Vocal cord paralysis and superior laryngeal nerve dysfunction in Reye's syndrome.

Bilateral vocal cord paralysis and absent laryngeal sensation are previously unrecognized sequelae for Reye's syndrome, a severe acute encephalopathy in children. Four patients were seen at varying periods ranging from 2 weeks to 5 years after their diagnosis of an episode of severe Reye's syndrome over a 20-year period at the Children's Hospital of Los Angeles (Calif). All of the patients had bilateral true vocal cord paralysis with absent laryngeal sensitivity that was documented at endoscopy. As many as 50% of survivors of severe Reye's syndrome have breathy voices or aphonia. It is suggest

Temporal bone findings in two cases of head injury.

Temporal bone findings in two cases of head injury are reported. In one patient, longitudinal fractures occurred in both temporal bones, and extended to the middle ear bilaterally. Bleeding was seen in the tympanic cavity, mastoid air cells, internal auditory meatus and facial nerve canal. Bleeding was observed in the scala tympani, cochlear aqueduct and endolymphatic sac, but there were no fractures. In the other patient, bleeding was seen in the internal auditory meatus and facial nerve canal. In the right ear, endolymphatic hydrops was observed in all turns of the cochlea.

[Postoperative facial and vestibular nerve palsy: experimental study of its pathophysiological mechanisms].

The 7th and 8th cranial nerves were shifted in the cerebellopontine (CP) angle of dogs by cerebellar retractions that were similar to those performed in humans with monitoring of auditory evoked brainstem responses (ABR). Postoperatively, the vestibular, facial nerves, and brainstem were histologically examined. Caudal-to-rostral shifts of the nerves could induce vestibular and/or facial nerve damages. The most vulnerable portion of the vestibular nerve was located between the vestibular ganglions and the area vestibularis-the most lateral end of the internal auditory canal. This indicated tha

Acquired trigemino-abducens synkinesis.

A 26-year-old woman sustained closed head trauma following a motor vehicle accident. Her injuries included complete trigeminal and abducens nerve palsies. Eleven months later eye muscle surgery succeeded in making her orthophoric in primary gaze although her right abduction deficit persisted. Eighteen months after injury the patient was able to fully abduct her right eye with jaw thrust to the left. With abduction of the right eye there was associated globe retraction and narrowing of the palpebral fissure. This is the second reported case of trigemino-abducens synkinesis. Mechanisms to explai

[Decompression of geniculate ganglion of facial nerve].

Decompression of geniculate ganglion of facial nerve in 20 cases of facial palsy caused by temporal bone fracture, herpes zoster otitic and Bell's palsy was reported. Sixteen cases had been followed up for half to four years. Complete recovery was observed in 13 cases and partial recovery in 3 cases. Transmastoid-attical approach was used. The geniculate ganglion and the distal part of the labyrinthine segment of the facial nerve could be exposed by this approach. The authors emphasized the importance of decompression of the geniculate ganglion, because of the anatomic characteristics and the

Facial nerve recovery following acoustic neuroma surgery.

A series of 76 consecutive patients undergoing unilateral acoustic neuroma excision is reported. Preoperative facial weaknesses and a tumour larger than 2.5 cm were predictive factors of poor facial nerve recovery, re-emphasising the need to detect and remove these tumours at an early stage. Electroneuronography performed around one week after surgery appears to be a good indicator of the likelihood of recovery in facial nerves which are anatomically intact, but which show no function at that stage. Multiple rehabilitative procedures are often required when inadequate function or poor cosmetic

[Surgical treatment of facial palsy].

The results of surgical treatment of 46 cases of facial nerve paralysis from 1974 to 1987 were reported. Among them, facial nerve decompression comprised 36 cases and facial nerve grafting 10 cases. The causes of facial nerve paralysis were head trauma in 13, surgical injury in 22, otitis media in 4, facial neurofibroma in 2 and Bell's palsy in 5. Thirty-six cases were followed up from 0.5 to 6.5 years. Twenty-five cases (69.5%) had totally recovered, 7 cases (19.4%) partially recovered and 4 cases (11.1%) had no recovered. The result of facial nerve decompression was better than that of facia

Acoustic neuroma: a review of 392 cases.

A retrospective study was made of all operations for acoustic tumour over a 40-year period. A total of 392 cases are reported. Trends over time and with the size of the tumour are reported. In the current decade all but two tumours were completely resected, the mortality rate was 1.4%, 94% of patients had a good or excellent result, the facial nerve was preserved in 64% of cases (90% of these with acceptable function) and the cochlear nerve in 13.5% (some with hearing). There is a striking dependence on the size of the tumour of the likely outcome of operation.

Pupillary automatism.

An unusual case of cyclic pupillary movements in an otherwise complete oculomotor nerve palsy in a five year-old girl is reported. This is considered to be due to destruction of somatic and visceral nuclei of the oculomotor nerve following injury to its fascicular part. Pupillary automatism has been explained on the basis of the presence of aberrant autonomic cells in the ciliary ganglion which are discharging in a regular rhythm independent of higher control.

[Problems of lymphangioma of the parotid gland and adjacent tissues].

Two cases of caverno-cystic lymphangioma of parotid gland in children were described. The tumor removal required the discovery of extratemporal part of the facial nerve. In spite of the microsurgery technique the preservation of the thin nerve branch of the 5 months old child was impossible. The reconstruction of the nerve gave only the reinnervation++ of the upper lip. The myoplasty of the orbicular eye muscle by means of a part of the temporal nerve was performed and the rehabilitation procedure was successful. The great dynamic of lymphangioma speaks for an immediate surgery.

[Long-term laryngoscopic follow-up in vocal cord paralysis following struma surgery].

Of 525 patients 17 (3.2%) showed a laryngoscopically established palsy of the recurrent laryngeal nerve after surgery due to goiter. A laryngoscopic follow-up of all these patients, performed at least one year after the operation, revealed that 76.5% of the recurrent nerve palsies were temporary and 23.5% were permanent. Danger of permanent palsy increased in the sequence--uncomplicated nodular goiter--struma maligna--recurrent goiter. The outcome of long-term follow-up showed a palsy rate of 0.8%, which was much lower than the corresponding rate reported by short-term control (p = 0.005). The

Post-traumatic asymptomatic saccular aneurysm of the internal carotid artery.

A patient presented with mild upper airway obstruction after being shot from behind through the neck and soft palate. Two days later he developed palsies of the IXth to XIIth cranial nerves inclusive on the traumatised side. These palsies resolved spontaneously after three weeks. Further investigation revealed the presence of a saccular aneurysm of the internal carotid artery. The internal carotid artery was ligated after gradual closure for three days using a Preston clamp.

The cricothyroid muscle after cricothyroidotomy. A porcine experimental study.

Cricothyroidotomy in elective use or emergency situations has been discussed by several authors during recent years. The main complication reported has been subglottic stenosis. We have used this method for several years and among our patients there has been no case of stenosis. The major complication in our material has been voice dysfunction. Some of the patients had a change in voice quality consistent with dysfunction in the cricothyroid muscles (diminished voice range and a lowered pitch). The aim of this study was to investigate whether cricothyroidotomy and insertion of a tracheostomy t

Intracerebral pneumocephalus after ventriculoperitoneal shunt.

A case of pneumocephalus, secondary to a liquoral shunt operation, is described. The patient had a massive neuroma of cranial mixed nerves occupying the whole region of the cerebello-pontine angle. The diagnosis was possible by changing the position of the patient's head under radioscopic control, while repeatedly applying pressure with the fingers on the shunt reservoir.

[Determining the safety of antral branches of the vagus nerve during selective proximal vagotomy].

A new method of determination of safety of antral branches of the vagus nerves during selective proximal vagotomy based on the potentiometric measurement of the oxidation-reduction potential from the side of the serous membrane on the anterior and posterior walls of the antral portion of the stomach was developed in experiments and approved in the clinic. Results of using the method during operation as well as findings of complex investigation of the motor-evacuatory function of the stomach in the nearest and remote terms after vagotomy show efficiency of the method. The method may be recommen

Establishment of latent ganglionic infection with herpes simplex virus via maxillary gingiva and viral re-activation in vivo after trauma.

Herpes simplex virus can remain latent for months or years in sensory and automatic ganglia of animals and man, and can be re-activated in vivo by several procedures such as neurectomy, irritation of epithelial surfaces, and administration of immunosuppressive agents. The objective of this study was to determine whether dental stimuli can cause re-activation of the latent herpes simplex virus. Homogenization and explanation of ganglia from mice showed that herpes simplex virus (type 1) traveled from maxillary gingiva to trigeminal ganglia, and remained latent. It was also shown that mice passi

[The transoral approach in neurosurgical practice].

Operations were performed on 25 patients by the transoral approach. All of the pathological processes were localized epidurally in the clival, craniocervical or upper cervical regions. These were tumors, developmental anomalies, inflammatory processes, and odontoid fracture with compression of the brain stem, cranial nerves, spinal cord or spinal radices. The advantages of the transoral operation are obvious in decompressing the neural structures from the direction of the exerted pressure. Surgery led to improvement in the 25 primarily operated patients and there was no operative mortality. Am
