Stridor: intracranial pathology causing postextubation vocal cord paralysis.

During an 18-month period in a pediatric intensive care unit, nine patients with vocal cord paralysis were identified using flexible bronchoscopy. When tracheally extubated, each child was found to have stridor. The children ranged in age from 17 days to 5 1/2 years. Two patients had unilateral paralysis, but neither required tracheostomy. Seven patients displayed bilateral abductor vocal cord paralysis. Of these, six patients required tracheostomy. Surgical injury to the recurrent laryngeal nerve was the probable cause in two patients. The other seven patients had neurologic disorders with do

[Missed and iatrogenic nerve lesions in surgery].

Missed and iatrogenic nerve lesions are not encountered very often. Nevertheless, they represent an increasingly important subject because of their implications for the patient and the physician and their legal aspects. We present an overview of the patients with missed or iatrogenic nerve lesion treated at the Division for Plastic and Reconstructive Surgery of the Inselspital in Berne from 1980 to 1989. After a look at the legal aspects a few typical lesions and their treatments are illustrated. The diagnosis of a missed or iatrogenic nerve lesion differs in no way from that in other nerve le

Tapia's syndrome caused by Paracoccidioidis brasiliensis.

Tapia's syndrome is due to extracranial involvement of the XIIth nerve and the recurrent laryngeal branch of the Xth nerve. There is ipsilateral paralysis of vocal cords, soft palate and tongue. The main causes are parotid and other tumors or injuries to the high neck. We describe here a Brazilian female patient who presented with a lesion in the nasal mucosa, and soon after had dysphagia and dysphonia. Examination revealed paralysis of the soft palate, vocal cords and tongue ipsilaterally. Microscopic examination of the lesion in the nasal mucosa revealed the presence of the fungus. The patie

Efficacy and complications of radiotherapy of anterior visual pathway tumors.

A progressive disturbance in visual acuity or visual field, along with an unexplained optic nerve atrophy, suggests the possibility of a tumor. Tumors that frequently affect the anterior visual pathway include primary optic nerve sheath meningiomas, intracranial meningiomas, optic gliomas, pituitary tumors, and craniopharyngiomas. The location of these tumors sometimes prohibits a complete surgical excision that might jeopardize the visual system. Radiation therapy, however, can be beneficial in these cases. This article reviews the indications for radiotherapy of tumors that involve the anter

[Flash-evoked visual potentials in the early diagnosis of optic nerve injury due to craniofacial fractures].

Impairment or loss of vision due to optic nerve injury occurs in about 10% of patients with cranio-facial fractures. The assessment of optic nerve function is important for decisions regarding optic nerve decompression. But examination of vision and pupillary reflexes may be difficult, especially in uncooperative patients with reduced consciousness and primary disturbances of pupillary functions. In these cases, optic nerve function can be monitored by means of flash-evoked visual potentials elicited by use of a LED-goggle stimulator. VEPs were recorded in ten patients with head injuries compr

Spinal accessory nerve palsy: an unusual complication of coronary artery bypass.

The neurologic complications of coronary artery bypass surgery have been well documented, with a reported incidence of 61% in one large study. Most injuries to the peripheral nervous system involve the brachial plexus. We report the first case of a spinal accessory nerve lesion after coronary bypass surgery. The patient presented with progressive right shoulder weakness. Electrodiagnostic studies revealed a partial lesion of the right spinal accessory nerve. Physical therapy, including strengthening, range of motion, and electric stimulation to the right shoulder, was prescribed to assist reco

Collet-Sicard syndrome resulting from closed head injury: case report.

A 67-year-old man developed paralysis of the right ninth, tenth, eleventh, and twelfth cranial nerves (Collet-Sicard syndrome) after sustaining a closed head injury. Plain x-ray films of the skull revealed two linear fractures of the occipital bone (one of them probably traversing through the right occipital condyle) and a prominent soft-tissue shadow in the region of the nasopharynx, suggestive of a skull base fracture. The patient was managed conservatively. This is the only case of unilateral multiple caudal cranial nerve palsies (IX through XII), seen by us over a 20-year period, from amon

An upper cervical spine injury producing paraesthesia in the distribution of the mandibular division of the trigeminal nerve.

A case is reported of a 37-year-old man who developed paraesthesia in the distribution of the mandibular division of the trigeminal nerve following an injury to the upper cervical spine during a game of rugby. The central pathways of the trigeminal nerve are discussed and it is proposed that mandibular fibres descended beyond their expected level in the spinal trigeminal nucleus, and that it was these fibres which were compressed in the unstable dislocation at the atlanto-axial joint producing the paraesthesia described. Other remote anatomical explanations for the paraesthesia are considered.

Intracranial reconstruction of the facial nerve. Clinical observation.

Nine cases of intracranial facial nerve reconstruction are reviewed in this paper. All patients underwent this procedure for severe injury or disruption of the facial nerve during surgery for acoustic neruroma through the modified extended middle cranial fossa approach (1). Satisfactory recovery of facial function was obtained in 4 patients. Three patients underwent hypoglossal-facial nerve anastomosis 1.3-1.5 years later for no or poor recovery of the facial function. One patient refused any further surgical treatment despite unsatisfactory recovery. The remaining 1 patient, during a telephon

Ultrasonography in optic nerve head avulsion.

The diagnosis of post traumatic optic nerve avulsion is often obscured by the presence of concomitant vitreous haemorrhage. Electrodiagnostic tests, CT scan and fluorescein angiography have not proved helpful in substantiating the diagnosis of this entity in the early stages. We herein present the echographic features in a case of post traumatic optic nerve avulsion that, to the best of our knowledge, have not been previously described. The role of ultrasonography in the diagnosis of suspected optic nerve head avulsion has been high-lighted.

Moebius syndrome with central hypoventilation and brainstem calcification: a case report.

Moebius syndrome (MS) is described in an infant with central hypoventilation and brainstem calcification. The patient had limb defects and bilateral paralysis of the 6th, 7th, 9th, 10th, and 12th cranial nerves. Mechanical ventilation was continued from birth because of shallow spontaneous respiration. Computed tomography revealed brainstem atrophy and four small calcifications restricted to the dorsal portion of the pons and medulla. Prenatal brainstem injury such as ischaemia may have caused MS and central hypoventilation.

Unilateral optic nerve hypoplasia and hydrocephalus in a Pekingese.

A 3-year-old, castrated male Pekingese was examined 2 days after automobile-induced trauma. Multiple pelvic injuries and visual deficits in the right eye were identified. During a subsequent postmortem examination, multiple pelvic fractures were confirmed; however, there was no evidence of head injury. Both globes were bilaterally symmetrical and grossly normal. The intraorbital and intracranial portions of the right optic nerve were threadlike and rudimentary in appearance, while the left optic nerve was grossly normal. Moderate dilatation of the left lateral ventricle was noted. Microscopica

Intra-oral condylotomy for the treatment of temporomandibular joint derangement.

Open condylotomy using Moose's intra-oral approach was performed on 46 patients suffering from anterior disc displacement. The results show that condylotomy is useful in the treatment of this disorder, but the technique used is not recommended due to the risk of dislocation of the condylar fragment.

[Herpes zoster oticus].

In 1985-89, 8 patients with Herpes zoster oticus were examined in the ENT department of the First Leningrad Medical Institute named after I.P. Pavlov. All the patients were treated, taking into consideration the severity of injury of cerebrocranial nerves. It is emphasized that the pathology may have many clinical forms and that it deserves special attention in AIDS diagnosis.

Detecting lower motor neuron dysfunction of the pharynx and larynx with electromyography.

This study assessed the utility of clinical electromyography (EMG) for detecting lower motor neuron (LMN) or upper motor neuron (UMN) dysfunction affecting the intrinsic muscles of the larynx and pharynx. Twenty-nine subjects were examined; their clinical diagnoses included perioperative nerve injury, cerebral infarction, and lateral medullary infarction. Resting activity, motor unit action potential (MUAP) morphology, and MUAP recruitment were evaluated in every case. Medical records (excluding EMG data) were analyzed for clinical evidence of LMN or UMN dysfunction in the intrinsic muscles of

On a case of early-onset post-traumatic facial hemispasm.

In a 49 year old woman an injury to the right frontotemporal region was quickly followed first by paresis and then by spasm of the right side of the face. The early onset of the spasm prompts a critical review of current views on the pathogenesis of facial spasm.

[Technique of thyroid gland operation: prevention of lesions of the superior laryngeal nerve].

The anatomical variant of the external branch of the superior laryngeal nerve is at risk during thyroid surgery. We recommend a careful dissection of the upper thyroid pole strictly avoiding even minor mass ligatures and all vascular structures have to be selectively identified. Of a consecutive series of 161 upper pole dissections the superior laryngeal nerve was found in 31% and no postoperative injury was detected by video stroboscopy.

[Intraoperative brainstem auditory evoked potential in the microvascular decompression of the 5th and 7th cranial nerves].

Microvascular decompression techniques are rational surgical procedures for treating trigeminal neuralgia and facial hemispasm, with results ranging from good to excellent in 90% of cases. Among the most frequent complications recorded in the literature concerning these decompressions, impairment of the facial nerve and auditory nerve account for 5% to 10% due to mechanical traction and/or vascular injury. The peroperative use of early brain-stem evoked potentials (BAEP) was performed on 17 patients in a series of 325 decompressions of the trigeminal nerve, and on 17 patients in a series of 25
