Late recovery of function after oculomotor nerve palsy.

We studied three patients who developed oculomotor nerve paresis from different causes. Each patient improved somewhat over several months, after which there was no further improvement for at least six months. Although the pareses were thought to be stable after the period of no improvement, each patient subsequently had further improvement in both motility and alignment with resolution of diplopia in primary position and in more than one of the cardinal positions of gaze. Patients with oculomotor nerve paresis may improve further after an initial period of improvement followed by several mont

Management of trauma of the facial nerve.

The general principles of management of trauma to the facial nerve from a variety of causes are presented and the indications for and timing of surgery are reviewed. Attention is given to management techniques unique to injuries of the extracranial, intratemporal, and intracranial divisions of the nerve.

Traumatic ischaemic optic neuropathy (a case report).

A 22-year-old patient presented with a clinical picture of unilateral optic neuropathy following blunt trauma to the orbit. Clinical findings, fundus fluorescein angiography and neurophysiological studies were suggestive of an ischaemia, a rare etiology.

Vagal paragangliomas: a report of nine cases.

Vagal paragangliomas are rare tumors of neural crest origin: fewer than 175 cases have been reported in the English literature. This slow-growing tumor occurs most often at the base of the skull in the parapharyngeal space, but may arise anywhere along the course of the vagus nerve and its branches. Nine clinical presentations and the surgical outcome in seven patients with vagal paragangliomas treated at our institution are reported. A review of the literature, emphasizing evaluation and treatment of these potentially fatal neoplasms, is discussed. Surgical removal is the treatment of choice

Predicting recovery of facial nerve function following injury from a basilar skull fracture.

Twenty-five patients with posttraumatic facial nerve palsy were studied. Partial recovery of function had occurred in 95% of these patients by 18 months after injury. At 5 months posttrauma, there was some recovery in 92.5% of those with a partial lesion compared with 10% of those with a complete lesion. This difference attains statistical significance. Complete recovery of nerve function had occurred by 10.5 months in 53.5% of the patients; in 62% of patients with a partial lesion, complete recovery had occurred by 4 months compared with 0% in those with a complete lesion. This difference als

Vestibular nerve injury as a complication of microvascular decompression.

We report a case of hemifacial spasm in which hearing was well preserved after microvascular decompression, but the vestibular nerve was injured selectively. We review the pathophysiological mechanisms relevant to this complication in the light of results of animal experimental studies we have conducted. In addition, we discuss the clinical significance of this particular type of cranial nerve injury.

Orbital roof "blow-in" fractures.

Seven patients with significant frontal trauma involving the supraorbital region associated with orbital roof "blow-in" fractures were admitted to the Maryland Institute for Emergency Medical Services Systems (MIEMSS) at the University of Maryland Medical System over a 16-month period. High-resolution computed tomography (HRCT) with three-dimensional reconstructions demonstrated a downward displacement of the orbital roof in the absence of any orbital rim discontinuity. Associated findings included contussive and hemorrhagic injury to the ipsilateral frontal and parietal lobes, proptosis, ptos

[Post-traumatic ptosis].

Traumatic ptosis is most often secondary to a fasciomuscular lesion of the levator system. Less frequently, it results from the paralysis of the levator muscle due to an injury to the third pair of cranial nerves. This ptosis is observed in comminuted orbital fractures or fractures of the base of the skull involving the orbital apex, and it is then associated with oculomotor paralysis. In favorable cases seen early, repairing surgery of the levator system produces excellent results. When associated with ophthalmoplegia or corneal anesthesia, neurogenic ptosis requires a watch and wait policy.

[Proptosis and optic nerve compression caused by unilateral ethmoidal pyocele: a case report].

This paper presents the case of a 29 year-old male with a left ethmoidal pyocele with orbital and intracranial extensions causing proptosis and a temporal visual field defect of the left eye. The patient had a history of nasal injury as a result of traffic accident eleven years ago, and was well until four months prior to his first visit at which time he had an onset of left eye pain, proptosis, and blurring of vision developed following a URI episode. Since then, fluctuation of symptoms was noted with exacerbation when contracting a common cold. The left ethmoidal pyocele was diagnosed via cl

Operative management of acoustic neuromas: the priority of neurologic function over complete resection.

The objective of surgical management of acoustic tumors is to remove them entirely and preserve facial nerve function and hearing when possible. A dilemma arises when it is not possible to remove the entire tumor without incurring additional neurologic deficits. Twenty patients who underwent intentional incomplete surgical removal of an acoustic neuroma to avoid further neurologic deficit were retrospectively reviewed. They were divided into a subtotal group (resection of less than 95% of tumor) and a near-total group (resection of 95% or more of tumor) and were followed yearly with either com

Management of traumatic injuries to the facial nerve.

Penetrating and nonpenetrating trauma to the head and neck produces myriad injuries to the facial nerve. This article presents a rationale of management of neural injury, emphasizing extracranial and intratemporal trauma and techniques of neurorrhaphy.

[Diagnostic approach in diseases of the parapharyngeal space].

Diseases of the parapharyngeal space (p.s.) are quite rare. Nevertheless, the clinical and therapeutic approach to this anatomical site is not easy in many cases. Possibly some of the patients suffering from lesions in the p.s. could be cured, if the disease were detected in an earlier stage. In the present paper the authors introduce the diagnostic evaluation of this region. Clinical and radiological procedures are demonstrated in selected patients with characteristic lesions of the p.s. in order to find out about reasonable and sufficient diagnostic approaches to this hidden area. While the

Facial nerve monitoring in acoustic tumor surgery.

Anatomic and functional preservation of the facial nerve during acoustic tumor surgery remains a primary goal. Intraoperative electromyographic facial nerve monitoring with auditory feedback has enabled the surgeon to more readily achieve this goal. We compared a group of monitored translabyrinthine acoustic tumor removals (N = 89) to a similar unmonitored group (N = 155) in regard to facial nerve function. Function was assessed immediately postoperatively, at time of discharge, and at 1 year postoperatively using the House six-point scale. Results were grouped as satisfactory, intermediate, o

Use of dilute epinephrine as an aid in facial nerve monitoring.

Facial nerve monitoring during otologic and neurotologic procedures has been previously described, and its use is becoming routine. Although these procedures are done under general anesthesia, lidocaine is often used as a vehicle for epinephrine to aid hemostasis during the procedure. The routine use of lidocaine in these preparations presents the theoretical and sometimes real problem of anesthetizing the facial nerve at the start of the procedure, thereby invalidating subsequent attempts at monitoring and stimulation. We present the data from our experience with 74 patients using an epinephr

Isolated 5th to 10th cranial nerve palsy in closed head trauma.

Isolated involvement of the 5th through 10th cranial nerves, in blunt head trauma, without any other neurological deficit, is extremely rare. Two such cases are reported and relevant literature reviewed. The mechanics of trauma involved and the poor recovery is outlined.
