and study of 215 inpatients.

A review of inpatient trochlear nerve pareses diagnosed over 23 years revealed head trauma as the principal cause, with surgical injury, inflammation, and brain tumors seen occasionally. Ischemic (microvascular) neuropathies were rare. About one-half of the patients (52%) had no other neuro-ophthalmologic signs, but only 5% were truly isolated, without other neurologic or ophthalmologic signs or symptoms. Fourth nerve palsies are underdiagnosed on hospital services, where stuporous patients encounter unsuspecting physicians.

Repair of facial nerve after removal of cerebellopontine angle tumors: a comparative study.

The results of repair of 18 facial nerves were examined by means of a modified House-Brackmann grading system. Six were repaired by end-to-end anastomosis and 12 by nerve graft. The reliability of the simplified House-Brackmann grading system was also assessed, using the kappa statistic to analyze the agreement between pairs of observers who examined the function of 40 nerves in 37 patients. Facial nerves studied had been either preserved, repaired or grafted, or divided and treated by faciohypoglossal nerve anastomosis. One nerve was not treated. The grading system proved to be somewhat unrel

Ultrastructural characterization of substance-P-immunoreactive synaptic terminals in the cat's normal and rhizotomized trigeminal subnucleus caudalis.

Deafferenting injuries often cause transient or permanent physiological alterations within the central projection field of affected primary afferent fibers. Aberrant sensory perceptions, dysesthesias, and hyperalgesias represent the clinical sequelae of such injuries; however, the results of experimental deafferentations have been subject to a variety of interpretations (Rodin and Kruger, 1984b). Neurochemical studies show an increased sensitivity of partially deafferented neurons to substance P (SP). Our previous studies (Hoffmann et al., 1991) documented, primarily at the light-microscopic l

Intra-operative monitoring of the facial nerve with an air inflated balloon. Technical note.

A simple, reliable and cheap device for intra-operative monitoring of the facial nerve during surgery for cerebellopontine angle tumours is presented. It consists of a single use tracheostomy tube with a low pressure air inflatable cuff placed in the mouth of the patient on the side of the tumour, connected by a pressure transducer to a monitoring unit. It records any pressure changes in the patient's mouth induced by muscular contractions as a result of excitation of the inferior parts of the facial nerve.

A ring electrode to record extraocular muscle activities during skull base surgery.

A ring-shaped electrode was developed and used in 20 patients to record evoked electromyographic responses directly from the extraocular muscles during skull base surgery. Intra-operative monitoring with this electrode helps the surgeon to localize the nerves that innervate the extraocular muscles precisely and to refrain from disturbing important neural structures during operations. Such monitoring also provides some insight into the pathophysiology of the dysfunction of these nerves resulting from skull base lesions.

[Possibilities and limits of reconstructive surgery of the recurrent laryngeal nerve].

The authors report on possibilities and borderlines of reconstructive microsurgery of the recurrent laryngeal nerve. When paralysis of the vocal cord is proved after thyreoidectomy exploration and control of the nerve should be done immediately. This must be done as fast as possible, latest at the 7th day after thyreoidectomy. If there is found nerve injury caused by intraneural hematoma or compressing sutures microneurosurgical neurolysis and decompression can be done very successful. 8 of 10 neurolysis of the recurrent laryngeal nerve reported by the authors were successful. Because of conse
