Traumatic intracavernous aneurysm in children: massive epistaxis without ophthalmic signs.

Traumatic intracavernous aneurysms are a distinctly uncommon lesion in children. They usually present with compressive symptoms of the III, IV, V, or VI cranial nerves rather than epistaxis. Epistaxis is rare and usually minor and brief in duration. A child with severe head injury and basal skull fracture, who did not have any preceding symptoms or signs of ophthalmoplegia or exophthalmus, developed massive epistaxis as a leading symptom of the ruptured intracavernous aneurysm. Since there are no reported pediatric cases with fatal epistaxis as a presenting sign, we would like to share our exp

[The neurosurgical treatment of traumatic fronto-basal spinal fluid fistulas. 66 cases (1982-1986)].

Between 1982 and 1986, 66 patients suffering from traumatic frontobasal CSF fistulas were treated neurosurgically i.e. via a transcranial approach. In 68% an additional transcranial space-occupying lesion a direct open endocranium and/or a large basal cerebral hernia were found. The cases and relevant findings are discussed, followed by deliberations on the points enumerated below: --diagnostic, especially neuroradiological advances --problems of indications for surgery --the problem of the best possible surgical approach Both cranial and rhinological approach have their pros and cons. Their i

Numb chin syndrome: a case report.

Neuropathy of the inferior alveolar nerve is common in dental practice. Its cause, when not a result of local anesthetic, is normally from dental disease or trauma. Isolated mental neuropathy (numb chin syndrome) is extremely uncommon, and its most common cause also is dental. The next most common cause is from an underlying neoplasm, and some cases have resulted from systemic disease (eg, multiple sclerosis). Some patients show no evidence of additional disease and experience spontaneous remission of the symptom. Numb chin syndrome cases require coordination of treatment between dentists and

[Myopathy in acromegaly. Report of two cases].

Acromegaly is often associated with neuromuscular disorders. Most of them are caused by compression of nerves with hypertrophic bone and soft tissues or complications of diabetes mellitus. Myopathy has rarely been reported in the Japanese literature. We report two cases with myopathy out of 14 cases of acromegaly. Case 1 is a 62-year-old woman who developed muscle weakness and atrophy in the shoulder girdle, pelvic girdle and femoral regions after a 10-year history of acromegaly. She showed positive Gowers' sign and normal DTRs. Basal growth hormone (GH) level in plasma was 1076 ng/ml. Electro

Carotid-cavernous sinus fistula associated with a primitive trigeminal artery.

Carotid-cavernous sinus fistulas are not rare, but they have never been reported in association with persistent primitive trigeminal artery. We recently encountered such a case. The Jaeger-Hamby procedure was employed, with mandatory occlusion of the primitive trigeminal artery.

[The therapy of zygomatic fractures. Clinical and experimental studies].

The analysis of 120 patients with zygoma fractures showed disturbances of sensibility in the field of the N. infraorbitalis in 84% and pathological statements of eyes in 20%. The re-examinations of 76 of these patients 0.5-4.5 years postoperative showed that more than the half of these patients, who were treated by only the bone-retractor have a redislocation of the zygoma, and 49% of all cases have permanent damages of the N. infraorbitalis. The reason for the damage of the nerve is not only the compression of the nerve for a short time by the dislocated zygoma but also regular present additi

[Computed tomographic assessment of orbital fractures in traumatic damage of the optic nerve].

Ten patients with orbital fractures and optic nerve trauma are reported. Fractures of the optic canal could be demonstrated by computed tomography in six cases and fractures of the orbital apex in another three cases. Surgical decompression of the optic canal was performed in seven cases. Computed tomography enhanced decision for surgery in cases of intraorbital haematoma with exophthalmus and narrowing of the canal by bony fragments, especially in those patients presenting with incomplete or progressive visual disturbance.

[Primary optic nerve decompression in mid-face fractures].

In severe craniofacial-frontobasal injuries the optic nerve is quite often damaged. We report the findings and the visual outcome in 21 patients with severe mid-face fractures, who underwent primary optic nerve decompression after showing an afferent pupillary defect. During the subcranial exploration and the optic nerve decompression, fractures of the optic canal were found in 13 cases; a dislocated bone fragment could be removed in 6 patients. 9 eyes remained blind, but another 9 eyes regained good final visual acuity between 0.5 and 1.0. We conclude that fractures of the optic canal and dis

Bradycardia and the trigeminal nerve.

A review of the literature suggests that the oculocardiac reflex is part of a wider phenomenon involving any structure supplied by the trigeminal nerve. This can lead to potentially fatal complications in maxillofacial surgery. A review of the literature is discussed and two further cases are reported.

[A case of evulsion of the optic nerve].

We examined a 2-year-old child with optic nerve evulsion after facial contusion. The optic nerve head could not be seen because of overlying preretinal and vitreous hemorrhage. Fluorescein angiography demonstrated no infusion of dye into the central retinal artery in the right eye. The electroretinogram was subnormal, and visual evoked potential revealed poor response with right eye stimulation. Ten days later, the fundus could not be seen due to vitreous opacity, so pars plana vitrectomy was performed 62 days after injury and optic nerve evulsion was recognized. This case revealed the possibi

[Disappearance of hemifacial spasm following spinal drainage: a case report].

This report is concerned with a case of right hemifacial spasm (HFS) occurring in a 65-year-old woman, who incidentally had a left internal carotid-ophthalmic artery aneurysm. The aneurysm was successfully treated by clipping its neck. However, spinal drainage had to be performed postoperatively in order to prevent possible cerebrospinal fluid (CSF) rhinorrhea, as the roof of the sphenoid sinus was accidentally opened during the aneurysm surgery. Liquorrhea was observed and stopped on the 5th postoperative day. The drain was then removed only to be reset because CSF rhinorrhea recurred 8 days

Grading facial nerve function: House-Brackmann versus Burres-Fisch methods.

Many systems for reporting results of facial function tests have been proposed, but after five International Facial Nerve Symposia, a perfect system for reporting has yet to be developed. In 1985 the American Academy of Otolaryngology-Head and Neck Surgery adopted the House-Brackmann (H-B) six-point subjective grading scale as a universal standard. This decision was based on the recommendation of the Facial Nerve Disorders Committee. The next year, 1986, Burres and Fisch proposed the Linear Measurement Index (B-F LMI) as an alternative, objective grading system. To determine the ability of eac

Diverse etiologies of facial paralysis in children.

Facial paralysis (FP) in children is most often idiopathic, however, many diverse and identifiable etiologies exist. Twenty-five cases of children admitted consecutively to the Children's National Medical Center over 8.5 years for the evaluation of FP were reviewed retrospectively. In 21 (84%) of the patients the FP was discovered to be secondary to a specific etiology or associated with a recognizable syndrome. In 7 cases, the FP was an initial manifestation of a serious underlying disorder. Causes of the FP in this series include: otitis media, mastoiditis, temporal lobe abscess, osteopetros

Ocular findings in primary hyperoxaluria.

Primary hyperoxaluria (primary oxalosis) is a rare autosomal recessive inborn error of glyoxylate metabolism that causes widespread calcium oxalate crystal deposition in diverse tissues. Because others have reported only occasional ocular involvement, we reviewed the ophthalmologic findings in our 24 patients with primary hyperoxaluria to document its funduscopic variability and to determine its visual prognosis and its possible systemic significance. Eight (30%) of our 24 patients with primary hyperoxaluria exhibited a bilaterally symmetrical retinopathy. The abnormalities were predominantly

Delayed radiation injury to the retrobulbar optic nerves and chiasm. Clinical syndrome and treatment with hyperbaric oxygen and corticosteroids.

Thirteen patients with delayed radiation injury to the optic nerves and chiasm were treated with hyperbaric oxygen (HBO) and corticosteroids. These patients experienced painless, abrupt loss of vision in one (6 patients) or both (7 patients) eyes between 4 and 35 months after receiving radiation doses of at least 4500 cGy to the region of the chiasm. Diagnostic evaluation including neuro-imaging and lumbar puncture showed no recurrent tumor and no other cause for visual loss. No patient's vision improved during treatment or follow-up lasting between 1 and 4 years. There were no serious complic

[Causes of paralysis of the hypoglossal nerve. Apropos of 32 cases].

Thirty-two consecutive cases of hypoglossal nerve palsy (excluding syringomyelia and amyotrophic lateral sclerosis) collected between 1971 and 1987 were reviewed. The XIIth nerve palsy was clinically isolated in 8 cases, associated with other cranial nerve palsies in 16 cases and with long tracts involvement in 8 cases. Seventeen cases were related to tumours. Malignant tumours were predominant, especially middle and posterior fossa bone metastases. Carcinomatous meningitis and brainstem glioma were also found, as well as lymphoproliferative disorders and benign tumours such as chemodectoma an

[The evaluation of intracordal silicone injection in patients with postoperative recurrent nerve paralysis].

This study aimed to evaluate the usefulness and efficacy of intracordal silicone injection in the treatment of cordal paralysis after thoracic surgery or due to tumor invasion of the recurrent nerve. A total of twenty two patients with cordal paralysis was treated with this procedure. The cause of the cordal paralysis was injury to the recurrent nerve incurred during operation for oesophageal or lung cancer in nineteen patients, and direct invasion of malignant neoplasm to the recurrent nerve in the other three patients. Among the nineteen postoperative patients, fifteen underwent silicone inj
