Optic nerve evulsion is an uncommon traumatic event, which may result from various orbital or facial injuries. Two patients in whom clear media permitted prompt diagnosis are described. The possible mechanisms are not quite clear, but extreme rotation of the globe seems to play an important role.

The recurrent laryngeal nerve (RLN) is one of the most frequently injured nerves in head and neck surgery. Routine identification of the RLN during thyroid surgery has reduced the injury rate from 10% to less than 4%. Difficulty in identification of the RLN contributes to this surgical morbidity. Devices previously used for intraoperative identification of the RLN have failed to achieve the simplicity and reliability necessary for clinical use. This animal study uses a simple double-ballooned endotracheal tube and pressure transducer system, which assists intraoperative RLN identification thro

Impaction of the sharp medial edge of the orbital plate of the greater sphenoid wing into the orbital apex is a unique type of lateral orbital wall fracture that can produce a potentially reversible optic neuropathy. Two patients in whom the lateral or temporal approach to the orbit was used to reduce this type of fracture will be presented. In both patients, improvement in vision appeared to be related to removal of a bone fragment compressing the optic nerve in the orbital apex.

A 75-year-old woman underwent panendoscopy and excision of a neck "lump". After antagonism of residual neuromuscular block and extubation of the trachea, she developed recurrent upper airway obstruction and stridor, necessitating a permanent tracheostomy. The possibility of vagal nerve involvement or injury should always be considered during excision of neck lumps. Preoperative indirect, atraumatic laryngoscopy by experienced surgeons is essential, as are tracheal intubation and vigilant postoperative observation.

Recent interest in silent angina deals in a sense with a double unknown since the pathogenesis of angina pectoris remains unexplained. In this report, we present evidence from two human postmortem studies and from experiments conducted in eleven awake dogs which supports a hypothesis that angina pectoris may be mediated by an intracardiac chemoreceptor receiving its primary blood supply from the proximal coronary circulation. The clinical events and the postmortem findings in both human subjects supported the hypothesis. The somatic responses observed in the awake dogs resembled those of human

Surgical texts do not clearly advise whether the ramus mandibularis should be identified in excision of the submandibular salivary gland for benign disease. An analysis of 74 patients was undertaken to assess the subsequent nerve function after submandibular salivary gland excision. Both glands were removed in two patients, making the total number of glands excised 76. In five cases the nerve was identified, in five patients the operative record did not clearly state whether the nerve was identified, and in the remaining 66 a non-identification technique was used. The subsequent analysis was u

From February 1981 to January 1984, 20 patients with a tumor of the upper clivus received proton irradiation at the Harvard Cyclotron Laboratory. For 15 patients with known neurovisual status (including visual acuity, color vision, visual field, and fundus examinations) we obtained a cumulative dose-volume histogram (DVH) of the optic nerves (ON) and the optic chiasm. The prescribed tumor doses ranged from 66.6 to 74.4 Cobalt Gray Equivalent (CGE) with a daily fraction size of 1.8 to 2.1 CGE. CGE is used because modulated protons have an RBE of 1.1 compared to 60Co. The follow-up ranged from 3

The results of facial nerve function in twenty-four cases of acoustic neuroma removal using translabyrinthine approach from 1982 to 1987 were reported. Facial nerve had been paralysed in one case preoperatively. The facial nerve function were preserved in 21 out of 23 cases (91.3%). The appearance of facial nerve during surgery was as follows: normally located in 9 cases, displaced and deformed in 12 cases and invaded by the tumor in 3 cases. The size and location of the tumor determine the chance of preservation of facial nerve function. The procedures of manipulating the facial nerve and pre

Two neonates born after traumatic deliveries presented with injuries of the upper brachial plexus and ipsilateral hypoglossal nerve. In addition, 1 patient presented with paresis of the diaphragm after breech delivery; the other patient presented with signs of recurrent laryngeal nerve involvement after vertex delivery. Both infants recovered spontaneously.

85% of abducens palsies recover spontaneously; then their surgical treatment has not to be hastened. However many non regressive cases are frequently observed. Whatever they may be, partial or complete, uni or bilateral, always they give a great functional handicap which warrants a surgical care. The analysis of 74 successive cases allows to reveal some rules concerning the operative plan. In that respect the electro-oculographic recording often provides many essential informations. The recovery of a normal oculo-motor balance is usual in monolateral paresis. In bilateral complete palsies, in

In eighteen patients who had long-standing severe rheumatoid polyarthritis, cranial subluxation of the odontoid process was caused by erosion and collapse of both the occipitocervical and the atlantoaxial facet joints. In five of the patients, the subluxation caused impairment of cranial nerves. One patient was tetraparetic. Six patients had a posterior fusion of the spine; of these, three also had laminectomy of the atlas. Operative treatment seemed to arrest the subluxation, but there was appreciable functional improvement in only four of the six patients. During an average of four years of

An orbital injury can be responsible for compressing the optic nerve in the optic canal. This compression leads to a syndrome of physiological division with homo-lateral blindness and absence of pupillary reaction of light during stimulation of the injured eye, whereas the light reflex is present with contralateral stimulation. Computed tomography studies show the compression site directly: optic canal fracture, or indirectly: hematoma of posterior ethmoidal cells and sphenoidal sinus, fracture of the posterior part of the orbit. This type of injury can be improved by surgical decompression of

The authors report the case of a patient having suffered a section of the oculomotor nerve during the excision of a tumor located in the tentorium incisura. The nerve was immediately sutured (end to end anastomosis). After 18 months, a partial recovery of the function of the nerve was seen clinically and on EMG. Ptosis had disappeared and adduction of the eye reappeared. An analysis of available data on the suture of oculomotor nerves is then given: reparation of the third nerve in animals and man give variable and partial results. Partial recovery and aberrant regeneration are explained by th

Over a period of ten years, 39 patients who had suffered optic nerve compression after a craniocerebral trauma underwent transethmoidal decompression surgery. The operation was performed bilaterally on 5 patients. Fifty percent of patients involved suffered a blunt head or brain injury, the others brain compression or contusion. On the side of optic nerve compression, we found specific signs and symptoms of the compression such as negative or sluggish direct light reaction of the pupil, wounds on the lateral side of the eyebrow, bleeding from the nose, eyelid hematoma, skull fractures and intr
